Job Description

Performing admission, assessment, and discharge functions.
Assessing mental health, substance use, and other status of clients.
Preparing client initial and master treatment plans as appropriate.
Conducting individual, group and/or family therapy as appropriate.
Conducting education and training regarding aspects of substance abuse and severe and persistent mental illness.
Preparing/submitting reports in accordance with state, agency, referral and CARF program requirements.
Obtaining/monitoring funding authorizations from the state and insurance companies.

NOTE: This position will be responsible for running some evening groups.
Qualifications

GENERAL STATEMENT OF DUTIES: Under the direction of the Supervisor of Family Support Services, or designee, the Community Health Worker works as an integral member of behavioral/mental health programs within Genesee Health System.
Work occurs in a variety of venues and locations, including but not limited to schools, healthcare facilities, community agencies, group and individual settings, community events, and canvasing. This individual will use the Community Health Worker model, as well as other evidence based crisis and trauma support methods. Provides education, training, and support to children and families as well as makes referrals for other needed services and supports. Work regular schedule with some night and weekend hours.
