Responsibilities

Participate in patient rehabilitation programs and prioritize patient education based on identified health care needs.

Communicate pertinent information regarding changes in patient status to ensure continuity of care.
Provide direction and supervision to ancillary nursing personnel, utilizing problem-solving techniques.
Ensure a safe work environment and patient handling per hospital policies.

Maintain effective communication with management regarding service needs.
Assume charge responsibilities as needed and pursue professional growth through continuing education.
Required Experience / Certifications / Licensure
Current BLS Certification prior to first shift.
ACLS Certification required before the first shift (specific to certain facilities).
A minimum of two (2) years bedside clinical experience is required.
ICU experience is preferred but not required.
Schedule
This position requires a day shift, working 3x12 hour shifts per week.

Responsibilities
The principal role of the Advanced Practice Provider within AlignMed Partners is to provide primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The AlignMed Partners APP works collaboratively with Center Leadership to achieve the center's mission of improving the lives we touch through the delivery of high quality health care and everyday compassion.
Distinctively, the Palliative Care APP provides palliative care services to nursing center residents with serious illness in collaboration with the primary care team. Emphasis is on the care of the individual patient and family with a focus on pain and symptom management, psychosocial support, and advance care planning as an individual navigates the spectrum of a life threatening illness. The Palliative Care APP serves as a specialized consultant within the interprofessional team who provides recommendations for the plan of care while incorporating patient goals and preferences. Qualifications
The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP).
