    We are seeking a compassionate and experienced Registered Nurse (RN) to join our Advanced Illness Management (AIM) Palliative Care Team. This is a field-based position that provides in-person and telephonic care coordination, education, and assessments to patients with serious or life-limiting illnesses. The AIM Palliative RN will collaborate with an interdisciplinary team including Nurse Practitioners, Social Workers, and Primary Care Providers. This role ensures high-quality, patient-centered care and supports comfort, dignity, and quality of life for patients in private homes, assisted living communities, and skilled nursing facilities.
