Key Responsibilities:
A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!
Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients' needs.
Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
Accepts clinical assignments that are consistent with education and competence to care for patients.
Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
Meets mandatory continuing education requirements of the Agency and licensing board.
Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients' diagnoses and needs.
Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
Requirements:
Current PT license
Knowledge or experience utilizing Homecare Homebase EMR would be a plus
Experience in home health or related healthcare setting preferred
Strong clinical and assessment skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively.

Compensation:

Responsibilities:
Verify, audit, and reconcile all financial records such as room charges, cash drawer activity, credit card transactions, final bill preparation, room charges, and occupancy percentages
Respond to inquiries, process reservations and check-ins, delegate housekeeping, security, and service requests, resolve guest complaints, and other front office duties as required
Perform additional bookkeeping, administrative, and accounting procedures as required
Prepare and distribute daily, weekly, and monthly reports to general manager, department heads, and hotel management
Ensure all guests feel they are having an exceptional experience at the property
Qualifications:
Previous hotel front desk, hospitality, guest services, or night audit experience is preferred, but not required
Must be computer proficient; experience with accounting or reservation software is a bonus
Requires a flexible schedule that allows for overnight shift hours
Excellent customer service and communication skills are required
Diploma or equivalent from high school is required; some college is desired
