Assists and/or prepares patients for surgical procedures, tests, treatments and other diagnostic or therapeutic procedures.

Document all patient assessment elements obtained.

Job Description:
Assist patients in reaching outcomes as established by the RPN in the plan of care. The LPN gathers, and documents data related to physiologic, cognitive, cultural, and spiritual status, assists in goal directed plans of care which are evidence-based and age specific, and implements care through utilization and adherence to established standards which define the structure, process, and desired patient outcomes of the nursing process, and communicates information needing referral to RPN and reinforces teaching plan established by RPN.

The LPN maintains confidentiality of information, demonstrates the knowledge and skills necessary to provide patient care appropriate to the age and special characteristics of the patients served, and performs data collection/documents interventions within protocol or plan of care and objective patient care data on Daily System Assessment.
The LPN may not initiate Plans of care or Protocols, complete 24 hour analysis or End of Shift Summaries, or give end of shift report.
