YOUR ROLE As an Oncology Navigation Nurse Practitioner, you will be a critical clinical team member caring for our members. You will have three primary responsibilities: First, using a telemedicine platform, you will evaluate and manage members identified by our nursing and provider team who have urgent symptoms, complications related to their treatment, or exacerbations of their medical conditions. You will assess presenting symptoms, develop comprehensive care plans, triage to the appropriate level of care, educate patients, and ensure follow-up with relevant primary treating clinicians, including oncology and primary care. You will also complete annual clinical intake visits for select members. Second, you will develop care plans for our members, in collaboration with our onboarding RNs, to address clinical and non-clinical needs and barriers. You will prioritize members' goals and develop concrete plans for our Oncology Nurse Navigators and Community Health Workers to reach these goals. To do this, you will review available medical records, work with external providers, give clinical insights to our care team, and adhere to best documentation practices to drive high-quality care for our members. Third, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members, in which individualized care plans are created and monitored by our team of Community Health Workers and Oncology Nurse Navigators. You will provide clinical oversight to the case management team and direct clinical care, as needed. In addition to these primary responsibilities, you may be asked to participate in some holiday, night and weekend emergency coverage as needed. This role reports to our Nurse Practitioner Team Lead and can be remote or hybrid based in our Nashville or New York City offices. All patient interactions will be virtual via telephone, video, text, or our proprietary virtual care platform. Pay Range: $510 - $600/week plus food and lodging (valued at $200) Dates: end of May- early August (exact dates TBD) ESSENTIAL DUTIES & RESPONSIBILITIES General Responsibilities Work on a team of 2-4 other ranch staff caring for horses and farm animals including but not limited to; feeding, grooming, tacking, general first aid of animals and mucking. Assist Barn Specialist with daily care of various barn animals need as needed. Maintain the cleanliness and organization of all farm/barn/arena/pasture areas. Assist in the care & management of program, horse, and barn equipment, including storage, maintenance, cleanliness, and usage in coordination with fellow team members. Peer Health Assistance Programs. As a national movement during the 1980s, peer health assistance programs developed as an alternative to discipline for professionals with substance use disorders. During euthanasias and emergencies we see clients in delicate emotional states who need our care, support and understanding. Supporting and understanding clients during these times is vital while also caring for their loved (furry) family members. Pay range is $17.00 to 19.00 per hour depending on previous experience. Job benefits are listed below. Paid time off is accrued so all staff receive two weeks (80 hours) PTO per year. Additional unpaid time off is accommodated on a case-by-case basis. Staff receive a $2500 credit to use toward veterinary care of personal animals (or animals of friends and family) annually. Retirement plan is offered with 3% match. Paid holidays include Memorial and Labor day, Independence day, Thanksgiving, Christmas day and New Year's day. Maternity leave is paid 100% for six weeks with additional six weeks unpaid if desired. Most staff prefer to work 4 ten hour shifts so that has traditionally been our scheduling strategy, but we are open to other scheduling options. Meetings: Attend all required staff meetings/retreats (ex: Weekly “Devos & Prayer”, Monthly Staff Luncheons, Annual Retreat, etc.) Work Schedule: Weekends: Must be available to work at least 44 full calendar weekends per year for worship service coverage as well as special services as scheduled (Thanksgiving Eve, Christmas Eve, Easter, etc.). Up to two additional weekends away to “Grow Fruit on Other's Trees” may be requested for outside ministry purposes (retreat, mission trip, training conference, etc.). Wednesdays: Must be available to consistently work Wednesday evenings throughout the school year and during summer programming. Office Hours: In coordination with your Zone Leader, establish and maintain regular weekday office hours considering KM programming and other staffing needs to ensure consistent KM presence in the office. Qualifications: Character: A Christ honoring life marked by personal holiness, prayer, bible study, spiritual and emotional maturity and increasing dependence on the Holy Spirit (Titus 1:6-9, 1 Timothy 3:1-7). Unquestionable integrity specifically in areas of handling finances, relationships and time management. A person who understands and models the Christian principle of servant-leadership. Someone who is eager, flexible and has a teachable spirit. Competency: College degree (preferably at biblical college) with at least 3 years of full-time ministry experience highly preferred. Broad knowledge of childhood development and needs. Pastoral gifting; naturally shepherds with strong gifts in leadership and teaching. Ability to creatively and clearly communicate biblical truths to children in a manner that is fresh, engaging and relevant. Proven experience in team building, envisioning and training people for ministry with a missional attitude and focus. Comfortable in interdenominational and multicultural settings. Effectively communicates with and relates to people of all ages. Willingness to work within a flexible schedule. Takes personal initiative in learning. Able to learn and effectively use the church database system. Solid computer skills with MS Office Suite and Google Suite.
