Overview Good Shepherd Hospice is seeking a compassionate Spiritual Leader to serve as our Chaplain/Bereavement Coordinator in Durant and surrounding communities. Responsible for the assessment of spiritual and religious needs of the patient and family and for providing and/or arranging for the provision of spiritual and religious support with community clergy in accordance with the wishes of the patient and family.
Hospice experience preferred
Responsibilities
Completes spiritual assessment within 5 days of each patient and family admitted to Hospice and writes a care plan appropriate to their needs in a timely manner.
Provides for spiritual and religious support and services in accordance with the wishes and beliefs of the patient and family and in accordance with the plan of care. If the patient and family have their own clergy, priest, rabbi or other religious leader, and if that person has not been informed of the patient's condition, offers to assist patient and family in contacting him or her and advising him or her of the situation in the home.
Serves as liaison to the patient's/family's community of faith. Is responsive to the needs of the patient/family. Advises patient/family's clergy, priest, rabbi or other religious leader of the issues related to the terminal illness of the patient, if desired by patient and/or family.
Documents the provision of care by community clergy and lay persons.
Attends Interdisciplinary Team meetings and participates in the establishment and review of the Plan of Care for patients and families.
Networks with clergy in the community to provide information about hospice.
Participates in developing and enhancing the spiritual and religious support services provided by hospice in collaboration with community clergy and the other members of the Hospice Interdisciplinary Team.
Completes appropriate documentation in a timely manner according to Good Shepherd Hospice policies and procedures.
Offers support to staff with loss and patient/family issues.
Maintains confidentiality of patient/family status
Assumes responsibility as needed for Bereavement Services.
Qualifications
A person who is ordained, commissioned, or credentialed according to the practices of an organized religious group and has completed:
At least 1 year's experience in pastoral care in a medical, educational or religious agency or institution.
Minimum of a two-year degree in the pastoral or other applicable field
Previous experience in pastoral field, preferably healthcare-related
Demonstrated commitment to the hospice philosophy of care.
Able to work weekends, nights, and on call, as needed

Provides spiritual care and emotional support to patients, families, and staff as designated by the Director of Pastoral Care Department. Provides Night on Call (NOC) and Weekend on Call (WOC) coverage as required. Conducts Sunday services at Behavioral Health as needed. Responds promptly to all emergency codes (e.g., Code Blue, Code Green). Responds to requests for services within twenty-four hours. Conducts pre-surgical visits as scheduled by the director and follows up with surgical patients within twenty-four hours after surgery. Assists staff chaplains in conducting support groups for clinical staff when required. Maintains open communication with the interdisciplinary team and cultivates professional relationships with all hospital staff members. Provides devotional services throughout the hospital as needed and leads Chapel Services when required. Maintains cooperative working relationships with local clergy and informs the patient's clergy of the patient's request for pastoral visits when requested. Assists staff chaplains in educating patients, families, staff, and the local community through seminars, in-service sessions, support groups, and individual pastoral counseling. Assists staff chaplains in planning and executing Critical Incident Stress Debriefing sessions when needed. Other duties as assigned.

On-call whenever not on-site, providing emergency coverage.
Perform clerical duties as needed.
Ecclesiastical endorsement by a faith group
Must have strong interpersonal and written/oral skills to interact effectively with patients, patient's families, visitors, hospital and medical staff

Provide direct spiritual care to patients, loved ones, and caregivers by carrying a regular caseload, completing spiritual assessments, and implementing individualized Plans of Care.
Accompany patients and their families through the end-of-life journey by providing compassionate presence, counseling, prayer (when appropriate), and emotional and spiritual support.
Document all assessments, interventions, and progress notes in accordance with agency policy and federal regulations.
Interdisciplinary Team Collaboration
Attend and actively participate in Interdisciplinary Team (IDT) meetings and Care Plan meetings.
Collaborate with nurses, social workers, aides, and other team members to address holistic patient needs.
Coordinate spiritual resources by collaborating with community clergy, faith leaders, and spiritual providers with whom patients or families already have relationships.
Program & Ministry Support
Assist with and participate in:
Funerals and memorial services
Annual hospice memorial services
Patient birthday recognitions
Veteran recognition and commemorative services
Facility-based memorials as appropriate
Participate in the orientation and support of new hospice personnel.
Actively participate in Quality Assurance and Performance Improvement (QAPI) activities and initiatives.
On-Call Responsibilities
Rotate on-call coverage for pastoral care and postmortem support during after-hours, weekends, and holidays, in collaboration with other chaplains.
Cover 1-2 weeknights (4:30 PM to 8:30 AM) and 1-2 weekend days (24 hours, 8:30 AM to 8:30 AM) on a rotational basis.
Be available by phone to attend postmortem situations at home or in facilities within the greater Kansas City metro area.
Provide spiritual support during postmortem visits when requested by on-call clinical staff.
Professional Development
Maintain clinical competency and remain current with hospice regulations, documentation standards, and best practices in spiritual care.
Demonstrate cultural humility and sensitivity to diverse spiritual, religious, and cultural traditions.
Other Duties
Perform other duties as assigned to support the mission and needs of the hospice program.

Qualifications:
Previous experience in providing spiritual care and bereavement support.
Ability to respond compassionately and professionally to the needs of patients and their families.

Essential Job Functions:
Provides appropriate spiritual care to the residents and their family members as well as to the employees of The Wesleyan.
Provides spiritual care and support to individuals of all faiths or no faith.
Regularly performs religious services, in an ecumenical spirit, at all community buildings and with individuals of all care levels. This will require an understanding of the various different communication levels related to Aging, Dementia and various theological frameworks.
Write and deliver sermons within a traditional liturgical format.
Administers the sacrament of Holy Communion under the supervision of the Director of Spiritual Life.
Leads study classes and assists in the recruitment of teachers and visiting choirs as needed from among residents and area churches.
Attends and participates in The Wesleyan's bi-annual memorial services and assists the Director of Spiritual Life in the planning of individual memorial services for residents and their families.
Complete hospital visitations of residents as needed and required and in a timely fashion.
Upholds the values and beliefs of the Wesleyan Board of Trustees and serves to meet the mission of The Wesleyan.
Qualifications & Experience:
Must be a current, active ordained minister, deacon or elder (priest/pastor), preferably of the United Methodist Church is required. A minister of another denomination will be considered if the candidate does not have strong doctrinal differences from the United Methodist Church and articles of religion and can preside at an open communion table.
Must be of good moral and ethical character and in good mental and physical condition.
Must have a strong understanding or willingness to learn about issues related to Aging, Dementia, Senior Health Care, Home Health, Hospice and Grief.
Must be willing and able to cooperate with regulatory agencies, board members, administrators, other team leaders, fellow employees and residents.
Strong oral and written communication skills are required as well as the ability to document resident/client/staff interaction in a timely and clear fashion.
Must be able to lift, move, push and manipulate at least 30 lbs. including the ability to push and move wheelchairs, rollators and Geri-chairs on both carpeted and tiled floors.
Must possess leadership ability.
Must have patience, tact, a cheerful disposition and enthusiasm, and be able to handle residents and family members based on whatever maturity or cognitive level at which they are currently functioning.
Must be available on Sunday mornings and on-call at least one week each month.
Travel throughout the surrounding area is required.
A valid Texas driver's license is required.
