Good Shepherd Hospice is seeking a compassionate prover performer for the role of RN Regional Quality Assurance. The Regional Quality Assurance Nurse is the primary resource for compliance in assigned locations. Works closely with the Compliance Officer.The Regional Quality Assurance Nurse ensures that the quality of care for patients is assured and that the client's/caregiver's expectations are met in the clinical care areas. In addition, the Regional Quality Assurance Nurse is responsible for the followingresponsibilities.

Responsibilities
* Assists the Compliance Officer in conducting monthly reviews at assigned locations to ensure compliance and assumes primary responsibility for formulation and execution of an action plan to correct findings.
* Works directly with clinical staff in assigned locations to ensure quality-nursing care according to the established plan of care in order to maximize the comfort as well as the health of the patient and their families through mentorship and education.

Qualifications
* Licensed as a Registered Nurse in the state of employment.
* A minimum of two years experience in hospice, and previous clinical experience in home health, long-term care, oncology or geriatric nursing. Utilization review, compliance, QI experience preferred.
* Previous experience in dealing with client organizations and management functions.
* Knowledge of Nursing theory and practice

Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator Preceptor or in collaboration with other Registered Nurses.
Performs ongoing systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record makes adjustments or modifications to treatment plan as indicated and notifies Team Leader Charge Nurse Supervisor or Physician as needed.
Assesses collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
Directs and provides in collaboration with direct and ancillary patient care staff all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients.
Administers medications as prescribed or in accordance with approved algorithm(s) and documents appropriate medical justification and effectiveness.
Initiates or assists with emergency response measures.
Serves as a resource for health care team participates in staff training and orientation of new staff as assigned.
Ensures correct laboratory collection processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
Identifies expected outcomes documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.

EXPERIENCE AND REQUIRED SKILLS:

The Hospice RN provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the Interdisciplinary Plan of Care.

Responsibilities
* Receives patient assignments from the Patient Care Manager or Case Manager. Initiates communication with attending physicians, hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
* Cultivates communication and bereavement skills.
* Maintains regular communication with the attending physician concerning patient/family care.
* Contacts families of new patient assignment, maintains regular communication as requested or desired by family.
* Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the Plan of Care.
* Maintains up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur. Turn paperwork into office as required by Medicare and Good Shepherd Hospice policies and procedures.

* Shares in providing 24-hour, seven days a week coverage to patients and families.
* Evaluates hospice aides every two weeks.
* Evaluate LPN at a minimum of one time per month.
* Through individualized studies, learn and develop understanding of hospice philosophy.
* Become proficient in pain management through individualized reading, in services or seminars.
