Integrated Behavioral Health Counselors work to increase access of behavioral health services. This position is eligible for a $4,000 hiring bonus. The selected candidate will receive half of the bonus on their first paycheck and the remaining half on the paycheck following their six-month anniversary. Both payments must be made prior to June 30, 2026, as the bonus is funded by a grant. Work is performed at a Federally Qualified Health Center located at 1132 SW 13th Ave, Portland, OR 97233 , and potentially at other locations as needed. Clinical supervision is provided. Essential Duties Provide behavioral health assessment and brief, solution-focused treatment for clients with medical illness, chronic disease, mental health and/or substance abuse concerns in a primary care Use brief interventions to educate and help clients build skills to better manage sometimes complex health concerns Provide care to patients of diverse ages including pediatric patients Coordinate care with other systems and make referrals to services. Conduct assessment and diagnosis of mental health and substance use disorders Support patient's in accessing gender affirming care, including writing letters for gender affirming surgeries Maintain accurate and timely documentation for all services provided. Consult with interdisciplinary primary care teams to provide education regarding psychosocial treatments Provide crisis intervention as needed, including assessment and appropriate referrals. Assess client need and suitability for access to in-house psych medication management, refer to community mental health as appropriate Collaborate and provide support to members of Integrated Behavioral Health Team including student interns and peer support specialists Qualifications Knowledge and Skills Knowledge Experience working with clients and staff from diverse racial, ethnic, cultural, socioeconomic, sexual and gender identity backgrounds. Documentation and Compliance * Clinical Documentation: Ensure all psychiatric assessments, medication management notes, and missed appointment notes are accurately documented and submitted to the Program Director (or designee) by the end of each day. * Prescription Logistics: Efficiently utilize the e-prescribing system to submit and transmit prescriptions directly to the appropriate pharmacy. * Confidentiality: Maintain the strict confidentiality of all program files and evaluation documents. * Program Oversight: Oversee and ensure full compliance with all mandated protocols (e.g., HIV testing, harm reduction services) and funder requirements. * Licensure and Waivers: Maintain all required regulatory documents, including essential waivers, licenses, and registrations (e.g., CLIA waiver, temporary authorizations). * Logistics and Inventory: Oversee the distribution of syringes and Narcan, and maintain accurate, up-to-date inventory logs for all supplies. * Reporting: Collect and submit all required monthly and quarterly reports/deliverables to the Program Director in the specified format and timeframe. 3. Team Collaboration and Professional Development * Team Meetings: Attend required weekly team meetings (including specific 9:00 AM meetings) to review caseloads, bed rosters, and discuss program operations. * Case Coordination: Participate in case conferences with the Case Manager and Program Director to ensure seamless, coordinated care. * Staff Training: Ensure all program staff are properly trained on current program policies, procedures, and compliance standards. * Professional Development: Participate in all mandatory training and meetings as directed by funding agencies and/or the Program Director. * Community Engagement: Engage in and support community events and outreach activities to advance program goals. Qualifications: Required Education & Licensure * Active and unrestricted Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) issued by the New York State Education Department (NYSED). * Must possess prescriptive authority for all necessary medications within the scope of practice, including the ability to be designated in the program's registration for prescribing Naloxone (opioid overdose reversal medication). * Current and valid DEA registration. Essential Clinical Expertise * Proven experience providing psychiatric care to adults and young adults with Serious and Persistent Mental Illness (SPMI), including those who are unhoused (homeless) and those with active Substance Use Disorders (SUDs). * Expertise in utilizing integrated treatment models for co-occurring mental health and substance use disorders (Dual Diagnosis). This includes proficiency in evidence-based practices such as: * Motivational Interviewing (MI) * Cognitive Behavioral Therapy (CBT) * Trauma-Informed Care (TIC) principles * Demonstrated ability to perform the following core functions: * Conduct comprehensive psychiatric evaluations. * Provide expert medication management, including Pharmacotherapy for SUDs (e.g., Medication-Assisted Treatment - MAT). * Perform thorough risk assessments (suicide, violence, relapse). * Ability and willingness to provide psychiatric services in diverse community settings, including on a mobile outreach unit and other non-traditional environments.     Job Description General Purpose: The Behavioral Health Counselor is working with a PROS program to conduct individual and group services. Personalized Recovery Oriented Services (PROS) is a comprehensive model that integrates rehabilitation, treatment, and support services for people with serious mental illness and possible other co-occurring disorders. The Behavioral Health counselor works with PROS participants towards goals in different areas: Living independently Building natural supports Finding and keeping a job Reaching higher levels of education Securing preferred housing Improving medication management This will be carried out as indicated by NYS OMH Part 512 Regulations and Medicaid requirements. Essential Functions: Manage a caseload of up to 25 consumers, including case management, collateral contacts, referrals, vocational assistance, and other needs, to coordinate services. Conduct required assessments including Wellness Recovery Action Plans, Psycho-socials, Psychiatric Rehabilitation, and Substance Use screenings and assessment. Develop Individualized Recovery Plans (IRP) in collaboration with participants, on a bi-annual basis, or more often as necessary. Maintain and update demographic, treatment and employment data in electronic health records (Foothold AWARDS, OMH CAIRS database, and NYESS). Facilitate member access to community support services including medical, mental health, substance abuse, housing and entitlements. Develop and facilitate approximately 6 -8 groups, according to protocol and curriculum, which will cover CRS, ORS, IR and Clinical Services as dictated. Conduct individual counseling sessions to assist program participants with attainment of the life role goals specified in their IRPs. Document consumers' progress towards goals in monthly progress notes and determine the need for addition or subtraction of services offered to each participant on an ongoing basis. Coordinate treatment including the sharing of Individual Recovery Plans and Progress Notes for participants who are co-enrolled with other agencies as necessary. Provide services according to Evidence Based Practices including Wellness Self Management, Family Psychoeducation (CCFC), Medication Management, Co-occurring Disorders (IDDT), and Supported Employment (IPS). Coordinate follow-up services once a participant is ready for discharge from the program. Act as a member of an interdisciplinary treatment team, including participation in weekly administrative, clinical and utilization review meetings. Assume overall responsibility for maintenance of client records, including timely documentation of services in Electronic Health Record (AWARDS), as required by Medicaid and the Office of Mental Health. Develop and maintain collateral relationships with consumers' other providers and family as clinically indicated Carry a 24-hour support cell phone in rotation with the clinical team. Other duties as assigned. Qualifications/Basic Job Requirements: . A minimum of a Bachelor's Degree in Psychology, Social Work, Human Development and related field, 3+ years of experience working with adults with mental illness and/or families dealing with mental illness. Masters Degree preferred. Team-based skills are essential. Previous experience working with individuals with psychiatric/co-occurring disorders preferred. Knowledge of running groups preferred. Computer skills and knowledge of service documentation required.
