Job Title: Mental Health Therapist - Close to Home Non-Secure Placement Aftercare Program
Reports To: Aftercare Supervisor
Hours: 35 Hours/Week; (Some Evenings Required. Position requires to staff to be on site. Hybrid schedules based on program needs & at leadership discretion)
Salary Range: 61k-72K based on relevant post-education, experience, and NYS License.
Job Summary: The NSP Aftercare program provides support to youth and their families following the youth s release from residential placement back into their communities. The Therapist provides short-term individual child and/or family therapy for youth and families in the aftercare program to promote successful adjustment back to the community. This position provides comprehensive assessment and treatment to address identified personal, behavioral, or family issues, utilizing a strength-based approach that focuses on youth and family development. All work is trauma-informed and ensures that the Sanctuary 7 commitments are embedded into practice, and that the tools of this model are utilized when appropriate to address the trauma symptoms of the population. The therapist provides Solution-focused, brief therapy, in-home, strength-based, comprehensive services to families whose children have been involved in the Juvenile Justice System and are back or transitioning back into the community.
Major Duties:
Administer individual mental health assessments, including but not limited to the Trauma Symptom Checklist for Children.
Provide solution-focused brief therapy, individual, family, and group counseling, as well as short-term crisis intervention, including advocacy and outreach for designated caseload using agreed-upon models, including CBT/TF-CBT.
Empower families through impactful therapeutic interventions and insightful assessments delivered flexibly via in-office sessions, telehealth, and personalized home visits.
Drive meaningful transformation in family functioning through dynamic therapeutic services and insightful assessments offered in-office, via telehealth, and directly in the home to meet families where they thrive.
Complete bi-annual treatment plans, psycho-social assessments, and trauma-based assessments on each assigned youth.
Facilitate and/or participate in psycho-educational groups.
Help and/or support youth through mental health crises.
Facilitate long-term referrals to alcohol, substance abuse, and mental health treatment facilities and other service providers, including escorting to appointments as needed.
Participate in Administration for Children Services (ACS) conferences, including Elevated Risk & Transition Conferences.
Participate in Treatment Team meetings to collaborate with Treatment team members to develop a discharge plan for the youth.
Provide youth and families with support to utilize and/or practice effective coping, communication, and self-care skills to promote for the de-escalation of crisis and processing of crisis situations.
Participate in Aftercare staff/team meetings and trainings.
Complete necessary documentation and maintain appropriate utilization of databases (Connections and Evolv),
Learn and support youth in the Aftercare program to apply the Missouri Approach as they transition back to their natural groups.
Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care, when applicable, in the department/program to create a non-violent and trauma-sensitive culture.
Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss, and Future) framework into daily practice to ensure quality care and an effective work environment.
Ability and willingness to be trained in and utilize Safety Crisis Management ( SCM) skills and techniques.
Perform other duties as assigned.
Qualifications:
Master's Degree in Social Work Or Mental Health Counselling Required.
NYS LMSW or NYS LMHC Required.
Bilingual (English/ Spanish/Creole) preferred.
Willingness to work in the community and provide in-home therapy to families.
Ability to use public transportation throughout the city.
Valid Driver's license strongly preferred.
Experience with in-home family therapy strongly preferred.
Must be willing to work flexible hours, including Saturday schedule if needed.
Excellent organizational, written, and communication skills.
Must have a commitment to work from a restorative, strength-based, trauma-informed, and youth development perspective.

About the Position
The Prevention Brief Strategic Family Therapist works as part of a prevention team to promote child safety and well-being, reduce risks to children and youth, and prevent foster care placement. This intervention employs a team approach between the therapist, a case planner, the program supervisor and the model developer to engage the entire family into the intervention, while simultaneously remaining attuned to environmental and familial safety and risk issues. Program will offer 8 clinical hours.
Duties & Responsibilities
1. Develops and maintains therapeutic relationships with all family members.
2. Continuously assess for safety and risk issues.
3. Conducts weekly sessions with the entire family using Brief Strategic Family Therapy, predominantly in the home.
4. Provides services to a caseload of 10 families.

Job Functions:

The Residential Treatment Center serves adolescents placed through the Child Welfare, School and Juvenile Justice systems. About the Program
The Family Treatment/ Rehabilitation Program (FTR) located both in Manhattan and Brooklyn offers intensive preventive case management services to families at risk of removal of children to foster care due to neglect and abuse. Risk factors for families served by the FTR program are sometimes advanced as a result of caretakers' drug use and/ or mental illness. The stressors of poverty, substance use, instability and criminal justice involvement, parental depression, family stress, substance use or domestic violence can greatly compromise a parent's capacity to care for her children, despite strong desires to be engaged. Using Solution Based Casework, we actively work with parents to help them develop a more active, consistent and productive role in their child's educational and social development; we encourage families to build connections to positive supports in their community who will help them sustain their progress. Position Overview:
The Licensed Clinician will provide clinical assessments of client disorders and use an eclectic approach of engagement and service delivery. The clinician will also support the case planning staff in the implementation of case specific goals of families; will be part of a multi-method program and has a consistent strong approach to partnering with families to promote the safety and wellbeing of all children; ensure that our families and staff have access to high-quality resources to meet their needs.
Duties and Responsibilities (but not limited to):
Participate in Clinical Diagnostic Team (CDT) meetings and ensure that clinical decisions meet best case practice standards as well as contractual requirements.

Qualifications:
Master's degree in Social Work, Social Services or a related field
Mental Health License and LCSW with a minimum of 2 years of experience working with adults and children with mental health illness is preferred
Mental Health License and LMSW with a minimum of 5 years of experience in working with a mental illness
CASAC certification and minimum three years experience working with substance abuse parents and their children is preferred
Must possess excellent assessment skills
Experience providing mental health counseling to adults, adolescents and children
Outstanding interpersonal and communications skills, both oral and written

Job Functions:
Provide individual, group, family, and collateral treatment and crisis services/interventions that are focused and goal oriented. Offer information and counseling on the best course of action during sessions.

The Sociotherapist Supervisor ensures the individualized behavioral and emotional needs of children and youth are addressed on a weekly basis for the purpose of improving the childs/youths daily functioning in the home, school and community. The Juvenile Justice Initiative Psychiatric Program, with SCO Family of Services consists of a small team of mental health professionals. The MST-Psych team provides mental health treatment as well as family therapy to families and youth living in the Bronx and Manhattan. Our mission is focused on helping youth and their families gain the necessary tools and skills to reduce hospitalizations and further court involvement while strengthening the family system. This community-based clinician provides in home counseling to adjudicated youth and their families 2-3 times per week. Youth referred to the program range from 9 to 17 years old, have moderate to serious mental health challenges, and live in the Bronx.
The therapist uses evidenced based clinical practices to empower youth and families by creating strength-based behavior change that will be sustained long after treatment ends. The MST team strives to keep youths out of the system and keep families together.
Field work comprises the majority of the therapist's time, but additional administrative requirements and supervision are an equally important component of the position. MST therapists are clinically trained in MST and carry a small caseload of approximately 4 to 5 families. The ideal candidate for this position is a clinician grounded in strength-based practices and a social justice advocate.
SPECIFIC RESPONSIBILITIES:
Provide in home family counseling, support and crisis intervention to a small case load of 5 families. Treatment is provided in-person and in the family's home.
Conduct MST assessments and adhere to evidenced based clinical counseling modalities.
Design cognitive behavioral interventions for youth and families.
Collaborate with stakeholders such as schools, DOP, and pro-social programs.
Advocate for families by attending court, hospital and other community-based meetings.
Update stakeholders with treatment progress.
Participate in weekly group supervision and consultation, as well as individual supervision.
Complete all required documentation.
Perform all other duties as assigned.
On call support to assigned MST families.
QUALIFICATIONS:
Must be fluent in Spanish.
Previous experience with family therapy, or in home counseling, to youth and their families.
Flexible work schedule. Some evening and occasional weekend work is required.
EDUCATION:
Master's degree required. MSW, MFT, MHC or related degree preferred.

NYS OMH licensed outpatient mental health clinic in Dix Hills, Suffolk County is seeking clinician to work minimum of 12 scheduled hours, combining evenings and/or Saturday hours. Clientele includes children, adolescents, adults and families.
Depending on credential and type of service provided, reimbursement ranges from $25- $50 per appointment. Sessions conducted in Spanish provide additional compensation.
As a Therapist at our Clinic, you will be responsible for:
Carry a clinical case load that will include providing individual, family, collateral and group treatment and crisis planning.
Design and implement treatment plans for assigned cases.
Provide direct treatment to clients in accord with clinical privileges.
Involve significant others in the course of treatment.
Schedule and conduct Intakes including all assessment paperwork for pre-admissions screening (PAS).
Complete daily charge tickets for all appointments.
Prepare referral material for clients to secure services from other community resources, such as SPOA services, housing, case management, vocation placement and/or otherwise.
Participate at staff meetings, in-service training programs and consultation with team members.
