Job Summary: Under the direct supervision of the Director of Safety and Security, the Security Officer will be responsible for performing the security and surveillance duties at Goodwin House Bailey's Crossroads. The Security Officer will provide monitoring and maintenance of the perimeter security, building entrance and egress areas used by Goodwin Living staff and residents, as well as the parking facilities, patrol the interior of the building perimeter, maintain safety and security standards under emergency conditions, communicate orally and in writing, complete reports of security operations, perform non-routine assignments, and non-urgent maintenance duties after maintenance hours. The pay for this position is $22/hr Job Duties: * Monitoring of security stations, operating and maintaining security equipment. * Provision of security services through enforcement of Goodwin House Bailey's Crossroads policies and procedures. * Tasks to be completed by Security Officer. C1. Provide security services as directed for any special events, including setting up of "No Parking" signs, conducting traffic, and parking, etc. C2. Assist in delivering / escorting emergency vehicles to building entrance where needed (when resident needs to be transported to Hospital). C3. Understand the Fire System to perform the following operations when system sounds: a. Reset system. b. Report supervisory troubles on the system to Maintenance Staff c. Restore / secure any activated pull stations * Provision of emergency and other services on weekends, holidays, and at other times when staff who normally perform these duties are not present. * Patrolling the interior of the secure perimeter and inspection of the facility. * Communication (oral and written and record-keeping) for Goodwin House Bailey's Crossroads security operations. Qualifications: * Previous Security experience required * Demonstrated knowledge of standard security practices * Demonstrated ability to operate telephone and surveillance systems * Demonstrated ability/technique for sound decision making in emergency situations * Clear and concise written and verbal communication skills for observation and documentation of incidents / investigations * Physical ability to lift 50 lbs. Planning innovative programs based on the six dimensions of wellness to develop and maintain cognitive functioning, enhance creativity, promote self-confidence, encourage socialization, and foster community involvement. Position is required to assist with applicable regulated documentation (assessments, progress notes, MDS, attendance notes on all programs). Key Duties Develops and coordinates a therapeutic recreation program of daily group activities (inclusive of all functional levels), and individual interactions which includes 1:1 visits, outside excursion planning, concerts, educational lifelong-learning classes, social gatherings, and elevated events. Monthly programming calendar drafting and oversight. Assistance with completion of attendance documentation and other documentation within time requirements. Monitors resident progress/recreation goals, interests, and needs to adjust as necessary. Remains abreast of advances and/or changes in field, and keeps supervisor informed of such. Orients and supervises volunteers by providing direction, instruction, and encouragement. Attends and participates in Plan of Care conferences and other relevant meetings as requested from Assisted Living Administrator. Job Requirements Certification as a Certified Therapeutic Recreation Specialist (CTRS) preferred. Minimum of two years' previous experience in recreation programming for older adults. About the Position The Physical Therapist evaluates patients, under physician referral, with decline in ability to perform functional tasks, such as gait and transfers, impaired safety level, pain, and impaired quality of life due to illness, injury, and disease processes. Develops effective treatment plans and goals, obtaining approval for services from the referring physician. Involves the patient and caregivers in the development of the Plan of Care. Communicates and teaches families and other health team members to improve outcomes and follow-through with exercise and gait programs and functional tasks. All tasks are performed in a home health setting, with assessment of eligibility of homebound status performed each visit. Job Duties * Provide comprehensive Physical Therapy evaluations and treatments, under referral from the attending physician, to overcome specific losses of function, impaired safety, and pain. * Open cases, completing Oasis assessment, if on intake the patient is assessed as having no nursing needs. * Evaluate the home environment and make appropriate recommendations. * Consult with DON and nursing staff, after Oasis assessment and Physical Therapy evaluation, on needs for additional services, such as nursing, home health aide, occupational therapy, or speech therapy. * Effectively communicate with the referring physician and nursing staff concerning changes in the patient's condition, and necessary changes to the initial Plan of Care due to such changes. * Coordinate treatment with nursing staff, home health aides, occupational therapists, and speech therapists, as necessary. * Document evaluations, treatment sessions and services provided in the home according to agency protocols and Medicare guidelines, completing all documentation in a timely and effective manner for both communication with other health professionals and billing purposes. * Effectively involve the patient and caregivers in the development of the Plan of Care, and any changes indicated to the Plan of Care due to a change in condition. * Provide education to patient and caregivers through effective teaching methods in home exercises, safety with functional activities, and carryover of other therapeutic techniques as appropriate. * Provide supervision of Licensed Physical Therapy Assistants (LPTAs) involved in the patient's care according to guidelines for supervision established by the Commonwealth of Virginia state practice act and Medicare guidelines. * Participate in teaching of LPTAs through required joint visits and as needed in patient specific procedures and treatment techniques. * Maintains privacy and confidentiality of patient information at all times. * Implement infection control measures per agency protocols and policies. * Use equipment and supplies effectively and efficiently. * Participate in case conferences and discharge planning per agency protocols. * Complete Oasis recertification and discharge assessments if indicated. * Attend at least 4 staff meetings per year. * Attend agency mandatory in-services and maintains 100% compliance with mandatory data based training. * Participate in implementing systems for evaluating the home health agency's delivery of services to maintain the highest quality of patient care and satisfaction with services possible. Job Requirements * Degree in Physical Therapy from an APTA approved and accredited college or university. * Current licensure as a Physical Therapist in the Commonwealth of Virginia. * CPR certification (adult only) * Passes an assessment of competencies prior to providing patient care, with any unmet areas addressed through education and mentoring prior to participation in patient evaluations and care. * Possesses the ability to deal tactfully and compassionately with patients, family members, and caregivers. * Has the ability to motivate patients to participate as fully as possible in their rehabilitation programs. * Possesses the ability to make independent decisions. * Professional dress, demeanor, and presentation. * Has the ability to move intermittently throughout the workday, lift, push, pull, and move equipment, supplies, etc. up to 50 lbs. getting assistance when needed is a must. * Possesses enthusiasm, flexibility, and the ability to adapt to the changing healthcare work environment. * Possess a Virginia Driver's license, proof of automobile insurance and readily available, reliable transportation. Key Duties Prepares schedules for the Nurses, Registered Medication Aides (RMAs), Certified Nursing Assistants (CNAs) on all shifts for the Small House Health Care Center, Assisted Living and the Clinic. Keep leadership informed of staffing and overtime trends, making recommendations for ongoing improvement. Maintains daily analysis of the H&W Department staffing needs, arranges 24-hour coverage for all three shifts, and posts a notice of available shifts, utilizing regular staff and PRN staff. Adjusts monthly schedules posted for RNs, LPNs, RMAs and CNAs to cover for unexpected absences due to illness, emergencies, etc. Centralize payroll coordination across health and wellness including timecard review and approval, time off requests. Escalate special requests and issues to leadership or human resources as appropriate. Coordinate annual evaluations for all team members. Leads the timely submission of payroll and HR process forms, e.g., Personnel Action Request Forms (PARFs), status change memos, etc. Reports to Administrator or designee when the need arises for additional or replacement personnel. Confers with and assists employees with scheduling problems related to work; serves as liaison between employees and the Administrator and refers employees to the Administrator as needed. Performs general clerical duties relative to employment activities. Assisted with training for H&W team members as needed. Maintains a sick leave/PTO log to ensure that leave requests are appropriate. . Monitors PTO and sick leave balances. Assists with medical supply and equipment requests as needed. Train team members on medical supply charge system. Monitors compliance and retrains as needed. Assists in inclement weather to contact staff for their availability in order to meet staffing requirements. Assist with technical programs, systems (nurse call system), and hardware (e.g., printers) to identify opportunities to improve efficiency. Manage position-based journal (PBJ) reporting which impacts CMS Staffing Start rating. Ensure accurate and timely submission of reports. Check for compliance with CMS requirements. Coordinates PBJ back-up coverage with the Admissions Case Manager when necessary. Troubleshoot the Palcare nurse call system in the Small House HCC in collaboration with the Administrator. Ensure server is functioning properly, trains team members as needed. Authorizes office supplies for all Health & Wellness departments, staying within budgetary guidelines. Research and resolve variances or discrepancies. Track labor hours and variances. Job Requirements Must be a High School graduate or have attained the equivalent; Associate degree preferred. Previous experience preferred in payroll, accounting and staffing. Certified Nursing Assistant license highly preferred. Valid First Aid/CPR instructor license preferred. Must be able to deal tactfully and effectively with staff members using good communication skills as required. Must also have prior working knowledge of computerized payroll and accounting systems. Must be organized, flexible, pay attention to detail, and work well under pressure.
