Salary: $47-$50 an hour and a sign-on bonus of $10,000! About the Position Goodwin Home Health is currently seeking a full-time Registered Nurses to work 40 hours weekly, day-time hours. In your role as RN, you will develop and safely deliver high-quality skilled Home Health nursing services to seniors living in their homes. Goodwin Home Health provides skilled care throughout Northern Virginia, focusing on the counties of: Arlington, Alexandria, Fairfax, Prince William and Loudoun. Duties * Performs/completes OASIS documentation. * Performs a detailed assessment resulting in the development of a patient specific Plan of Care. Client clinical record documentation is done per agency policy. * Provides direct nursing care in a Home Health setting under the guidelines of a physician's order. * Communicates with other team members regarding the Plan of Care and works closely with team members for optimal outcomes. * Effectively communicates with referring physicians and provides accurate treatment notes and other necessary documentation in a timely manner, per policy. * Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician. Qualifications: * Graduate of an accredited school of nursing. * Currently licensed by the state board of nursing to practice as a Registered Nurse. * At least one-year acute care experience, Home Health preferred. * Current CPR certification. * Possess a driver's license, proof of automobile insurance and readily available, reliable transportation. Rounds are to be made no less than daily. * Provides clinical supervision of nursing care delivery. * Collaborates with household charge nurse and other health professionals in planning health care regimen and delegates nursing intervention as appropriate and makes appropriate referrals when beyond scope of practice. * Assists the Director of Nursing and Assistant Director of Nursing in completing performance appraisals for front-line team members. This includes evaluation completion on-time per policy, delivery of evaluation to team members, and review of their PARF. * Reports nursing concerns involving other departments, injuries to team members or residents, violation of policy, safety hazards and teaming needs to the Director of Nursing. Documents in the electronic record all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service. Teaches or supervises the teaching of household members, facility staff and volunteers who are responsible for assisting the resident with nursing or personal care needs. Competent in evidence-based goals of care conversations with residents and families. Triages referral calls and assesses whether patient needs immediate admission. Is able to competently complete all parts of the admission process, including receiving physician certification, if needed. Per shift, informs the Director of Clinical Services or Administrator of unusual or potentially problematic resident/family issues. Responds calmly and tactfully in stressful and emergency medical and psychosocial situations. Understands the necessity of full partnership with staff at all skilled nursing facilities, including actively soliciting feedback, responding appropriately and educating tactfully. About the Position The Admission Nurse is responsible for the care of patients assigned by the Director of Hospice Admissions. Promotes education for patients, family members and caregivers. • Demonstrates current knowledge regarding professional career. • Attends mandatory in-services and maintains 100% compliance with mandatory data-based training. • Participates in orientation of Hospice/Home Health care staff members. • Immediately reports abuse/suspected abuse to supervisor, and DHHS or APS. Job Duties * Assesses need and provides support by phone or a visit, depending on acuity of needs and in accordance with standards of practice and evidence-based knowledge for hospice programs set by the National Hospice and Palliative Care Organization. * Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family. * Obtain data on physical, psychological, social and spiritual factors that may influence patient health status and incorporate that into the plan of care. * Assist in determining the need to change the level of care being provided for hospice patients based on assessment and collaboration with attending physician and/or hospice medical director and manager on-call. * Provide or arrange for delivery of medications and supplies needed after business hours, weekends or holidays. * Provide appropriate support at the time of death, per agency policy, and support the patient's and family's unique spiritual and cultural beliefs. * By end of shift, report on-call activities to the hospice team via email, including phone calls, communication, visits, change in condition and other pertinent clinical information. * Documents in the electronic record all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service. * Teaches or supervises the teaching of household members, facility staff and volunteers who are responsible for assisting the resident with nursing or personal care needs. * Competent in evidence-based goals of care conversations with residents and families. * Triages referral calls and assesses whether patient needs immediate admission. Is able to competently complete all parts of the admission process, including receiving physician certification, if needed. * Per shift, informs the Director of Clinical Services or Administrator of unusual or potentially problematic resident/family issues. * Responds calmly and tactfully in stressful and emergency medical and psychosocial situations. * Understands the necessity of full partnership with staff at all skilled nursing facilities, including actively soliciting feedback, responding appropriately and educating tactfully. Job Requirements * A registered nurse who is currently licensed to work in the state of Virginia. * Minimum 5 years experience as a professional nurse, preferably in a hospice or in an acute care or long-term care setting. * Certified Hospice and Palliative Care, preferred. * Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings. * Display a positive and professional image and attitude in all relationships with patients, families, peers and in the community. * Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. * Proficient in Word, and able to master documentation software competently enough to assist others in its use. * Must be able to work independently, have demonstrated patient care planning and patient teaching skills and strong written and verbal communication skills. * Current CPR certification. * Valid drivers license and daily access to a reliable and insured vehicle. * Belief in the hospice concept. * Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested. * Participate in orientation of new employees or volunteers, as requested. Job Duties * Presents hospice services and philosophy of care to patients and families faced with the challenges of arranging end of life care. * Performs in person assessments to determine eligibility for hospice, receives certification from the hospice physician and obtains consent for service. * Completes admissions process, initiates plan of care and ensures immediate care needs are addressed. * Communicates with the receiving interdisciplinary team about the goals of care, initial plan of care and patient/family expectations. * Provides direct nursing care in a Hospice setting under the guidelines of a physician's order. * Performs/completes admission/start of care (SOC), recertification, evaluations, discharges and accompanying documentation in a timely manner. * Performs a detailed assessment resulting in the development of a patient specific Plan of Care. Client clinical record documentation is done per agency policy. * Communicates with other team members regarding the Plan of Care and continues to work with team members for the duration of the patient's case. * Effectively communicated with referring physician, either verbal or written, and provides accurate treatment notes and other necessary documentation. * Documents information regarding interpretation of Hospice care policy, agency philosophy, objectives, and long-range plans. * Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician. * On-going observational recording/reporting of changes in patients condition as soon as possible to the patient's physician, and other team members, * Makes recommendations for involving other services, as deemed necessary. * Advises and consults with the patient's family in the patient's therapy program. * Attend staff meetings at least 4 times/year. * Documents all communication with other personnel to contribute to patient's case conference notes. * Uses equipment and supplies effectively and efficiently and request supplies needed for patient care. * Maintains confidentiality of patient information as well as co-worker's personal information if applicable at all times. * Implements infection control and safety measures as appropriate. * Consistently addresses patients concerns. Promotes education for patients, family members and caregivers. * Demonstrates current knowledge regarding professional career. * Attends mandatory in-services and maintains 100% compliance with mandatory data-based training. * Participates in orientation of Hospice/Home Health care staff members. * Immediately reports abuse/suspected abuse to supervisor, and DHHS or APS. Job Requirements * Graduate of an accredited school of nursing. * Licensed to work as a Registered Nurse in the Commonwealth of Virginia. * At least on year acute care experience, Hospice preferred. * Current CPR card. * Ability to communicate effectively in English, both verbally and written. * Knowledge of the duties and functions of Home Health and Hospice care and its relationship to the healthcare community is preferred. * Must know or willingness to learn and perform HIS documentation and SOC. * Must possess the ability to make independent decisions and work independently thought out the workday. * Possess a driver's license, proof of automobile insurance and readily available, reliable transportation. * Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers. * Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patient's care/case. About the Position The Hospice team is seeking candidates to work Full-Time After Hours RN to support a team covering the Northern Virginia area within the beltway. Work with seasoned hospice professionals in caring for patients and families in their community and facility homes at the end of life. Candidates will be clinically competent and compassionate RN's with hospice or critical care background. We are accepting candidates interested in full-time work. Job Duties * Triage by phone all calls received after business hours. Assesses need and provides support by phone or a visit, depending on acuity of needs and in accordance with standards of practice and evidence-based knowledge for hospice programs set by the National Hospice and Palliative Care Organization. * Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family. * Obtain data on physical, psychological, social and spiritual factors that may influence patient health status and incorporate that into the plan of care. * Assist in determining the need to change the level of care being provided for hospice patients based on assessment and collaboration with attending physician and/or hospice medical director and manager on-call. * Provide or arrange for delivery of medications and supplies needed after business hours, weekends or holidays. * Provide appropriate support at the time of death, per agency policy, and support the patient's and family's unique spiritual and cultural beliefs. * By end of shift, report on-call activities to the hospice team via email, including phone calls, communication, visits, change in condition and other pertinent clinical information. * Documents in the electronic record all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service. * Teaches or supervises the teaching of household members, facility staff and volunteers who are responsible for assisting the resident with nursing or personal care needs. * Competent in evidence-based goals of care conversations with residents and families. Job Requirements * A registered nurse who is currently licensed by the Virginia State Board of Nursing. * Minimum 5 years experience as a professional nurse, preferably in a hospice or in an acute care or long-term care setting. * Certified Hospice and Palliative Care, preferred. * Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings. * Current CPR certification. * Valid VA Drivers License and daily access to a reliable and insured vehicle.
