SUMMARY:
The Partial Hospitalization Program (PHP) Clinician will assist with the initial evaluation and patient intake interviews and assessments, assess patient's presenting problems, formulate diagnoses, and make recommendations regarding appropriate treatment. This position requires demonstrated knowledge of the disease of addiction and/or mental illness, twelve-step recovery principles, and other evidence-based practices in addiction or mental health treatment.
ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
Assist with developing, implementing, and modifying, as necessary, treatment plans and continuing care plans.
Conduct case management services and prepare discharge summaries.
Provide crisis management for patients, including, if required, on-call consults.
Interact with referral sources and other external providers to ensure coordination of patient services.
Participate in continuing education activities and in-service training programs to remain knowledgeable in area(s) of expertise and to meet credentialing and privileging requirements.
Attend meetings as directed and meet with supervisor to receive supervision and participate in clinical reviews.
Monitor caseload to guarantee compliance with third-party payers, including chart reviews, completing pre-certification requests, initial and continuing reviews, and authorizations.
Serve as a resource person to Gosnold and the community in areas of expertise, including presenting classes, speaking engagements and agency consultation, as directed by the Program Director.
Document patient records in accordance with Gosnold standards and regulatory and payer requirements, utilizing electronic formats, as required.
Serve as professional mentor to Interns, Counselors, and visiting students.
Abide by the professional staff code of ethics and maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
Perform all other duties as assigned.

MINIMUM KNOWLEDGE, Experience, AND SKILLS REQUIRED:

Education: Master's degree in counseling or related discipline.
Experience: Minimum one to two (1-2) years supervised counseling experience as part of a behavioral health education curriculum. Working familiarity and/or direct experience in the use of relevant technology (EMRs, tele-health, etc.).
Skills: Training in the special population areas necessary for provision of effective counseling and care management. Ability to interact and relate to persons in early recovery from addiction and chronic mental health concerns.
Certificates, Licenses, and Registrations: Independent license (LICSW, LMHC, or LMFT) preferred.

Essential Duties and Responsibilities:

The Provider Enrollment Coordinator will be responsible for the insurance carrier enrollment and re-enrollment of clinical staff and ancillary providers, the maintenance of these providers in the Medical Staff Office system and in CAQH, and the dissemination of the provider information to both internal and external customers.
The Provider Enrollment Coordinator must possess a comprehensive knowledge of the MGB payer policies, contract language and procedures to prepare and disseminate provider enrollment applications to the MGB payers. Payer enrollment applications must be executed accurately to ensure the correct set up with health plans. The Provider Enrollment Coordinator is required to demonstrate effective communication skills and the ability to work as a team player with various levels of staff both internally and externally.
Principal Duties and Responsibilities:
• Prepare paperwork, deliver to provider, then collect all enrollment materials required to enroll or re-enroll a provider with the insurance carriers.
• Insure the integrity of and carries out the enrollment process in accordance with the Unit policies.
• Acquire a CAQH ID for the provider, complete CAQH information, and update the appropriate enrollment system with the CAQH ID information for the provider.
• Submit all enrollment paperwork to the carriers, including but not limited to name, location, and practice guidelines.
• If available by the payer enter all enrollment information into the appropriate online portal or system and adhere to all appropriate rules and regulations as established.

Under the clinical direction of the Gastroenterology (GI) Division Ambulatory Nursing Manager and in collaboration with the supervising providers, the GI Triage Nurse will be responsible for the care of patients within MGH Gastroenterology.
We are looking for a RN with experience and knowledge of GI conditions who is familiar with working in a tertiary/academic hospital setting to join our fast-paced busy outpatient clinical practice. The RN will assist assigned physicians and advanced practice providers and work with their provider team to facilitate access to the clinic, return patient calls, process prior authorizations, respond to patient portal messages, provide patient teaching sessions, and work closely with the administrative staff in coordinating patient care.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Facilitates access for patients of the GI Clinic by filtering incoming records for appropriate patients. Will review patient charts and assist in obtaining information needed for an effective consultation with the GI specialist.
Triage of patient concerns received through phone calls and Patient Gateway messages. Will gather information about relevant clinical signs and symptoms and makes accurate triage decisions regarding patients' care, in conjunction with the patient's provider.

Ensures that referrals to other specialists, radiological, endoscopic and laboratory testing are coordinated with patients and office staff.
Assists with letters of medical necessity for prior authorizations, disability forms, and FMLA forms.
Performs injections/ vaccinations as needed.
Act as a resource regarding complex medications for patients and their families, including information regarding co-pay assistance programs, grants, and available community resources.

POSITION SUMMARY:
Community Action's Tax Scheduler will be responsible for answering the phone for Community Action Pioneer Valley's Free Tax Assistance Program (VITA), conducting phone eligibility screening and scheduling virtual and in-person tax preparation appointments. Call volume is high and the Scheduler will screen approximately 900 households during the tax season. Responsibilities also include recording call data into Excel and agency databases, utilizing Outlook Calendar and AppToto appointment software, preparing mailings, helping to follow-up with taxpayers, data entry and other duties as assigned by the Program Coordinator. Training in VITA advanced tax preparation will be provided with a requirement to become certified within the first 30 days. Evening and weekend hours may be required during tax season.
ESSENTIAL RESPONSIBILITIES
Answer and respond to a high volume of phone and electronic messages efficiently and promptly.
Manage all aspects of scheduling for the tax program including initial contact, screening for eligibility, appointment reminders, cancellations and ensuring the maximum number of participants are scheduled and prepared for their appointment.
Assist with maintaining VITA-specific customer relationship software used for virtual tax clinics.
Assist Tax Team in various activities to prepare for the tax season including mailings, preparing materials for volunteers and other duties.
Maintain taxpayer files and enter data into Excel and agency database for reporting purposes. Ensure required data entry and reporting is completed in an accurate and timely manner in accordance with program guidelines.
Maintain high level of customer service following Community Action's established Customer Service Standards and all applicable guidelines set forth by the program.
Remote work from home will be assigned and determined by the Money Matters Coordinator; when schedule requires remote work, expected to be available for meetings, calls, responding to emails, and conducting regular duties during standard service/agency hours.

Comply with agency and funders' paperwork requirements and procedures.
Attend all required staff meetings, supervision, training and contractual meetings.
Attend mandatory agency trainings and other relevant trainings.
