The Advanced NursePractitioner (NP) is responsible for all aspects of patient care, including diagnosis, treatments, and consultations as allowed within the tribal health agreement. In an ambulatory and out-patient environment, the ANP is responsible for participating in development, execution and periodic review of the written policies governing the program services and activities prescribed in the tribal health funding agreement.

Assist in the development and implementation of an ambulatory health care system as prescribed in the tribal health agreement to meet the health needs of the tribal community.
Develop and conduct health education classes and/or specialty clinic.
Engage with other health professionals to develop treatment protocols, standards of practice, and policies.
Participate in quality improvement activities, including audits, chart review, development of review criteria, and development of plans for improving performance, and evaluation of the effectiveness of actions to improve performance.
Reviews and critique current research, offering recommendations to medical staff to adapt to current practices.
Conduct a health and developmental history from the patient and/or family member and performs a physical examination.
Orders and/or performs screening, diagnostic tests, and procedures to provide direct physical and psychological care in emergencies and in the presence of illness or disability to maintain life.
Maintain patient records by documenting data, patient assessments and care plans.
Formulate and implement a care plan, prescribe medications to treat symptoms, and monitor medical regimens.

. The company provides NursePractitioners to Skilled Nursing homes to work with their nursing teams to improve the quality of care and reduce re-hospitalizations. This position requires 2-3 days of work at a skilled facility in the Lewisburg, PA area. Scheduling can be flexible. (no weekends required). The company provides dedicated Scribes and a support staff to assist with documentation, medical records, and scheduling.
The ideal candidate will be a board-certified NursePractitioner with experience assessing, evaluating, and documenting skin and wound conditions, and have an in-depth knowledge of treatment protocols. Experience working in a Skilled Nursing Home or long-term facility is preferred.
The N-P will report to the company's Regional Clinical Director and work closely with the facility's Medical Director and Director of Nursing.
Some Responsibilities:
Conduct routine check-ups and examinations to assess skin and wound conditions
Diagnose, treat, and manage acute and chronic conditions and develop plans of care
Implementing orders and treatments
Evaluating the progress of specialized skin and wound conditions
Evaluate new admissions
Document status and care plans and monitor changes
Notify the House physician and family of changes
Provide Wound Care education to nursing staff
Cultivate a climate of trust and compassion for patients
Requirements:
Current PA State RN license
PA Licensed NursePractitioner
At least 2+ years' experience in the care and treatment of wounds
In-depth understanding of Wound Care protocols and treatments
Ability to train Nursing teams on policies and protocols for the prevention and care of wounds
Experience in Skilled Nursing Homes is a

Basic Computer skills and experience with EMR software
