    Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, patient family, and other health team members while maintaining high standards of professional nursing. Provides high quality care to the following patient populations: Adult patients experiencing a life limiting illness deemed to be in the last six months of life. Coordinates all hospice services required for optimal end of life care. Manages hospice resources for optimal patient care.The Case Manager works as a part of the interdisciplinary care team, addressing the patient's quality and plan of care, goals and expected outcomes, and readmissions prevention/reduction strategies. The case manager coordinates discharge education while serving as a resource to the care team with progressive work towards discharge planning in both the inpatient and Emergency Department settings. What you will do * Accurate and timely record patient psychosocial assessments and interdisciplinary team meetings * Recognize opportunities for documentation improvement and collaboration for plan of care. * Effective and appropriate communication with providers, nursing and ancillary staff, and leaders. * Ability to multi task and manage multiple priorities * Accurate input of data and review of quality metrics. * Demonstrate an understanding of medical complications, co-morbidities, severity of illness, risk of mortality, secondary diagnosis, and an ability to impart this knowledge to providers and other members of the interdisciplinary team. * Evaluate nursing intervention outcomes, conferring with other health care team members as necessary. * Gather and analyze information pertinent to documentation findings and outcomes. * Timely follow up of culture results and commmunication. * Understanding of social determinants of health screening and assisting the interdisciplinary team with communication of resources to the patient. * Ability to manage patient's health concerns utilizing technology * Regular and reliable attendance. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures. * Knowledge of principles and processes for providing customer and personal services. Review potential admits with Clinical Liaison to assess for discharge planning needs/barriers. 2. Complete care management initial evaluation within two business days of admission. 3. Attend daily (M-F) rehab huddles as indicated by medical necessity and/or discharge planning. 4. Guide weekly required TEAMS meetings. 5. Complete Interdisciplinary Plan of Care (IPOC) power forms as indicated. 6. Complete concurrent insurance reviews. 7. Initiate post discharge follow-up phone calls on discharged patients within two business days. 8. Position Description - What the Role Entails: The Case Manager uses an evidence-based model to serve participants between the ages of 17 and 21. The Case Manager is provides services to young people who are transitioning out of care and need highly individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining, and maintaining employment, building a support system, and remaining free from legal involvement. Areas of Accountability - What you will be doing: Provide individual services to 6-8 young adults in the home and/or community living setting. Assist participants with the transition to adulthood, meeting a minimum of one time per week (face-to-face) and providing additional support via phone, text or in person, as needed. Responsible for all interventions associated with a participant. Collaborate with external key stakeholders and referral sources, such as probation, child welfare workers, therapists, etc. Use a solution-based perspective to conduct assessments of participants' needs and strengths. Develop service plans and document all service contacts with participants within the timelines required by RFS, DCFS, and the LifeSet model. Attend meetings with participants to gather information specific to intervention development and advocate in the best interest of a participant's permanency and long-term stability. The Case Manager will be included in an on-call rotation and is expected to respond in person when it is determined necessary by the Supervisor, Program Director, and Program Administration. Participate in weekly individual development, including clinical supervision and group consultation. The consultation will be led by a licensed professional who is an expert in the LifeSet model. Adhere to all ethical and legal guidelines, including confidentiality and mandated reporting. Specialists are part of a national network across multiple public and private organizations all striving to enhance the standard of care for older youth. Other duties as assigned. Assign and reassign youth to MCM services within required timeframes, including: New custody cases within 5 business days after the 45-day interim period (95% benchmark). Reassignments within 14 days of receipt from the Ongoing Medical Case Management Agency. Conduct outreach to PCP/Dental providers for transfers involving youth ages 0-5 and pregnant youth. Prepare and forward complete MCM assignment packets-including health information, placement details, and contact information-according to contractual timelines. Generate and distribute written correspondence informing caregivers, youth, caseworkers, and providers of assigned MCM services. Work with the Medical Records Clerk to ensure closed cases are properly prepared for storage and long-term retention. Produce standard and custom reports using information system tools as directed by the HealthWorks supervisor. Gather and maintain data for reporting and analytics needs within HealthWorks. Complete all assigned data entry in the HealthWorks Database/SACWIS and Athena systems. Review and complete tasks associated with SACWIS reports and other DCFS reporting requirements. Use system reports to support data integrity and sanitization of the information system.
