We are seeking a self-motivated, detail-oriented, and highly organized Case Management Coordinator to support Medicaid Long Term Care/Comprehensive Program members in Miami-Dade County, FL. This role is primarily field-based, requiring approximately 75% travel within the assigned region, with 25% work-from-home responsibilities. The coordinator will assess, plan, implement, and coordinate case management services to support members' medical, social, and wellness needs across home, assisted living, and nursing facility settings.
Key Job Duties
Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees
Conduct telephonic and face-to-face assessments of members in homes, assisted living facilities, and nursing homes
Perform comprehensive member evaluations using care management tools and data review
Provide coaching, education, and support to empower members to make informed healthcare decisions
Monitor, evaluate, and document care activities in compliance with regulatory and accreditation guidelines and internal policies
Utilize case management and quality management processes consistently and accurately
Experience & Qualifications Required
Bachelor's Degree required, preferably in Social Work or a related field
Case management experience required
Long-term care experience preferred
Bilingual Spanish/English strongly

Purpose

THE WORK:
You'll design, develop, and maintain ETL (Extract, Transform, Load) workflows using Esri's Data Interoperability Extension and Workbench.
You'll create, update, and manage related data tables within enterprise geodatabases to maintain accurate Customer Service Location relationships.
You'll perform ongoing quality control and maintenance of large utility, parcel, and service location datasets to ensure data accuracy, consistency, and integrity.
You'll develop automated processes using Python, ModelBuilder, or SQL to streamline data workflows.
You'll produce high-quality maps, dashboards, applications, and analyses for operational and planning teams.
You'll query, extract, and organize asset data for GIS analysis and thematic mapping.
You'll prepare and publish GIS services for use in online GIS.
You'll collaborate with technical and field staff to ensure GIS data meets system and reporting needs.
You'll perform other duties and assume other responsibilities as apparent or assigned.
Remote: This role allows for remote work for the majority of your work hours.

Must live within the state of Florida
Bachelor's degree in Geography, GIS, Environmental Science, Civil/Environmental Engineering, Computer Science, or a related field
Minimum 2 years of professional GIS experience
Proficiency with Esri ArcGIS Pro and ArcGIS Enterprise
Hands-on experience with ETL tools, including Esri's Data Interoperability Extension and Workbench
Demonstrated experience creating and managing related data tables within enterprise geodatabases
Strong understanding of relational database design, data integrity, and QA/QC best practices
Experience with Python and/or SQL for data automation
Excellent attention to detail and a meticulous, careful work ethic when working with large datasets
Ability to critically think about technical problems and work diligently to resolve them in a timely manner
BONUS POINTS IF YOU HAVE:
Experience with FME, ModelBuilder, or other workflow automation tools
Knowledge of spatial databases (e.g., SQL Server, PostGIS) and asset management systems (OpenGov, Cityworks, etc.)
Ability to manage related tables within GIS environments, including maintaining relationships, performing large-scale updates, and analyzing feature class data based on updates to associated tables

Summary of Position Description:The Scribe-X medical scribe is a critical member of the patient care team. Medical scribes will work side-by-side a healthcare provider to document patient encounters in real-time. Medical scribe training is provided to all scribes to assure they are prepared to support their assigned provider(s).
Duties of a Medical Scribe
Perform chart preparation per clinic protocol
Accompany the provider in all scheduled patient visits
Document the patient history, physical exam, procedures, and patient plan, as performed by the provider
Remind provider of relevant quality metrics when appropriate, documenting to support quality metrics
Enter laboratory and radiographic studies, as ordered by the provider
Enter in medication orders, as dictated by the provider
Document and print instructions for the patient
Review completed charts with the provider between patients or at the completion of shift
Update provider preference and clinic preference documents as necessary
Education and Skills
Excellent verbal and written English skills
Strong computer skills with the ability to learn and navigate new software quickly
Healthcare track (e.g. pre-med, pre-PA, pre-nursing) is preferred
Bachelor's degree strongly preferred with a GPA of 3.00 or greater
High School diploma or equivalent required
If no college degree, 3+ years of full time work experience strongly preferred
Most assignments require a typing speed of at least 60 WPM
Some assignments require a typing speed of at least 80 WPM
Spanish-speakers are eligible for a higher pay.

$11/hour - No scribe experience
