Requirements
Qualifications:
Bachelor's degree in social work, psychology, rehabilitation, education or a related human services field. Or, an associate's degree in social work, psychology, rehabilitation, education or a related human service field along with either two years of experience working with persons who have a serious emotional disturbance or serious mental illness, or two years of experience that provides an understanding of mental illness. Applicants that are in process of obtaining a degree may also be considered.

Clinical Team Manager, Adult Services

Greater Nashua Mental Health is looking for a Clinical Team Manager to clinically and administratively manage teams within the Community Support Services Department. Responsible for the Team's performance on KPI's, including productivity expectations and paperwork completion. Oversees the professional development of direct reports. Expands Evidence Based Clinical Practices within the department, spearheads and champions quality initiatives, and refines clinical practice guidelines, processes, and procedures within the department.
Duties & Responsibilities:
Provides weekly one (1) hour individual supervision to master's level therapists (four (4) to six (6) individuals). Completes all documentation of the supervision sessions in a timely manner
Prepares for and facilitates weekly team meeting (1 hr), weekly Therapist meeting (1 hr) weekly Leadership Meeting (1 hr), weekly Community Based Services (CBS) staff meeting (1 hr), and Community Support Services (CSS) department meeting (1.5 hr every other month)
Provides supervisory consult to team members as needed as well as ad hoc support to staff for crisis management including guidance around completing IEA's and CD revocations. Also provides supplemental counseling to members of clients' support systems (e.g., family members)
Provides a minimum of six (6) hours per week of psychotherapy for individuals, groups, and/or families, as well as case management services
Completes treatment plans, Adult Needs and Strength Assessments (ANSA) and Case Management Care Plans (CMCP) by the required due date listed in EMR and supports and monitors staff doing the same
Completes all 6-month reviews within a two (2) week window at the end of each review period, and completes all other documentation within three (3) business days (with the first day being the dates of service/completion) and supports and monitors staff doing the same
If working a hybrid schedule, maintains a standard schedule of working on site at a GNMH location 3 days (24 hours) a week
Provides internship and/or licensure supervision, as appropriate based on credentials
Performs other duties or special projects as required or as assigned

Counsels and assesses eligibility uninsured clients for State and Federal medical financial assistance programs, such as disability determination, Medicaid eligibility, and benefits determinations
Assists clients through the eligibility determination process and completion of necessary documentation whenever case managers are either unassigned or unable to do so
Calculates cost of new or revised treatment plans, and reviews with clients their responsibility after all expected insurance and benefits are applied, and obtains client acknowledgement of GNMH financial policy
Assesses clients for potential charity care based on GNMH charity care policies, and completes all required documentation
Maintains positive working relationships with GNMH Clinical, Billing, and Client Access staff to ensure client financial information is accurate and up-to-date within the EMR system and addresses the client's financial needs while securing funding

Completes, monitors, and ensures assigned clinical staff holds all necessary credentials

Attend planning sessions, provide accurate information and recommendations in areas consistent with position responsibilities. Major Functional Areas of Responsibilities
Responsible for supervision of IT positions: User and Network Systems position and the IT Support User Security position. Support ITS staff in completion of their duties and provide coverage to address essential departmental responsibilities. Provide positive timely and appropriate assistance to staff working with CLM ITS tools and resources. Provide exceptional user-support, and escalate requests to the IT Development Manager. Coordinate services provided by vendor supports toward management of network system operations and security related duties. Enforce policy (e.g. report and remove unapproved applications, track and install software updates and patches).
Supervise and complete Network and System Operation tasks setup, testing, monitoring, maintenance, and troubleshooting and ongoing improvement of local and remote technology resources, applications and network operations. Configure, monitor, troubleshoot, and maintain agency VOIP phone systems, ensure appropriate operation, messaging, use, and documentation, support, and coordinate approved position related vendor work.
Supervise and complete User access management tasks; provide and manage secure passwords, secure folders for users/groups, establish/maintain controls to prevent unauthorized access, implement approved measures to increase data security, track and install security patches, monitor, manage, and report security issues. Maintain anti-theft processes/systems.
Supervise and complete tasks to maintain up to date virus protection software/virus definitions, report and log instances of activity that increase risk to resources/systems, implement approved options to decrease virus risk.
Supervise and complete tasks to maintain and monitor backup software, hardware, and media to ensure successful backup of user data, database systems, server and application files and images. Assist clients in identifying and achieving their employment goals, based on the clients' interests and preferences. Engages all available supports, including treatment team members and identified natural supports. Provides services through all phases of employment, including resume building, preparing for job interviewing, and sustaining employment through the provision of follow-along supports.
Duties & Responsibilities:
Provides direct client care to individuals based on assigned caseload; provides a minimum of 50% of billable service hours (20 hours if full time) each week.
Completes documentation of progress notes for each service delivered, including outreach attempts, within 3 business days (with the first day being date of service).
Maintains schedule in the agency's Electronic Health Record (EHR).
Conducts research in order to prepare for client visits that support targeted intervention, including evidence-based interventions, e.g., Motivational Interviewing.
Participates in weekly individual supervision with the Coordinator of Supported Employment, and weekly group supervision with the supported employment team.
Reaches out to and connects with new referrals within 7 business days from receipt of referral.
Engages in weekly job development with a goal of six job development contacts per week; providing no fewer than three job development contacts per week.
Attends mandatory CSS weekly team meetings.
Attends stakeholder meetings, such as local Vocational Rehabilitation, Annual Employment Connect, or Monthly SE meetings with other mental health centers, as identified by the Coordinator of Supported Employment.
Collects and shares employment statistics with SE group during weekly SE group supervision and with assigned treatment team.
Participates in rotating job board activities, promotes SE programs to interested stakeholders, i.e. HEARTS Peer Support Center, and conducts presentations at department meetings.
Participates in peer chart reviews and annual audit preparations.
Conducts and submits mileage/expense sheets within the pay period they are due.
Performs other duties or special projects as required or as assigned.
Requirements
Qualifications:
Bachelor's degree in Psychology, Social Work, Vocational Rehabilitation, or related field
Previous experience working with individuals with disabilities.

Under the supervision of the Nursing Manager with direction from the ACT psychiatrist and ACT team leaders, the Nurse provides comprehensive nursing support to clients on the ACT team. Supervises the medication administration of all ACT clients, which includes verifying that medications given reflect the most recent orders, packaging medications for delivery by ACT team staff, providing medication management at the Agency for a select number of clients, scheduling and delivery of long acting injectable medications, and maintaining accurate records of all related activities. Provides medication education to clients, families and staff.
Duties & Responsibilities:
Provides functional support services through medication management/education daily and as scheduled for ACT team clients and document accordingly.
Obtains and packages medications for delivery to clients both at the Agency and in community. Verifies the accuracy of medication regimen. Maintains timely records of administration. Coordinates the delivery of medications daily.
Conducts proper nurse assessment, schedules and delivers long-acting injectable medication, provides patients with education on medications, and maintains timely and accurate documentation of interactions related to patient care, with a 25% productivity expectation. Collects specimens for laboratory testing and enters orders into online systems.
Completes all documentation within 3 business days (with first day being date of service/completion).
Reconciles 95% of all prescribed medications at time of client's hospital discharge.
Ensures that 90% of refill requests are processed 2 business days.
Conducts outreach to outside providers, both medical and psychiatric, on behalf of clients. Attends outside appointments as necessary.
Attends team meetings in order to best provide integrated care to clients.
Interacts with pharmacies and insurance companies to ensure prompt delivery of needed medications.
Conducts nursing assessments for ACT clients, preferably at their place of residence. Develops nursing care plans as warranted.
Provides coverage across ACT teams when necessary.
Provides occasional nursing coverage for CSS clients.
