PORT Health group facilitator job description
This position will maintain an average work schedule of 40 hours per week. Generally, working the hours between 8:00am-9:00pm, but flexibility will be expected depending on client and Program’s need. In the event of inclement weather, the employee may be required to remain on the unit beyond their normal eight-hour workday.
Primary Purpose of the Position:
The primary purpose of the SAIOP Qualified Professional is to provide treatment, after-care, and referral services to adults/children in need of Substance Abuse Intensive Outpatient treatment. The employee will work closely with the Program manager, Physician, other clinical staff, and community agencies. The employee will provide services to persons served which include, but are not limited to:
-Individual/Group counseling and support,
-family counseling, training or support, biochemical assays to identify recent drug use,
strategies for relapse prevention to include community and social support systems in treatment,
-Life skills, Crisis contingency planning, Disease management, Treatment support activities that have been adapted or specifically designed for persons with physical disabilities, or persons with co-occurring disorders of mental illness and substance abuse/developmental disability and substance abuse/dependence.
-“First Responder “ Crisis response 24/7/365 basis to recipients receiving this service. Includes face-to-face and also telephonically at all times, with the capacity for face-to-face emergency response within 2 hours
The SAIOP Qualified Professional is clinically supervised by the SAIOP Program Manager.
SAIOP Program Documentation Requirements:
Minimum standard is a daily full service note for each day of SAIOP that includes the person’s served name, Medicaid Identification number, date of service, purpose of contact, describes the provider’s interventions, the time spent performing the intervention, the effectiveness of the interventions, and the signature and credentials of the staff providing the service. A documented discharge plan will be discussed with the person served and included in the record.
D. Staffing Requirements:
Persons who meet the requirements specified for CCS, LCAS, LCAS-A, and CSAC under Article 5C may deliver SAIOP. The program must be under the clinical supervision of a CCS or a LCAS who is on site a minimum of 50% of the hours the service is in operation. Services may also be provided by staff who meet the requirements specified for QP or AP status for Substance Abuse according to 10A NCAC 27G .0104, under the supervision of a LCAS or CCS. Individuals shall register with the Board prior to providing professional services. The maximum face-to-face staff-to-beneficiary ratio is not more than 12 adult beneficiaries to 1 QP based on an average daily attendance. The ratio for adolescents shall be 1:6. Paraprofessional level providers who meet the requirements for Paraprofessional status according to 10A NCAC 27G .0104 and who have the knowledge, skills, and abilities required for the population and age to be services may deliver SAIOP, under the supervision of a LCAS or CCS

The Strengthening Families Program (SFP) is a nationally and internationally recognized parenting and family strengthening program for high-risk and general population families. The position will provide community-based services via groups to Racine County families. The focus is to improve parenting skills and family relationships and reduce delinquency and alcohol & drug abuse in children. Child maltreatment also decreases as parents strengthen bonds with their children and learn more effective parenting skills.
The Brighter Futures Initiative program provides a county-wide substance abuse prevention program targeted to elementary school- and middle school-age children and their parents. The primary focus is to provide services to registered youth in schools, churches, community centers, etc. A major emphasis of the program is helping youth identify their interests, identify short and long-term goals, develop short and long-range plans for implementing their goals, and assist youth in achieving their goals.
SALARY: $19.00 per hour
LOCATION: Office is based in Racine, WI. Hybrid model of time spent at the office and out in the community meeting at schools, churches, or other community centers. Hours are varied. About 15 hours per week.

KEY RESPONSIBILITIES:
Recruit families and coordinate services with other community providers, including substance abuse prevention services. Utilize the Strengthening Families Program and Brighter Future Initiative curriculum, which is an evidence-based parenting skill, children's social skills, and a family life skills training program specifically designed for high-risk families. Demonstrate competency in the areas of diversity, e.g., culture, race/ethnicity, religion, language, social customs, disabilities, economics, gender, and age. Have a strong knowledge of community resources and refer families to needed services. Conduct face-to-face contact with families, per program standards, to promote and teach healthy parent-child communication. Provide flexible hours and diverse and accessible locations and make meeting families where they are a priority. e.g., community/neighborhood centers, churches, schools, family resource centers, hospitals, public meeting rooms, etc. After training is complete, staff are added to on-call duties to monitor after-hours calls.
Requirements:
Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance. Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related fields is required.
