Position Title: PATH Outreach Worker (part-time) Program: Housing, Disability and Advocacy Programs Position Summary The PATH Outreach Worker is responsible for providing assistance and trauma informed support to homeless and formerly homeless residential clients with serious mental and/or physical disabilities. The PATH Outreach Worker is expected to identify homeless mentally ill adults in the community, assess their medical and psychiatric needs, and assist them in obtaining appropriate services, including but not limited to referrals to Behavioral Health, Drug and Alcohol, and the Department of Social Services (for Cash Aid/Food Stamps/MediCal and Social Security Disability application support). This is a part-time position of 24 hours per week with the ideal candidate able to work three full 8-hour days during the regular work week. The PATH Outreach Worker will uphold and promote the program philosophy in all duties performed. Hours/ Salary 24 hours per week. Receive referrals from a variety of sources and meet with clients in a timely manner to assess their needs and appropriateness for services (includes completing intake paperwork with potential clients and following their progress until outreach services are no longer needed or desired. Conduct VI-SPDAT surveys for determination of severity of needs and placement onto the Coordinated Entry Program for housing and support services. Recognize and be available to clients in psychiatric crisis or emotional distress, linking them to appropriate support systems, including outpatient and crisis/inpatient treatment. Function as a role model to clients with regard to rehabilitation and recovery, communication skills and work behavior. Transport clients to and from appointments. Work with up to 25 unhoused, dual-diagnosis clients at a time for the PATH program, in collaboration with the Homeless Encampment Case Manager, providing services and support as listed above. Site management: Includes inspecting sites and assisting with cleaning and/or repairs. Administrative Duties: Complete program paperwork in a timely fashion, including time sheets and financial receipts. Administrative duties may also include: maintaining individual paper and/or electronic client files; evaluating and charting client progress; developing service plans, admission and/or discharge paperwork; HMIS data entry, which includes entering and exiting all PATH clients; managing client notes and entry/exit dates in HMIS as well as Excel, Word and One Note; collect and record client rents and security deposit receipts, and other required forms for housed clients; meet program deadlines for vehicle and housing maintenance sheets, petty cash and other agency requirements that are time-sensitive. 10. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA’s Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication. Complete all assigned trainings. Attend all HDAP, agency and other required staff meetings to coordinate case management activities with supervisors and program staff. Must be comfortable working in a diverse and unpredictable environment and meeting clients “where they are at” with a “whatever it takes” non-judgmental, trauma-informed approach. Experience as a medical assistant. Must maintain continuing education credits as required by the state employed Ability to administer medications. CPR (Cardiopulmonary resuscitation), First Aid, and BLS (Basic Life Support) certification. Preferred Graduation of a post-secondary medical assisting education program accredited by an agency recognized by the U.S. Department of Education. Current certification from the Certifying Board of the American Association of Medical Assistants (AAMA), or registration from the American Medical Technologists (AMT), or the National Center for Competency Training (NCCT), or any recognized certifying body approved by the Board. CNA (Certified Nursing Assistant), RMA (Registered Medical Assistant). Two (2) years’ experience or knowledge in the field of behavioral health recommended. Responsibilities: Works as part of the intake team to ensure patients are seen promptly. Observe urine samples for urine drug screens, test onsite and package for laboratory for confirmation testing. Obtain vital signs and report findings to nurse when needed. Makes appointments for patients to see physician, provide lab work and referrals for additional follow up care. Maintain and keep updated all medical records and documentation pertaining to the patients. Assist with Purchasing and ordering of medical supplies Assists in education with pregnant patients about prenatal care and assists primary counselor in coordinating outside services as needed.
