The right candidate will provide essential care and support to our patients and their families, helping with daily living activities and ensuring comfort in alignment with each patient's individualized Plan of Care. As a valued member of our care team, the Hospice Aide will: Provide direct patient care under the supervision of the RN Case Manager, including assistance with bathing, grooming, oral hygiene, skin care, nail care, and other personal needs. Support patients with activities of daily living (ADLs) in a way that promotes dignity, comfort, and safety. Report observations and changes in patients' physical or emotional condition to the RN Case Manager and interdisciplinary team. Respect professional boundaries while providing care in homes and healthcare facilities. Safely assist with transfers and mobility, including the use of hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches. Offer emotional support and compassionate presence to patients and caregivers. Requirements: Current CNA (Certified Nursing Assistant) or HHA (Home Health Aide) certification in the state of Indiana. Reliable transportation, a valid driver's license, and willingness to travel within our service area. Flexibility to participate in the on-call rotation schedule. Physical ability to perform the duties of the role. Strong time management skills and the ability to work independently while being part of a supportive team.
