The Ambulatory Pharmacy Navigator program helps HMH Discharge and Specialty Pharmacy patients receive their discharge/specialty medications quickly and efficiently. This allows patients to be discharged from the hospital smoothly and begin their recovery at home or start their specialty medication therapy right away. The Pharmacy Navigator I plays a crucial role in ensuring patients receive their prescriptions smoothly. They gather all the necessary information, prioritize tasks, address any financial, insurance, or clinical challenges, and make sure the prescriptions are delivered to patients before they leave the hospital or directly to their homes. The Navigator ensures OBRA compliance by ensuring an offer to counsel is made and that patient medication educational needs are met. The Navigator I functions will include all job functions of an Ambulatory Pharmacy Technician plus additional duties described below.

includes:
The Navigator duties includes all job functions of an Ambulatory Pharmacy Technician plus additional duties.
Establish an initial greeting to patients admitted or who have a clinic appointment and explain services available via the HMH Pharmacies.
Performs prescription order entry duties as necessary.
Obtains patient authorization to process prescriptions through the ambulatory or specialty pharmacy.
Engages in a collaborative process with floor nurses, discharge planners, case managers, social workers, physicians, APN's, PA's, Medical Residents, unit clerks and other teams/initiatives as required to ensure patients scheduled for discharge to home or who have clinic appointments can receive the critical medications needed in a timely manner.
Works to determine patients being scheduled for discharge or who have a clinic appointment to ensure HMH Pharmacies are selected by patient choice to provide services.
Determines information sources necessary to process prescriptions (e.g., insurance coverage).
Identifies and Troubleshoots patient medication access issues related to insurance, financial or insurance coverage and helps to resolve so patients can receive needed medications.
Adheres to all established HMH Pharmacy and Federal/State privacy and compliance policies to protect patient confidentiality.
Ensures prescription fulfillment by in-house pharmacy based upon priority needs to allow for timely discharge and/or start of specialty medication.
Coordinates and facilitates patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas or home.
Ensures patients receive offer for pharmacist counseling and distributes pharmacist contact information in the event patient has prescription-related questions at home.
Facilitates distribution of medication-related educational materials and coordinates counseling with staff pharmacist where needed.
Assists Ambulatory Pharmacy staff as needed.
Performs duties of Ambulatory Pharmacy Technician if required.
Ensures all guests/patients receive safe care. Maintain a 'safety first' work ethic. Report any medication errors or medication variances as required by HMH Pharmacy Policy.
Other duties and/or projects as assigned.
Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
Excellent written and verbal communication skills.
Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Education, Knowledge, Skills and Abilities Preferred:
Retail/Specialty Pharmacy experience.
Strong knowledge and experience with PBM Claims and coverage.
Licenses and Certifications Required:
Pharmacy Technician License.
PTCB or Equivalent Certification Required within 180 days of start of employment with the following exceptions:
Hired prior to January 2024
Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before 1-1-24

The Pharmacy Technician functions under the supervision of a licensed pharmacist, and assists the pharmacist(s) with the completion of their responsibilities. This includes: the preparation of medication orders for review by the pharmacist; maintenance of adequate drug supplies in filling areas; and delivery of medications to patient care areas.
Responsibilities
A day in the life of a Pharmacy Technician at Hackensack Meridian Health includes:
1. Inventory - Maintains accurate and appropriate inventories of medications and supplies. Supports the drug inventory process including rotation and restocking of drug dispensing areas with drugs and supplies needed to process patient medication orders.
2. Regulatory compliance - Maintains a safe and clean work area by complying with procedures, rules, and regulations.
3. Communication and teamwork - Communicates with other team members, physicians and patients in an effective and positive manner.
4. Helps prepare intravenous admixtures according to established practices of aseptic techniques and pertinent department policies under the supervision of a registered pharmacist; assists in accurately labeling and dispensing inpatient unit dose medications according to department policy; delivers medications to patient care areas as needed.
5. Assists in compounding medication by means of weighing, measuring, mixing, blending, filtering, distilling, emulsifying and titrating, according to standard pharmacy practices and department policy.
6. Delivers and fills controlled substances and other medications to nursing units' automated medication dispensing cabinets (ADC) timely and accurately; assists in reconciling all narcotic discrepancies on a daily basis.
7. Prepares medications and subsequently loads and unloads medications into ADC with appropriate barcode scanning.
8. Answers phones according to established phone etiquette and directs calls to the proper personnel.
9. Prepares and packages hazardous medications and other medications requiring special handling for subsequent administration to patients under the supervision of the pharmacist.
10. Maintains all necessary paperwork, legal documents, and records for all medications, prepared for administration.
11. Participates in departmental quality-assurance and safety activities for drug usage, infection control, and handling hazardous materials according to state/federal regulations and departmental policy; assists in the collection and review of patient-specific data used by pharmacists for monitoring drug therapy.
12. Assumes responsibility for performance of job duties in the safest possible manner, to ensure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
13. Other duties and/or projects as assigned.
14. Adheres to HMH Organizational competencies and standards of behavior.

A Nuclear Medicine Technologist II ability to operate a PETCT and SPECT/CT unit under the guidelines of the State of NJ.
Maintains additional licensure to perform CT portion of the Nuclear Medicine exam with either an X-ray license of a NJDEP Limited or a Fusion Imaging CT Technology license.

Position Summary
Pharmacy technicians, under the direct supervision of a licensed pharmacist, are responsible for providing technical support for the Department of Pharmaceutical Services. Responsibilities

6. Delivers and fills controlled substances and other medications to nursing units' automated medication dispensing cabinets (ADC) timely and accurately; assists in reconciling all narcotic discrepancies on a daily basis.
7. Prepares medications and subsequently loads and unloads medications into ADC with appropriate barcode scanning.
8. Answers phones according to established phone etiquette and directs calls to the proper personnel.
9. Prepares and packages hazardous medications and other medications requiring special handling for subsequent administration to patients under the supervision of the pharmacist.
10. Maintains all necessary paperwork, legal documents, and records for all medications, prepared for administration.
11. Participates in departmental quality-assurance and safety activities for drug usage, infection control, and handling hazardous materials according to state/federal regulations and departmental policy; assists in the collection and review of patient-specific data used by pharmacists for monitoring drug therapy.

5. Perform basic courier function for the pick-up and delivery of meds and supplies.
6. Prepares individual unit dose packages under the direction of a pharmacist, as needed, with an accuracy of 100%.
7. Receives and records patient`s medications from home, following hospital policy, including recording, storage and destruction, with an accuracy rate of 99%.
8. Operate various IT programs, including Cerner pharmacy system, Omnicell, and retail system with accuracy and efficiency.
9. Prepare retail pharmacy prescriptions under the direct supervision of a pharmacist.
10. Maintains all logs, as required, including, but not limited to the albumin log, IV log, etc.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
