The Oncology Unit Based Pharmacist is a patient-centered, unit-based model of care that integrates pharmacists with responsibility for both the clinical and the distributive activities of the pharmacy department. Pharmacists are actively engaged in medication management with strong focus on areas such as medication utilization assessment, pain management and antimicrobial stewardship , and exhibit a high degree of ownership and accountability for the medication-use process.
The Oncology Unit Based pharmacist, in collaboration with physicians and the interdisciplinary team, supports the multiple oncology practices within the institution.
REQUIRED:
* PGY1 and /or 3 to 5 years of Clinical Oncology experience
* Possess strong interpersonal skills and work well at the team level , as well as independently
* Excellent written and verbal communication skills
* Registered Pharmacist License in New Jersey
* Advanced Cardiac Life Support Certification within 6 months of entering position
PREFERRED:
* PGY2 and / or greater than 5 years of Clinical Oncology experience
* Board Certified Oncology Pharmacist ( BCOP)
* Practice experience in an Oncology inpatient setting
Responsibilities
A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes:
1. Validate medication reconciliation: Reconcile medications upon admission for admitted patients. Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care.
2. Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients. Educate patients about new medications. Counsel patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization.
3. Facilitate discharges: Identify barriers to patient medication adherence and work with the interdisciplinary team to address these issues prior to discharge.
4. Collaborate with the doctors to optimize cost-effective and evidence based: Review and validate all orders prior to the first dose administered. Monitor patient medication profiles daily. Participate in patient care rounds.
5. Drive improved outcomes: Prevent 30 day readmissions, increase HCAHPS scores, and improve patient safety indicators.
6. Other duties and/or projects as assigned.
7. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications
Education, Knowledge, Skills and Abilities Required:
1. Bachelor's Degree from an accredited college of Pharmacy.
2. Possess strong interpersonal skills and work well at the team level, as well as independently.
3. Excellent written and verbal communication skills.
4. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Licenses and Certifications Required:
1. Registered Pharmacist License.
2. Advanced Cardiac Life Support Certification within 6 months of entering position.

The Ambulatory Pharmacist is responsible for dispensing prescriptions and monitoring of medication therapy in accordance with all applicable regulations and standards.
Responsibilities
A day in the life of an Ambulatory Pharmacist at Hackensack Meridian Health includes:
1. Complies with all state and federal regulations and Medical Center policies and procedures relating to drug dispensing, including appropriate handling of drug products, adherence to security procedures, and completion of necessary documentation. This includes appropriate control and dispensing of controlled dangerous substances and investigational drugs.
2. Reviews and dispenses prescriptions and clarifies any and all potential problems with prescriber.
3. Monitors patient drug therapy regimens for clinical appropriateness, including, but not limited to issues such as; allergies, drug-drug interactions, drug-food interactions, potential adverse reactions, appropriate dosing, and compliance.
4. Performs assigned unit and pharmacy inspections to assure proper maintenance and storage of medications, supplies, including emergency drug supplies, controlled substances, and hazardous drugs.
5. Oversees activities of ancillary personnel (Pharmacy Technicians, Liaisons, Navigators, Clerks and Couriers).
6. Employs principles of safe medication practice as dictated by departmental and organizational policies and procedures.
7. Lifts a minimum of 10 lbs., pushes and pulls a minimum of 40 lbs. and stands a minimum of 4 hours a day.
8. Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.) This process includes communicating with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
9. Other duties and/or projects as assigned.
10. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications

1. Graduate of an ACPE accredited school of pharmacy, or equivalent, with a minimum of a Bachelor's Degree in Pharmacy.
2. Excellent written and verbal communication skills.

Education, Knowledge, Skills and Abilities Preferred:

Job Summary and Qualifications
As a Pharmacy Clinical/Operations Specialist, you will be responsible for participating and leading pharmacy clinical or operational programs. This includes monitoring patient medication regimens and providing appropriate interventions. The Clinical Specialist shall also assist in mentoring pharmacy staff and precepting students/residents and assist in the evaluation and implementation of clinical/operational programs.
What you will do in this role:
* As a Clinical Pharmacist Specialist you will interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system
* You will accurately compound and dispense pharmaceuticals
* You will comply with facility policy and procedures and regulatory requirements
* You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
* You will provide clinical consultation and clarification to practitioners. You will suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
* You will sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection
* You will attend and actively participate in daily multidisciplinary and bedside rounds
* You will complete drug utilization reviews to promote rational drug therapy and implement improvements identified
* You will provide accurate and timely medication information to healthcare staff and patients
* You will lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership. Develop, manage and mentor antimicrobial management program (AMP).
What qualifications you will need:
* Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
* Minimum of 3 years' experience as Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required; preferably within area of focus or completion of PGY2 pharmacy residency or fellowship
* AND BCPS certification or other Board of Pharmacy Specialty (BPS) certification
ONE of the following is required prior to the first day of employment:
* Current Registered Pharmacist licensed by the Texas State Board of Pharmacy
* Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy.
* ACLS/PALS and NALS required for critical care/ED position dependent on population served.

Complies with and enforces all state and federal regulations and HMH and site-specific policies and procedures relating to all aspects of pharmaceutical care and medication management.
Responsible for the review and verification of all medications prior to dispensing from the pharmacy department. This review includes, but is not limited to, allergies, polypharmacy, drug-drug interactions, drug-food interactions, and clinical appropriateness. Appropriate communication with the provider will occur if necessary.
Responsible for the oversight of dispensing, chain of custody, and accountability of controlled dangerous substances.
Appropriate handling of investigational drugs at sites where applicable.
Adheres to USP 795, 797, and 800 standards in accordance with current regulatory standards for sterile and nonsterile compounding and hazardous drugs.
Responsible for the oversight of all activities of ancillary personnel (Pharmacy Technicians and Assistant/Couriers).
Employs principles of safe medication best practices as recommended by the Institute for Safe Medication Practices, Joint Commission and outlined in HMH guidelines, policies and procedures.
Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.) This process includes communicating with the patient, parent and/or primary caregiver(s) at their level (developing/age, educational, literacy, etc.)
Provides pharmacological information by researching and answering questions and requests of other healthcare professionals.
Actively participates in pharmacy clinical initiatives including, but not limited to, renal dose adjustments, IV to PO conversions, pharmacokinetic dosing, antibiotic stewardship, opioid stewardship, etc. Pharmacists are available for patient counseling upon request.
Responsible for training pharmacy staff and residents in all areas of pharmacy practice, including the location of medication management resources and guidelines. Accountable for educating pharmacy students and interns while on rotation.
Responsible for completing network and departmental competencies, and licensing and certification requirements.
Performs or oversees assigned unit inspections to assure adherence to medication management standards, including the appropriate storage and security of medications.
Other duties and/or projects as assigned.

Qualifications

The Oncology Clinical Pharmacy Specialist performs a variety of clinical, logistical, and operational functions to streamline care for oncology patients at the MD Anderson Cancer Center at Cooper. The functions include but are not limited to:
Collaboration with interdisciplinary care teams to propose best treatment practices in accordance with the National Comprehensive Cancer Network (NCCN) guidelines and primary literature
Provides clinical pharmacy services for oncology patients, including patient counseling, selection of appropriate therapies, monitoring parameters and frequency of monitoring, management of adverse effects, and identification/ management of drug-drug interactions
Deliver new oncology therapy in-services to the oncology healthcare team
Lead hematology/oncology pharmacology and therapeutics lectures for hematology/oncology fellow physicians
Navigate oncology drug shortages, including recommendation of alternative therapies when appropriate
Precept pharmacy residents and students on ambulatory oncology elective rotation
Build treatment plans for chemotherapy regimens into EPIC and partake in necessary workflows to add new oncolytic agents to hospital formulary
Actively participate in oncology pharmacy & therapeutics (P&T) subcommittee and hospital P&T committee meetings
Assist with medication acquisition for oral and injectable oncolytic therapies, including prior authorizations, appeal letters, and peer-to-peer reviews
Identify and complete patient care quality improvement projects
Assist with policy development with regards to oncology patient care

The Clinical Pharmacist I is responsible for involvement in the clinical aspects of pharmaceutical patient care including interaction with physicians, nurses, and other healthcare providers.
Responsibilities

includes:
Identifies, resolves, and prevents drug-related problems.
Recommends and monitors drug therapy.
Participates in the department's drug formulary review process, including the preparation of drug monographs for review by the Pharmacy and Therapeutics Committee.
Participates in drug guideline and order set development.
Participates in drug research related activities.
Participation on various Medical Center committees and service lines as dictated by specific area(s) of expertise (e.g., cardiology, oncology, pediatrics).
Provides drug information to various health care providers.
Serves as a resource to the department in area(s) of expertise by helping to ensure that drug treatment plans and drug therapies are safe and effective.
Participates in and assists in conducting educational programs for physicians, nurses, pharmacists, pharmacy students, and other health care providers.
Participation in pharmacy student rotation program.
Participates in the Pharmacy Practice Residency program as a preceptor and member of the Residency Advisory Committee.
Assists with staffing as deemed necessary by the Director of Pharmacy.
