Completes discharge plans for each patient's transition to the next level of care. Introduces programs and services designed to meet the needs of the patient after discharge. Facilitates discharge planning group on each unit. Responsibilities A day in the life of a Discharge Specialist at Hackensack Meridian Health includes: 1. In conjunction with the treatment team, formulating discharge plans for patients on the first day of admission for the patient. 2. Begins discharge planning process within 24 hours of assignment. 3. Participates in the treatment planning process. 4. Continuously updates ACT database. 5. Facilitates discharge planning group on a weekly basis on assigned units. 6. Demonstrates positive communication techniques in working with other members of the treatment team in order to enhance patient care. 7. Other duties and/or projects as assigned. 8.
