As a member of the primary health care team, this position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc.. This position provides care to patients with special awareness of vulnerable populations treated in the health centers that may face barriers to care due to residence, health, age, personal characteristics, ability to communicate, chronic illnesses and comorbidities, high ER utilization, developmental status or financial status. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management. Assign and distribute work, review work for accuracy and completeness, and return assignments with recommendations for proper completion. Serve as a leader to youth and staff. Recommend corrective action plans to the BCS Manager as needed. Perform daily facility maintenance inspections. Assigned to mandatory extra shifts when needed.
