The Role:

Essential Functions:
The Hospice RN, Asisstant Director of Nursing (ADON) role is perfect for an experienced hospice nurse who loves people and is committed to providing exceptional care with a touch of hospitality. The ideal candidate will be adept at administrative tasks and comfortable using the Home Care, Homebase EMR system.
Assist the Director of Nursing in the daily operations of the hospice program.
Perform administrative tasks, including documentation, reporting, and compliance monitoring, using the Home Care, Homebase EMR system.
Promotes the philosophy and administrative policies of the Hospice, in an effort to ensure quality of patient care.
Supervises and provides direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients' needs, and continuity of services by appropriate health care personnel.
Provide leadership and guidance to the nursing staff, promoting a positive and supportive work environment.
Ensure high-quality care is delivered to patients by adhering to all policies, procedures, and regulatory requirements.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Participate in the recruitment, training, and evaluation of nursing staff.
Address and resolve any patient or staff concerns promptly and professionally.
Stay current with the latest hospice care practices and regulatory requirements.
Promotes and educates regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
Requirements:
Bachelor's degree in Nursing (BSN) required; Master's degree in Nursing (MSN) preferred.
Current license as a registered nurse (RN) in Texas and/or in accordance with the Board of Nurse .
Minimum of 2 years of nursing experience, with 1 years in a leadership role in hospice or palliative care.
Strong understanding of hospice regulations and standards.
Home Care Home Base experience strongly preferred.
Proven track record of delivering excellent customer service.
Excellent interpersonal skills and ability to communicate effectively.

Key Responsibilities:
A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!
Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.
Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients' needs.
Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
Accepts clinical assignments that are consistent with education and competence to care for patients.
Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
Meets mandatory continuing education requirements of the Agency and licensing board.
Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients' diagnoses and needs.
Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
Requirements
Current PT license
Knowledge or experience utilizing Homecare Homebase EMR would be a plus
Experience in home health or related healthcare setting preferred
Strong clinical and assessment skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively.
Salary Description $50.00-$150.00 per hour

Requirements Responsibilities
Full assistance with Activities of Daily Living (ADLs) including personal care, dressing, hygiene, and mobility
One-person assist with transfers; client can stand and pivot with assistance into his wheelchair
Showering and toileting assistance
Incontinent of bladder
Can toilet independently for bowel movements
Support with communication, companionship, and engaging intellectual conversation
Transportation to and from medical appointments, therapy sessions, and social outings
Errand running, grocery shopping, and light household support related to the client's care
Creating and maintaining a calm, safe, motivating, and supportive daily environment
Monitoring overall well-being and communicating observations as needed
Ideal Candidate
CNA preferred, or highly trained with strong caregiving experience
Post-stroke care experience required
Female caregiver preferred
Mentally resilient, emotionally intelligent, patient, and dependable
Outgoing personality with a warm, motivating, and kind approach
Comfortable communicating clearly, respectfully, and thoughtfully
Values dignity, autonomy, and discretion when caring for an elderly individual
No dementia experience required
Must have a reliable vehicle, valid driver's license, and auto insurance
Position Details
Live-in position: 6 days per week with 1 full day off
Daily personal time provided
All meals included
Private and respectful living arrangement
Caregiver will stay in the client's guest room
Start date: Mid-January
Salary Description $6,000 minimum per month
