Job Summary
The Medical Social Worker is responsible for providing psychosocial support to patients and their families in the Home Health and Hospice healthcare setting. They work collaboratively with medical staff to ensure comprehensive patient care by addressing social, emotional, and financial needs that impact health and wellness. You will play a crucial role in evaluating patients and developing individual treatment plans in collaboration with patient's physicians. Graduate of a school of social work approved by the Council of Social Work Education with a master's degree (MSW) or has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and will be supervised by an MSW. (Diploma or transcripts to be provided to agency)
If education level is baccalaureate, then employee must have one year of experience in a healthcare setting.
Prior Home Care experience preferred but not necessary.
Demonstrate experience in working with patient/families.
Demonstrates good verbal and written communication and organizational skills.
Licensure by the state.
Must maintain current driver's license, auto insurance, CPR card and TB test on file.

Assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.
Carries out social evaluations and plans intervention based on evaluation findings.

Works with family as needed.
Consults and coordinates services with other disciplines.
Provides information to patients or families/caregivers and community agencies.
Serves as liaison between patients or families/caregivers and community agencies.
Maintains collaborative relationships with organization personnel to support patient care.
Maintains and develops contacts with public and private agencies as resources for patient and organization personnel.
Participates in; the development of the total plan of care in consultation with the physician and other care team member and case conferences as required.
Assists physician and other team members in understanding significant social and emotional factors related to health problems.
Participates in discharge planning.

Essential Duties and Responsibilities *
Completes an initial, comprehensive and ongoing comprehensive assessment of the terminal diagnosis on the patient's physical, functional, psychosocial & environmental needs and activities of daily living to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.

Summary
This position implements occupational therapy to patients with acute or chronic physical impairments and/or disabilities through the use of therapeutic techniques to relieve pain, restore physical functioning and facilitate independence. The COTA may also be involved in the prevention of disabilities or injury reoccurrence.
Essential Duties and Responsibilities
The Certified Occupational Therapy Assistant is responsible for carrying out the plan of care as developed and supervised by the Occupational Therapist. These duties include, but are not limited to, the following:
Upon assignment by the OT, contacting the patient within 24 hours and begin therapy as prescribed.
Providing treatment as ordered and approved by the attending physician and Occupational Therapist.

Implementing therapy according to the established plan, utilizing appropriate providers, equipment and techniques within the scope of occupational therapy as delivered by a COTA.
Helping Occupational Therapist to assess patient progress and making recommendations accordingly.
Consulting with other patient care team members to discuss patient progress and integrate occupational therapy with other activities.
Educating the patient and/or caregiver(s) about established goals of the occupational therapy program, plan of treatment, home programs and other related information.
Providing documentation of patient care according to established criteria, including regular case conference notes and submission of caseload rosters per the established schedule.
Practicing in a manner consistent with the professional and Harmony code of ethics.
Maintaining confidentiality in relation to all patients, healthcare staff and documentation; seeking direction from the Occupational Therapist/Harmony Supervisor if asked to share any patient information with outside parties.
Utilizing universal precautions and following OSHA guidelines at all times.
Maintaining and enhancing skills through continuing education.
Appearing professional and complying with the Company dress code.
Participating in department staff meetings, case conferences and in-service programs.

Summary
This position evaluates and treats patients with neurological, musculoskeletal and/or integumentary pathologies and/or other acute or chronic physical ailments through the use of skilled physical therapy techniques used to relieve pain, restore physical functioning and promote independence. Summary

Responsibilities
Perform physical and functional patient assessments
Develop a plan of care with patient goals
Provide evidence-based direct intervention with patients in their place of residence
Perform Home Safety Evaluations
Communicate effectively and respectfully with a diverse patient population and co-workers
Educate patients and family members in home programs, use of the equipment and available resources
Submit required documentation in a timely manner
Qualifications
Active State Physical Therapist License
Current CPR with BLS card
Valid drivers license, at least state minimum auto insurance, and an operational vehicle
Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operations
Great organization skills and an ability to work with minimal supervision
Capacity to appropriately supervise others
Ability to remain calm in emergency or crisis situations
Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
Physical Requirements
Duties require extensive standing, walking, and sitting
Requires lifting, positioning, pushing, and/or assistance with transferring patients
Requires frequent reaching, stooping, bending, kneeling, and crouching

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Rotating weekend on-call schedule
Qualifications
Currently licensed Registered Nurse (RN) in state of employment
Previous experience as a Registered Nurse is preferred
Excellent interpersonal and client care skills
Valid drivers license
First aid/CPR certification required
