Summary This position implements occupational therapy to patients with acute or chronic physical impairments and/or disabilities through the use of therapeutic techniques to relieve pain, restore physical functioning and facilitate independence. The COTA may also be involved in the prevention of disabilities or injury reoccurrence. Essential Duties and Responsibilities The Certified Occupational Therapy Assistant is responsible for carrying out the plan of care as developed and supervised by the Occupational Therapist. These duties include, but are not limited to, the following: Upon assignment by the OT, contacting the patient within 24 hours and begin therapy as prescribed. Providing treatment as ordered and approved by the attending physician and Occupational Therapist. Discussing the plan of care with the occupational therapist and other care-team members. Implementing therapy according to the established plan, utilizing appropriate providers, equipment and techniques within the scope of occupational therapy as delivered by a COTA. Helping Occupational Therapist to assess patient progress and making recommendations accordingly. Consulting with other patient care team members to discuss patient progress and integrate occupational therapy with other activities. Educating the patient and/or caregiver(s) about established goals of the occupational therapy program, plan of treatment, home programs and other related information. Providing documentation of patient care according to established criteria, including regular case conference notes and submission of caseload rosters per the established schedule. Practicing in a manner consistent with the professional and Harmony code of ethics. Maintaining confidentiality in relation to all patients, healthcare staff and documentation; seeking direction from the Occupational Therapist/Harmony Supervisor if asked to share any patient information with outside parties. Utilizing universal precautions and following OSHA guidelines at all times. Maintaining and enhancing skills through continuing education. Appearing professional and complying with the Company dress code. Participating in department staff meetings, case conferences and in-service programs. Assuming other duties, and/or attending other meetings, which are deemed appropriate and necessary by the supervising Occupational Therapist or Regional Director. Qualifications Qualifications and Experience This position requires completion from an accredited COTA instructional program. Must be a licensed COTA in the practicing state. Implement therapy according to the established plan, utilizing appropriate procedure, equipment and techniques within the scope of physical therapy. Observe and report findings about the patient's condition to the attending physician and other agency staff, and document information in the patient's record Potential participation in wound management through wound debridement, dressing changes, exercise, stretching and splinting. Recommend adaptive equipment, complete the equipment justification forms and arrange for the delivery of equipment. Evaluate outcomes of care and assess patient progress and make recommendations regarding discharge. Consult and co-treat with other patient care team members to discuss patient progress, set objectives for patient treatment and integrate physical treatment with other activities. Professional and appropriate communication with other disciplines providing care. Establish and maintain effective communication and a professional relationship with client, families, and coworkers. Educate the patient and/or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of care, home programs, injury prevention and other related information. Prepare clinical and progress notes in alignment with agency policies. Evaluate home and/or work environments and makes physical modifications or ergonomic recommendations. Provide written instructions for the home health aide to promote re-enforcement of therapy services Perform supervisory visits with or without physical therapist assistant or home health aide's presence at least every 10 th visit if there is a designated therapy skill being performed in the home. Understand all treatment and services being supervised. Clinicians working in home health are on the front lines of health care and often practice at the top of their license. Responsibilities Provide physician-prescribed physical therapy under a plan of care established by the PT Provide evidence-based direct intervention with patients in their place of residence Communicate effectively and respectfully with a diverse patient population and co-workers Work under the supervision of a PT: Promptly communicating any patient concerns or changes in status Consulting with PT with regard to suggested changes in treatment Participating in patient and family education as delegated by the PT Submit required documentation in a timely manner Qualifications Active State Physical Therapist Assistant License Current CPR with BLS card Valid drivers license, at least state minimum auto insurance, and an operational vehicle Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing home health operations Great organization skills and an ability to work without direct supervision Ability to remain calm in emergency or crisis situations Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system Physical Requirements Duties require extensive standing, walking, and sitting Requires lifting, positioning, pushing, and/or assistance with transferring patients Requires frequent reaching, stooping, bending, kneeling, and crouching The above statements are only meant to be a representative summary of the major duties and responsibilities performed by the employee in this job.
