STATEMENT OF JOB:
The Personal Care Aide (PCA) provides quality care and assistance with activities of daily living to residents of the community. The PCA encourages residents to function at their highest level of independence and to participate in community activities. Conducts activities and provides for a meaningful experience for all residents. Responsible for ensuring a pleasurable dining experience and marketing the community in a positive and professional manner.
Responsibilities include but are not limited to:
Reports incidents involving residents to include care concerns or issues, to appropriate individuals (ex: HCC, RN/LPN, or ED)
Must maintain a working knowledge of all policies and procedures pertaining to the position
Assists residents with activities of daily living
Perform housekeeping duties, such as making resident beds, cleaning resident apartments, common areas, dining room, etc. as directed by supervisor
Respond promptly to all resident calls for assistance or emergency alerts
Must be able to remain calm and competent during emergencies
Assists in planning and conducting activity programs for residents
Follows confidentiality policies regarding release of resident information
Assists residents to and from the dining room, and to and from activities
Checks on residents, ensures attendance at meals and activities
Checks on residents who are ill and reports status or concerns to appropriate individuals
Communicate verbally and in writing to report resident status, upcoming community information, resident appointments, admissions, and transfers
Assists new residents with move-in transition: provide emotional support
Other duties as assigned

Position Purpose
The ALC & DSH CNA/Transportation Professional supports Villagers during medical appointments by advocating for their needs, assisting with the appointment process, and empowering them to make informed decisions about their care. This role ensures Villagers receive the necessary support while respecting their autonomy in healthcare choices.
Required Qualifications
CertifiedNursingAssistant (CNA) in good standing, meeting all relevant Georgia and federal requirements
High school diploma or GED
Proficiency in English (read, write, follow instructions)
Valid Class C Georgia Driver's License, proof of insurance, and acceptable driving record
Successful completion of background check, drug screen, and TB screening
CPR/First Aid certification

Key Responsibilities
Transportation & Medical Appointment Support (Primary Focus)
Provide safe, timely, and compassionate non-emergency medical transportation (NEMT) for Villagers to off-campus doctor visits, therapies, labs, hospital outpatient appointments, pharmacy runs, etc.
Use accessible practices (ramps, lifts, securement) as needed, ensuring that each Villager's mobility needs are addressed and accommodated
Offer emotional support, reassurance, and assistance to Villagers before, during, and after appointments to reduce stress and promote positive experiences
Advocate on behalf of Villagers-help them communicate needs or concerns to medical staff, assist with check-ins, accompany them as needed
Maintain detailed trip logs, mileage records, and required documentation of each outing
Monitor vehicle condition: ensure cleanliness, appropriate fuel levels, safety checks, and report any mechanical or safety concerns
Plan routes to optimize time and safety (considering traffic, Villager comfort breaks, accessibility)
Coordinate pick-up and drop-off times with the care team to minimize waiting and overlap
Follow all vehicle, traffic, and safety regulations; secure mobility devices, seat belts, restraints appropriately
Support Role in Care (Secondary, When Off-Transport Duty)
When not transporting, assist as needed with personal care tasks such as grooming, toileting, dressing, positioning-always in a supportive role and in a way that honors Villager choice
Be available to help fill in gaps in care, offer respite support, or respond to situations during non-transport periods

Position Purpose
Performs non-professional direct care duties under the direction and supervision of the LPN Charge Nurse and assists in maintaining a positive physical, social, and psychological environment for the residents.
Required Qualifications

Provides nursing functions as directed by the LPN Charge Nurse including daily perineal care, catheter care, turn residents in bed, sponge baths, measure and record temperature, pulse and respirations, weigh and measure residents, perform restorative and rehabilitative procedures and observe and report presence of skin breakdown.
Monitors all clinical documentation to ensure accuracy, timeliness, and completeness.
Report changes in resident conditions to the charge nurse; and document all necessary information and data and report all accidents, incidents, and safety issues.
Prepares residents for meals and snacks; identifies food arrangement and assists in feeding residents as needed; records food intake; and performs after-meal resident care.
Maintains resident confidentiality; treats residents with kindness, dignity, and respect; knows and complies with Resident's Rights rules; and promptly reports all resident complaints, accidents, and incidents to the charge nurse.
Assists as directed in proper admission, transfer, and discharge of residents; inventories resident possessions and reports food articles and medications found in resident rooms; and reports defective equipment to administration.
Understands and uses Universal Precautions and follows established infection control, hazardous communication, and other safety rules; ensures cleanliness of assigned residents' rooms; properly maintains and records resident restraints; and promptly reports all violations of safety and sanitation rules to the charge nurse.
