JOB SUMMARY: Sycamores is partnering with the 988 mental health crisis hotline to provide support to people experiencing a mental health crisis. The 988 hotline will provide initial phone support to all callers and triage calls to the Sycamores Mobile Crisis Outreach Program for in-person support. The Mobile Crisis Outreach Program responds 7 days a week from 7:30pm to 8:30am, broken down by flexible shifts. The Mobile Crisis Outreach Clinician will respond, in person, to provide timely support including crisis stabilization, safety planning, assessment for danger to self/ others, and gravely disabled write 51/50 holds as needed and provide follow up support. Utilizing the Mobile Crisis Outreach Program vehicle to respond to calls, this role co-responds with a Mobile Crisis Outreach Program Peer Support Specialist and is an integral part of the Sycamores larger Mobile Crisis Outreach Program including close partnership with the 988 call center, Riverside County Dispatch Center, police, sheriff, psychiatric hospitals and ambulance transport companies.
The Mobile Crisis Outreach Department evaluates the individual experiencing a "mental health crisis" out in the field and takes measures to ensure the individuals safety (i.e., Evaluates if the individual requires involuntary hospitalization to ensure their safety). The Mobile Crisis Outreach Clinician takes the lead on care coordination and works collaboratively with the Mobile Crisis Outreach Program Peer Support Specialist to coordinate care i.e., locate hospitals, ambulances, housing, etc.). The Mobile Crisis Outreach Program responds to an individual's need for in-person evaluation for safety within specified geographical areas (i.e., Desert, Mid-County and Western Regions).
Starting Pay Range:
$93,150 - $100,136.25 per year for Full-Time employment (starting pay will be based on previous work experience and educational background.)
Shift Schedule: Wednesday - Saturday 6 pm - 4 am
JOB QUALIFICATIONS
Masters' Degree in related field required.
Licensed in the State of California as LCSW, LMFT, LPCC or Psy. D/Ph. D required. • Must maintain relevant licensure as a condition of employment.
2 + years of successful licensure and experience preferred.
Attend and satisfactorily complete all trainings required by the Agency, federal, state and/or local regulations.
RUBHS experience preferred.
Valid California Driver License and a driving record acceptable to the Agency's insurance carrier.
Ability to use agency vehicle with appropriate insurance for all transporting consumers and adhering to federal, state, and local motor vehicle safety standards when transporting children.

JOB SUMMARY

Participates in information gathering and decision making related to involuntary psychiatric holds as Lanterman Petris Short laws and regulations.
Provides direct billable mental health services, documents in alignment with MediCal regulations, and achieves set productivity expectations.
Engages and supports family members and other natural supports in the midst of an acute psychiatric crisis which includes psychoeducation related to de-escalation strategies, steps to secure home environments specifically related to weapons, dangerous items, substances that could put the client at risk of harm to self or others.
Facilitates transport of individuals on 5150 holds including providing clinical information to accepting hospitals or CSU/other psychiatric facilities. Remains with client until transport has arrived and client safety enroute to accepting facility.
Assists individuals with identifying triggers, unmet needs, psychosocial factors that are contributing to ongoing acute mental health crisis. Utilize strengths-based approach to develop a comprehensive risk management and safety plan to support the client in stabilizing the behaviors that may present as danger to themselves others after the conclusion of service.
Provides linkage, referrals and resources in the community based on identified needs which may include but not limited to mental health services, substance use services, housing supports, and additional identified social services.

JOB SUMMARY

Under general supervision, implements the Addiction Prevention Services (APS) at one or more of the following settings: designated schools, in the community, with the family, or on site at and agency campus. Fulfills a role in the collaboration and communication between the Agency and the assigned school or program and its respective Supervisor. The APS II has the ability to provide Level I & II prevention activities.
RESPONSIBILITIES AND DUTIES
Serves as an active participant in providing Pacific Clinics APS services with the school or programs core team (or equivalent) for clients referred to APS.
Works in collaboration with the referral source to identify and provide relevant presentation to groups (e.g., students, parents, teachers, support staff) as appropriate.
Co-leads APS groups following the designated curriculum.
Receives, reviews, and processes referrals of clients as necessary. Screens each client referred for APS services with the support of the Supervisor/Manager.

Initiates and receives written consent for each participant .

About our Program: Mobile Crisis services for Solano County will include the provision of emergency crisis intervention services to Solano County residents - both youth and adults - who are acutely suicidal, homicidal, or gravely disabled. Services include phone crisis screening and triage; in-person crisis evaluation and crisis intervention services; and linkage to an appropriate level of follow-up service including, but not limited to, referring individuals to the crisis stabilization unit (CSU) or a local emergency department (ED).The Mobile Crisis Unit will deliver - in partnership with local law enforcement, the CSU, local EDs, and Solano County Behavioral Health (SCBH) - a coordinated system of crisis services.
Works within the vision, mission, and philosophy of the agency. Within a team process, develops a strength-based relationship with client in the context of a mental health or behavioral crisis. Intervenes with individuals presenting with crisis behaviors including but not limited to agitation, aggression, active psychosis, suicidal ideation and/or self-injury. The position requires shift-based work in which individuals are responding to community or Crisis Stabilization Unit based crisis on immediate emergency requests from individuals, community partners, law enforcement or family members/natural supports. Responsible for crisis de-escalation and stabilization in situations that have the potential for becoming volatile in nature. Responsible for linking to community resources. RESPONSIBILITIES AND DUTIES
May support mental health crisis hotline by gathering information, supporting callers and identifying needs related to current crisis situation. May provide phone stabilization and resourcing as needed. Provides crisis assessment and de-escalation of adults or youth who are experiencing an acute mental health or behavioral crisis and may be presenting as a danger to themselves or a danger to others.
Participates in information gathering and decision making related to involuntary psychiatric holds as Lanterman Petris Short laws and regulations.
Provides direct billable mental health services, documents in alignment with MediCal regulations, and achieves set productivity expectations.
Engages and supports family members and other natural supports in the midst of an acute psychiatric crisis which includes psychoeducation related to de-escalation strategies, steps to secure home environments specifically related to weapons, dangerous items, substances that could put the client at risk of harm to self or others.
Facilitates transport of individuals on 5150 holds including providing clinical information to accepting hospitals or CSU/other psychiatric facilities. Remains with client until transport has arrived and client safety enroute to accepting facility.
Assists individuals with identifying triggers, unmet needs, psychosocial factors that are contributing to ongoing acute mental health crisis. Utilize strengths-based approach to develop a comprehensive risk management and safety plan to support the client in stabilizing the behaviors that may present as danger to themselves others after the conclusion of service.
Provides linkage, referrals and resources in the community based on identified needs which may include but not limited to mental health services, substance use services, housing supports, and additional identified social services.

Independently coordinates the integration of comprehensive Functional Behavior Analysis and plans into overall child and family service plans with family members, referral sources, collateral contacts, community resources and other agencies and support persons involved with a child's plan for stabilization and wellness.
