Responsibilities
Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care.
Identify teaching needs and goals related to each patient's diagnosis in collaboration with the patient, family, and interdisciplinary team.
Document all assessments, interventions, and patient responses accurately and timely in the medical record.
Participate in the patient rehabilitation program and ensure direct patient care utilizing the nursing process.
Initiate discharge planning and patient education from the day of admission.
Communicate changes in patient status effectively to all care providers.
Provide supervision and direction to ancillary nursing personnel.
Ensure personal and environmental safety according to hospital policies.
Contribute positively to the unit's functioning and participate in team building activities.
Assume charge responsibilities as needed after orientation.
Engage in professional growth and continuing education opportunities.
Required Experience / Certifications / Licensure
BLS Certification required prior to the first shift.
ACLS Certification required prior to the first shift (CIRH ONLY).
Minimum two (2) years of bedside clinical experience is highly desired.
ICU and critical care experience required.

Licensed Practical Nurse (LPN/LVN)
We are seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our healthcare team at our senior living community. The LPN will be responsible for providing direct patient care under the supervision of a registered nurse or physician. The LPN will also be responsible for administering medications, monitoring vital signs, assisting with patient assessments and care plans, and overseeing the patient care provided by caregivers.

Shift Details:
We are currently searching for an LPN to join our team for PRN/PT and have day/afternoon/evening openings available. Please note, shifts may change throughout employment as needed with notice.
Primary Responsibilities:
Administer medications and treatments as prescribed by a physician or registered nurse
Monitor and record vital signs, such as blood pressure, temperature, and pulse
Assist with patient assessments and care plans
Provide direct patient care, including bathing, dressing, and feeding
Assist with diagnostic tests and procedures
Document patient care and progress in medical records
Communicate with patients and their families about care plans and progress
Collaborate with other healthcare professionals to provide comprehensive patient care
Qualifications, skills, and abilities:
Active LPN license valid in the State position is located.
Minimum of 1 year of experience as an LPN
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong attention to detail and organizational skills
Ability to prioritize and manage multiple tasks
Basic computer skills
Ability to treat residents, families and team members with respect and compassion.
