    Role and Responsibilities Provide personal care and supportive services to individuals in the community, in the privacy of their home. Essential Functions Provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health. Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home. Immediately report changes in client's condition to supervising professional. Ensure the safety of the client, client's family and/or the employee. Maintain HIPAA. Complete and submit necessary patient documentation including charting tasks and observations on android device. Attend ongoing in-service programs in a timely manner, to maintain certification. All other duties as assigned. Education Requirements High School diploma or GED preferred. Completion of an approved program of PCA or HHA certification as designated by NYS Department of Health. Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program. ) Must have worked in a certified HHA or PCA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry). Work Environment The HHA/PCA is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work: Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Maintenance of a clinic panel of people living with HIV and other patients appropriate to ongoing infectious disease oversight. Prescribe or administer medication, therapy and other specialized medical care to treat or prevent illness, disease or injury. Explain procedures and discuss test results or prescribed treatments with patients. Monitor patients' conditions and progress, re-evaluating treatments as necessary. Provide health and wellness advice; order age-appropriate screenings, immunize patients against preventable diseases. Maintain an ongoing relationship with back-up hospital and its professionals. Ultimately, have admitting privileges at that institution.
