Job Role Summary
The Discharge/Planner Social Service Coordinator is responsible for providing comprehensive social services which assist patients, clients and their families to find practical ways of overcoming social, emotional and environmental barriers to optimum functioning and attainment of health. The Discharge/Planner Social Service Coordinator alters activities/behaviors to reflect and ensure adequate care and services appropriate to the age of patients served (i.e. neonatal, infant, pediatric, pre-school, school age, adolescent, adult, geriatric).
Essential Functions and Responsibilities
Interviews hospital patients and their families during the course of an inpatient or observation admission
Responsible for managing assignment of specified unit or clinic, ensuring that 100% of patients with an identified need have been assessed and evaluated
Initiates social service plans for patients/clients through professional collaboration with medical, nursing and health care personnel utilizing screening questionnaires, diagnostic assessments and individualized therapeutic care plans
Coordinates the overall interdisciplinary plan of care for a patient from admission to discharge
Serves as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively
Coordinates discharge plans to meet the patients social and emotional needs to ensure a safe and effective discharge
Assists patients and families in understanding the implications and complexities of their medical situation and its impact on their current lifestyle
Provides direction in the selection, analysis, summarization and presentation of patient/client and related information for service delivery planning, case conferences and professional consultation
Provides complex social service intervention to individuals, families and groups ranging from diagnostic assessment, short term counseling, crisis intervention, community resource referrals, client advocacy, support, outreach, follow-up and intra/inter agency consultations
Provides education and training to introduce or improve coping skills of individuals and families
Documents services rendered in clinical records
Responsible for following the required assessment format to ensure all elements for effective discharging have been met with the ultimate goal of reducing readmissions
Responsible for partnering with the multidisciplinary team to ensure financial resources have been assessed and a determination made to ensure there are limited placement barriers post discharge
Conducts social service orientation for physician, nurses, and students as appropriate
Utilizes problem-solving skills to identity and facilitate problem resolution in team functioning
Participates in staff meetings and education programs for staff and community agencies as assigned
Assumes responsibility for continuous professional self-development

Job Requirements
Minimum of a Master's of Social Service or related field degree including but not limited to a MSW
Social service experience in a healthcare setting dealing with pediatric, adolescent, adult and geriatric patients is preferred
Knowledge, Skills & Abilities
Effective communication skills
Knowledge and skills necessary to provide care appropriate to the age of patients served. Must demonstrate knowledge of growth and development principles over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age-specific needs. Ability to use age appropriate communication and skills detailed in the department/area/unit job specific/age specific competencies
Analytical, psychosocial assessment and counseling skills
Knowledge of pertinent State/Federal laws and regulations, community systems and resources
Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, initiative

Develop safety plans as needed to address immediate concerns and stabilize risks.
Identify cases requiring further evaluation, including referrals for psychological testing and discovery groups, to ensure comprehensive care.
Encourage participation in group activities, peer support, and community resources to supplement individual treatment.
Maintain consistent communication and documentation, including face-to-face and phone contacts, adhering to clinical standards and agency policies.

Uphold licensure, certification, and ethical standards in all professional activities.
Complete mandatory training annually and stay current with best practices and evolving guidelines.
Travel as needed within the service area, with mileage reimbursement provided at IRS rates when using personal vehicles.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution.
Valid LMWS license to practice in Michigan.

Carries out social evaluations and plans intervention based on evaluation findings.
Maintains clinical records on all patients referred to social work.
Prepares clinical and progress notes.
Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
Perform all duties with a passion for compassion.
Who you are?
Graduate of a master's program in social work accredited by the Council on Social Work Education.
Licensure by the state of Michigan.
Minimum of one (1) years' experience in health care.

**Department:** Psychiatric Unit **Hours:** 16 **Shift:** Afternoons (United States of America) As a SocialWorker, you will coordinate the facility placement, community agency referral and social coping aspects of your assigned patients.
You will complete an initial assessment and compile discharge plans reflecting clinical and social needs by collaborating with the patient, their family and medical staff. In the discharge placement options, you will educate patients and families about facility placement and work with community mental health centers.
In this role, you assess patients' suicidal/homicidal potential, take appropriate actions and implement seclusion and restraint procedures as needed.

**ACCOUNTABILITIES**
+ The licensed socialworker demonstrates knowledge of the principles of growth and development, the ability to assess and interpret age/developmental specific data, make treatment recommendations, and evaluate patient/family response to treatment. Provides appropriate data to other caregivers and involves patient (patient centered care), family, significant others, and community providers in decision-making related to plan of care as appropriate, when indicated, and according to department/hospital policies and procedures. The patient population served includes: infants, children, adolescents, adults, and geriatric patients.
+ The licensed socialworker completes timely psychosocial assessments for patients admitted to the Coping Center and having a length of stay of 24 hours or more. The licensed socialworker plans interventions and implements psychosocial interventions based on patient assessment of needs and strengths.
+ Performs individual, couple, family, and/or group counseling for patients on the unit and elsewhere in the hospital, according to patient need. Utilizes evidence-based approach. Performs social psychotherapy under supervision according to social work scope of practice requirements.
+ Assists in scheduling family, marital or significant other meeting with L.I.S.W.-S when indicated.
+ Initiates discussions with the physician and treatment team regarding potential patient discharge plans. Facilitates patient discharge plans per approval of the multidisciplinary team.
+ Demonstrates knowledge of community resources and assists with development of referral processes. Provides educational information re: community resources, coping skills, etc. to patients, families, and other staff.
+ Documents patient observations, activities, and data collected in the electronic medical record and/or on paper forms per current process. Documentation is accurate, clear, legible, descriptive, using correct forms, terminology, and abbreviations. All entries are signed, dated, and timed. Errors are handled appropriately. Documentation reflects concise, efficient use of time. Documents timely individual, group, and counseling note on each session conducted.
+ Provides assistance to facilitate the therapeutic transfer of patients from one hospital unit to another unit or to other hospitals/agencies.
+ Completes authorization forms for release of patient information. Notifies case managers and/or prehospitalization care providers of patient's admission (with consent of the patient). Maintains communication with external care providers. Complies with HIPAA regulations.
+ Participates as directed, in making post-discharge follow-up contact with patients.
+ Attends and participates in a variety of meetings including multidisciplinary team, community, and departmental meetings as directed.
+ Safeguards hospital supplies, property, and equipment from damage, misuse, or loss. Efficiently uses supplies and inventory.

+ Reports promptly any problem related to patient care, or hospital activity. Reports hazards promptly.
+ Handles oneself in a professional and ethical manner. Reports any known or suspected improper business practices to the Hospital Compliance Officer or to any administrative staff member.
+ Assists in the assessment of quality and appropriateness of Coping Center programs and levels of patient satisfaction. Participates in monitoring, evaluating, and modifying social work programs. Actively participates in hospital-wide quality and safety programs.
+ Supports and builds teamwork/communication within the department and between other departments. Responds appropriately to visitors, patients, and public.
+ Performs other related duties as assigned.
**JOB REQUIREMENTS**
+ **Education:** Must possess a Bachelor of Social Work, or comparable social work degree.
+ **Skills:** Extensive knowledge of the community and discharge planning required. Strong interpersonal skills to work independently. Knowledge of community resources and/or federal regulations.
+ **License:** Licensed SocialWorker (LSW) (Must meet all Ohio Counselor, SocialWorker, and Family Marriage Therapist Board requirements for licensure as a licensed socialworker).
+ **Certification:** Certified to provide substance abuse treatment services. Employees hired prior to 3/1/18, will not be required to obtain the certification. Anyone hired after 3/1/18, will have up to 12 months to obtain the certification. Maintain CPR certification.
