**Department:** Administration **Hours:** 40 **Shift:** Days (United States of America) As the Pediatric Primary Care Social Worker, you will provide proactive care management for high-risk pediatric patients and their families by addressing psychosocial issues that may affect medical care and health outcomes.
You will interview pediatric patients, families, and caregivers to assess personal and social adjustments and look for any needed interventions to mitigate risks. Once identified, you will deliver interventions and connect patients to appropriate resources.
In this role, you will focus on supporting families in managing complex conditions hindering overall well-being.

REQUIREMENTS
+ Education:Master's Degree in Social Work
+ Skills:Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in self-directed environment with attention to detail, follow through, and has a high level of computer literacy. History of positive rapport with patients, families, physicians, and interdisciplinary team.
+ License: Active LISW license
PREFERRED REQUIREMENTS
+ Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty.

You make your own schedule, and work can be done remotely, except the ADOS appointment does need to be conducted at our Kalamazoo East center. Position requires LCSW to complete a minimum of 1 assessment per week with report writing (approx. 10 hours).
