Performs social psychotherapy under supervision according to social work scope of practice requirements. + Assists in scheduling family, marital or significant other meeting with L.I.S.W.-S when indicated. + Initiates discussions with the physician and treatment team regarding potential patient discharge plans. Facilitates patient discharge plans per approval of the multidisciplinary team. + Demonstrates knowledge of community resources and assists with development of referral processes. Provides educational information re: community resources, coping skills, etc. to patients, families, and other staff. + Documents patient observations, activities, and data collected in the electronic medical record and/or on paper forms per current process. Documentation is accurate, clear, legible, descriptive, using correct forms, terminology, and abbreviations. All entries are signed, dated, and timed. Errors are handled appropriately.
