The speech and language pathologist accurately assesses patients' needs; identifies and initiates appropriate speech and language therapy interventions; provides patient and family teaching; and works cooperatively with other patient team personnel to maintain standards of professional speech and language therapy practice. Qualifications: Required: Graduate of an accredited program of speech-language pathology Certificate of Clinical Competency (CCC) in Speech Pathology from the American Speech and Hearing Association (ASHA). -OR- Clinical Fellowship Year (CFY) with completion of Certificate of Clinical Competency within three months of completing Clinical Fellowship Year. HOW YOU'LL WORK: You'll provide home-based skilled speech therapy care for patients as directed by the attending physician. You will educate patients and their family members and ensures the safety of the patient. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function. Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision. Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service training and mandatory agency meetings. HARD & SOFT SKILLS: Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions. REQUIREMENTS: Speech Therapist with a current license in the state of employment. Minimum one-year experience as a ST in an acute care setting. Home care experience preferred. Demonstrates skill in appropriate selection, administration and interpretation of standardized and non-standardized assessments using age specific criteria for each age population served. + Develops and implements treatment plan according to a valid prescription and in accordance with policies and procedures. + Establishes a plan of care for each patient including mutually established goals, taking into account patient age and educational level. Progress toward goals will be documented and reported to the physician per departmental and applicable regulatory policy. + Prepares and maintains accurate patient care records according to departmental protocols and demonstrate basic competency for completing accurate billing and electronic documentation. + Demonstrates knowledge and skill to utilize departmental equipment safely and appropriately for patient care activities. + Maintains clean and safe work environment to comply with hospital and department safety standards. + Demonstrates professionalism in clinical practice.
