As the Family Support Specialist (FSS), you will initiate and maintain regular contact with families, primarily in their homes. In this role, you will: Conduct client home visits on a regularly assigned schedule with the goal of establishing trusting relationships in accordance with Healthy Families America Best Practice Standards. Prepare appropriate activities for home visits using approved evidence-based curriculum and other supplemental materials. Prepare appropriate activities and facilitate group socialization experiences and field trips for families. Provide appropriate parenting modeling, education, and other supportive services as outlined in the Healthy Families America Best Practice Standards. Conduct developmental screening and evaluations using instruments such as ASQ-3, ASQ-SE-2, PHQ-9 Depression Screens, and CHEERS Check-in with families. Make referrals to other services as needed and work with other community providers to provide collaborative services to clients. As needed, use bilingual skills to facilitate communication between participant families and other program staff, providers, and/or partners. Prepare documentation and maintain client files in accordance with program requirements and NVFS guidelines. Enter data for record keeping and reporting to meet federal/state/program guidelines. Participate in regular supervision with Program Supervisor, providing regular updates and preparing case presentations. Maintain current own level of knowledge on program software and other information on early childhood programs. Your Background: We understand that you may obtain skills through a variety of professional, personal, educational, and volunteer experiences. Oral and written fluency in English; oral fluency in Spanish required High School graduate or GED; college coursework preferred Infant Mental Health endorsement preferred Experience working with or providing services to children (aged 0-3 years) and families; this may include volunteering, internships, and/or personal lived experiences Must be willing and able to provide services in accordance with Healthy Families America Best Practice Standards Experience and humility to work with the culturally diverse populations present among the program's target population Knowledge of infant and child development Ability to establish trusting relationships with an acceptance of individual differences Willing to engage in building reflective capacity (i.e., has the capacity for introspection, communicates awareness of self in relation to others, and recognizes the value of supervision) Proficiency in usage of basic technological tools including laptops/computers, email, phones, and internet; working knowledge of Microsoft Office Suite Must be able to demonstrate and live the organizational values of Communication, Integrity, Respect, Collaboration, and Accountability Additional Requirements: Successful completion of background checks upon hire Must be able to lift up to 25 pounds of baby items and program materials. In this role, you will: * Conduct intake for all referred clients. * Development of the Individualized Family Service Plan. * Provide case management services to assigned case load, in accordance with Service Delivery Model, including assessment, goal plan development, and referral. * Assisting individuals and their families with access to services and supports. * Provide life skills education and intervention in accordance with clients' goal plan and resource needs. * Support clients in recognizing signs and symptoms of mental health and substance use issues and provide guidance on seeking appropriate care. * Provide psychoeducation on mental health and substance use in accordance with clients' goal plan and resource needs. * Liaison between the family and service providers. * Participate actively in team meetings and collaborative service delivery models with other team members to effectively address community, family, and individual needs. * Initiate and maintain collaborative contact with other agencies working with the youth and their family, including the referral source, and appropriate school personnel in order to effectively coordinate service delivery and monitor clients' progress towards goals. * Develop and maintain collaborative relationships with other community services providing services for youth and families. * Participate in regular individual and group supervisions as directed by supervisor. * Assure confidentiality policies and procedures are followed to protect the rights of clients, that reports to authorities occur in the mandated time frames and that critical incident reviews related to these procedures are completed and filed. * Maintain client files in accordance with program/licensing requirements and NVFS standards. * Prepare statistical and narrative reports as required.
