Contract Coordinator & Collaborating Physician Liaison Job Responsibilities:
The Contract Coordinator & Collaborating Physician Liaison plays a vital administrative role in supporting the Company's clinical and operational teams. This position serves as the primary liaison for collaborating physicians, ensuring seamless communication, efficient onboarding and offboarding, and ongoing compliance with program requirements. In addition to physician support, the role includes contract preparation, tracking, and file management in coordination with the Contract and Business Development department. Success in this role requires strong organizational skills, attention to detail, and the ability to manage multiple administrative functions across departments.
Contract Administration
Draft, format, and manage various contracts including Service Agreements, Business Associate Agreements (BAAs), vendor contracts, Affiliation Agreements, and Collaborative Practice Agreements (CPAs) using approved templates.
Coordinate contract execution by tracking edits, circulating documents, and ensuring timely completion.
Maintain organized electronic records, tracking sheets, and CRM/CLM systems for all executed contracts.
Collaborate with Divisional Directors, Client Onboarding Manager, and Business Development team to ensure contract compliance and smooth client transitions.
Collaborating Physician Liaison
Serve as the primary administrative contact for collaborating physicians, ensuring consistent support and communication.
Manage onboarding/offboarding processes, training coordination, chart review tracking, insurance updates, and roster maintenance.
Maintain and update the Collaborating Physician Handbook and related documentation.
Prepare CPAs based on new hire alerts and specific requests using standardized templates.
Monitor chart review deadlines and send timely reminders to collaborators.
Maintain credentialing and collaboration documentation in the HUB with standardized naming conventions.
Communicate with the Invoicing team regarding collaborating physician payments.
Implement and monitor state-specific quality assurance requirements.
Stay current with prescriptive authority regulations for Nurse Practitioners in each practicing state.
Update CPA templates with current state requirements and physician information.
Coordinate with the Credentialing department to ensure timely execution of CPAs for provider integration.
Cross-Departmental Coordination and Support
Provide support to Credentialing team, Collaborating Physicians, Medical Director, and client implementation team as needed.
Collaborate with invoicing to ensure timely and accurate payment of Collaborating Physicians.
Process & Continuous Improvement
Maintain standardized contract templates and clause libraries.
Identify workflow efficiencies and propose system/process improvements.
Contract Coordinator & Collaborating Physician Liaison Qualifications and Requirements:
Bachelor's degree or equivalent experience (minimum four years in a related a health-related, administrative or legal support role) preferred
Effective and exceptional written and verbal communication skills, with the ability to convey complex information clearly and professionally across diverse teams and stakeholders.
Effective interpersonal skills and proactive approach to problem-solving.
Strong understanding of contract language and document handling.
Excellent organizational and file management skills.

Payer Enrollment Specialist Job Responsibilities:
The Payer Enrollment Specialist will oversee our provider's payer enrollment and revalidation process. They will be responsible for their assigned state rosters, and payers per state where our providers see patients. They will report to the Manager, Payer Enrollment.
Maintains the credentials of new and existing physician and mid-level providers, their medical licenses, DEA certificates and managed care applications.
Responsible for entering in ongoing data and supporting documentation for all providers within the internal credentialing and CAQH databases to ensure all expirable/personal information updates are current and correct.
Responsible for all CAQH (re) attestation requests, new location entries, and updating annual professional liability insurance renewals.
Acts as liaison between practitioners and payers, identifies delays with payers, escalates and communicates delays or issues timely.
Administers payer website access (government, third party insurance, third party websites) for new and existing business office staff.
Complete payer enrollments for ERAs and EFT and maintains database of payers enrolled.
Works with leadership to apply and maintain “in-network” status with insurance companies for practice(s) and providers, as well as maintains a current provider roster list with in-network providers.
Audit our payer rosters and make sure we are revalidating our providers with their payers.
Works on both initiating and completing the contract process for new payers to make we are enrolling appropriately.
Investigate and work through any credentialing issues as identity by the Revenue Cycle Team.
Communicate to all stakeholders about approved enrollments, denied contracts, closed networks, and other items that will help our providers, leadership, Revenue Cycle Team, and others make appropriate business decisions.
Assist the Credentialing team on packets or projects as needed.
Credentialing Specialist Qualifications and Requirements:
High School Diploma, required, bachelor's degree preferred
A minimum of one year of relevant experience in credentialing, provider enrollment, or related field required.
Excellent interpersonal skills
Excellent communications skills both verbal and written.
Excellent organizational skills and an ability to manage follow-up efforts for multiple tasks.
Focused attention to detail to ensure accuracy and timeliness of credentialing information.
Ability to work under time constraints to meet deadlines.
Proficient use of Microsoft Office applications (Excel, in particular), internet resources and Adobe Acrobat.

Plan, direct and evaluate resident and employee health care in long-term care

Regional Float Nurse Practitioner Job Responsibilities:
The Regional Float Nurse Practitioner will float in a designated region covering schedules, filling in for the local team and might have some consistent nursing facilities. The Regional Float Nurse Practitioner will plan, direct, and evaluate resident and employee health care in long term care; examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses; performs selected laboratory testing; offer referrals to physicians, write prescriptions, and perform bedside procedures within the scope of expertise, protocols, and available equipment. The Regional Float Nurse Practitioner will also work with the medical director in the care and treatment of residents and staff.
Debridement of wounds by sharp, enzymatic, autolytic or surgical methods able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
Application and management of topical wound care and associated wound management products.
Direction and recommendation of associated services to improve wound healing outcomes. This includes but is not limited to labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
Interpretation of diagnostic tests and recommendation of appropriate follow up.
Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines
Education of patients as to the topical and systemic management of their wounds.
Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
Provision of regular updates to the PCP, CM, or referring Physician on the status of their patient's systemic conditions associated with wound care.
Acts as a resource person for decisions, planning, and policies affecting the health of residents.
Collaborates with staff to identify residents' needs and to help develop plans to meet these needs.
Regional Float Nurse Practitioner Qualifications and Requirements:
Successful completion of an approved nurse practitioner program.
Current approval by the Board of Nursing to perform medical acts.
Must be currently licensed as a registered nurse in the state.
Current Active, unrestricted APRN license in the state in which the applicant has applied.
Master's degree of Nursing specializing in Nurse Practitioner programs.
Physical Requirements
:
Able to stand and walk for 4+ hours
Able to lift 100 pounds to assist fallen residents.
Able to lift 10 pounds for routine performance of essential functions.
Pushing/Pulling 70+ pounds when administering patient care
Bending/squatting/kneeling/twisting when administering patient care
Hearing in order to perform physical assessments (aids permitted)
Vision for performing physical assessments (glasses or contacts permitted)
Depth perception when performing medical procedures
Fine motor skills for grasping and controlling medical equipment, and to perform precise procedures such as sterile dressing changes
Travel Requirements:
This position requires regional travel a minimum of 4 days a week.
Travel maybe scheduled last minute at times if coverage is needed.
May require overnight stays
Work Environment:
May be exposed to adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.

Work Location: On the road, travel to Skilled Nursing Facilities across the defined region.
