SUMMARY
The Licensed Practical Nurse (LPN) provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM within specified geographical territory. The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.
QUALIFICATIONS & ATTRIBUTES:
Licensed Practical Nurse with current license in the state of employment
Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.

MAJOR AREAS OF RESPONSIBILITY:
Provides skilled nursing care in a patient's home as ordered by the attending physician
Performs care by following order sets and goals made by RN
Observes and monitors patient conditions
Notifies RNCM of any changes in patient condition
Administers medication as prescribed by the physician
Counsels patient and family on the disease/injury processes and how to manage
Incorporates patient and family in development of plan of care
Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
Listens to patient and family members
Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
Documents all aspects of treatment, assessments, and patient education
Maintains active LPN license in state of employment
Communicates with scheduler any changes outside the normal
Participates in all on-call requirements and case conferences
Attends in-service trainings and mandatory agency meetings
Maintains a professional appearance as a representative of the company
Understands and practices agency policies and procedures
ESSENTIAL FUNCTIONS:
Drives to patient's primary location per scheduled visit.
Daily attendance at assigned visit locations.
Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines. Completes and submits all required documentation within specified company requirements.
Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
Per visit compensation refers to a lump sum payment for all activities in connection with a visit to a patient. It is compensation not only for the time involved in doing patient care but for preparation time, travel, lab drops, picking up supplies, charting, and communication with the patient's family, physician, and other health care providers.
HEALTH QUALIFICAITONS:
Health Requirements: Must be able to meet all physical position requirements. Evidence required of a current negative tuberculin skin test or x-ray or receive TB testing at the date of orientation. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous TB test. Employee must be free of physical/medical conditions, which would limit or restrict their ability to perform the job functions listed below. Employees in this position may be at risk for exposure to bloodborne pathogens.

Maintains patient flow during clinic hours. Documents review of systems; measures vital signs such as pulse rate, respirations, blood pressure, weight and height (see Appendix A for specifics); updates past and current medical, social and family history; abstracts new procedures; reviews medications and updates medication list at each visit. Correctly records patient information in the electronic medical record (EMR).
Assessment is an ongoing process that consists of participation with the registered nurse in the determination of nurse care needs based upon collection and interpretation of data relevant to the health status of a client.
Prepares patient charts in EMR for visits by scanning, abstracting, and reviewing to make certain that all diagnostic test results, hospital reports and other medical records necessary to help the physician care for the patient are in the patients chart. Accesses and navigates websites or calls outside facilities to obtain necessary information.
