This role combines exceptional front desk operations with clinical assistance capabilities, ensuring seamless patient care from registration through discharge. PSR's are responsible for maintaining a professional, welcoming environment while adapting between front office and clinical duties to support our mission of providing comprehensive healthcare to homeless and medically underserved populations. Core Responsibilities Front Desk Operations & Patient Registration Open and secure front office operations daily by 7:45 AM, ensuring all systems are operational and lobby area is prepared for patients Serve as primary patient contact providing warm, professional greeting and assistance to all clinic visitors Complete comprehensive patient registration for new and established patients, ensuring accurate demographic and insurance information Verify insurance eligibility and benefits in advance of appointments to prevent service delays Review and collect patient financial responsibility for medical, behavioral health, and dental services according to sliding fee schedule during registration and annual renewal processes Monitor front lobby for safety and emergencies ensuring patient comfort and addressing immediate needs Manage patient communication systems including retrieving and returning voicemails Conduct appointment confirmation calls Coordinate appointment scheduling and rescheduling for services as directed Process daily financial reconciliation by balancing open journal reports with actual collections for accuracy Handle all incoming telephone calls using professional etiquette, taking detailed messages, providing service information, and routing calls to appropriate departments Collect, scan, and process registration documents including identity verification, proof of income, and residency documentation Analyze and evaluate medical record requests in accordance with departmental policies and HIPAA regulations Reports all HIPAA violations and/or potential risks related to the release, use, and/or maintenance of medical records to the Privacy Officer. Clinical Support & Patient Care (As Needed) Provide clinical assistance when operational needs require, including vital signs collection, patient preparation, and basic medical tasks Assist medical staff with patient examinations and procedures under direct supervision Prepare examination rooms maintaining cleanliness, organization, and proper equipment setup Support patient education activities regarding medical procedures, medications, and health maintenance as directed Training & Professional Development Lead, guide, and train staff, student employees, interns, and volunteers in patient health information tasks Participate in continuing education opportunities to maintain current knowledge of healthcare operations and regulations Cross-train in multiple areas to provide flexible coverage based on organizational needs Support quality improvement initiatives and operational enhancement projects Perform miscellaneous job-related duties and/or functions as assigned. Required Qualifications Education & Experience High school diploma or GED with 2 years of experience directly related to healthcare operations Medical Assistant certification preferred (CMA or RMA) with front desk operations experience Healthcare front desk experience within community health center or physician practice environment strongly preferred Experience serving diverse populations including homeless and medically underserved communities Technical Skills & Knowledge Microsoft Office Suite knowledge including Word, Excel, and database management Standard office equipment operation including fax, copier, printer, scanner, and multi-line phone systems Personal Attributes Commitment to HCCH mission and dedication to serving vulnerable populations Flexibility and adaptability to changing operational needs and varying work schedules Team collaboration skills with ability to work effectively across departments Patient-centered approach with empathy and understanding for complex patient needs Professional integrity with strict adherence to confidentiality requirements Bilingual capabilities preferred especially Spanish and/or Haitian Creole .

    Job Description FSLA Status: Non-Exempt Reports to: Manager of Outreach & Case Management Services Provides comprehensive support services to homeless and indigent clients determined to have higher acuity and vulnerability due to high medical needs, including assessment, case planning, and evaluation of services to assist while enrolled in OBFH's residential Health 2 Housing medical respite care with a focus on medical recovery and access to supportive housing. Primary Responsibilities: Advocates for the mission of the organization and promotes the vision and direction of the agency's values Delivers services with safety, respect, and effectiveness Follows all policies & procedures of the organization Works with referral sources to ensure continuity of care and compliance to funding source requirements Upon admission, gathers baseline data concerning client's use of medical services and housing prioritization needs Participates in bimonthly case conferencing and rounds on-site with all involved team members Provides and arranges referrals to appropriate service providers within the agency and externally Balances and maintain a caseload of six (6) or more clients based on acuity, length of time in program, status of housing stability, etc. Advocates for clients as needed Provides hands-on modeling and teaching to assist clients with developing daily living skills (cleaning, cooking, meal planning, shopping, budgeting, etc.). Reviews and evaluates client progress on an ongoing basis and adjusts care plans as appropriate Organizes, documents, and maintains client case files/documentation in a timely fashion in accordance with policies & procedures and professional practice Employees will set up, maintain and use daily the following communication tools to keep abreast of agency updates: agency voicemail and agency email. In the provision of above services, the employee will utilize HCCH HOPE Team Outreach support and approved HCCH/OBFH rideshare resources to coordinate transportation of clients based on priority service needs. Only agency approved vehicles will be used by authorized employees to transport clients in accordance with agency transportation policy. Performs other duties as assigned Requirements: High School Diploma, GED or equivalent experience (Bachelor's degree in human services and/or certification in Case Management with commensurate experience preferred) At least two years' experience in Human Services field providing supportive case management services to at-risk, indigent, and/or homeless population (advocacy, housing placement, etc.) Knowledge, Skills and Abilities: Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, development delays, serious mental illness, traumatic brain injuries, literacy issues, and/or other conditions or situations that have impacted housing stability Ability to work in a fast-paced environment Ability to be flexible and able to multi-task Must be solution-focused and organized in achieving objectives with all service users Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments Ability to maintain confidentiality and follow all federal and state HIPAA laws and regulations Ability to handle stress and high pace of crisis intervention and follow protocols for safety and security Excellent oral and written communication skills, including use of phones, in-person assistance, computers, fax, and other office machinery Computer proficient and experience with EHR (Electronic Health Record) documentation preferred Experience with HMIS (Homeless Management Information System) preferred Additional Requirements: Sitting, walking, lifting to 20 pounds, reaching, carrying, listening, speaking (Physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this position. Qualifications: Education & Experience • Graduate of an accredited nursing program. • Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice. BLS, ACLS certified • Minimum of 1-2 years of clinical nursing experience preferred, preferably in ambulatory, urgent care, or emergency care settings.
