Spiritual Care Coordinator - Hospice - PRN (On Call) - Allentown Office

This position will fulfill On Call shifts after hours and weekends.



Spiritual Care Coordinator Education Requirements:



Bachelor's degree from an accredited school of theology or equivalent, two units of Clinical Pastoral Education(CPE) preferred,



or Master's in Counseling or Master's in Social Work with two units of Clinical Pastoral Education (CPE) required;



or Master's degree in theological studies or equivalent and completion of two units of Clinical Pastoral Education (CPE) preferred. If affiliated or endorsed by a denomination or faith group must be in good standing with said group, as evidenced by a certificate of ordination or certification.



Responsible to provide spiritual support to clients, families, and the hospice staff via phone contact, home visits and referrals to the client/family, local clergy. Assists with memorial services and activities relating to spiritual and religious issues of client, family and interdisciplinary team.

Educational Requirements

Experience in client/family counseling. Demonstrated experience in working with clients and families coping with terminal illness and death. Excellent communication and interpersonal skills.

Position Purpose: Conduct screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment.
Identify special needs members through the completion of health screens and other resources.

Educate providers, members and their families on behavioral health issues including symptoms, relapse prevention, stress reduction, lab work, and healthy lifestyle choices.

Coordinate behavioral health treatment with PCP's and behavioral health providers.

Participate in the development and modification of care plans in conjunction with member, member's family and managing physician to include, monitoring for treatment compliance and assisting with problem solving in breaking down barriers to treatment to include referrals to community resources.

Coordinate urgent care evaluations when necessary.

Assist members with locating providers and scheduling appointments as necessary.

Assist with development of plan specific literature and education materials in conjunction with medical director and corporate oversight.

Will be required to make face to face visits with members.

Education/Experience: Bachelor's degree in Social Work or equivalent experience. 2+ years of experience in managed care and/or behavioral healthcare setting. Practical knowledge of Medicaid and Health Plan administration.

About your Role
The
Care Coordinator I
provides intervention, manages client cases and acts as a resource link to children and families who desperately need assistance.

• Assist in case Management of children of children and take an active role in their case management
• Work with treatment teams, offering insight to be evaluated as you develop plans together
• Serve struggling individuals as an advocate, connecting them to organizations that improve their situation

Does the Following Apply to You?

• A Bachelor’s degree in a Human Service discipline from an accredited four-year college or university
• CPR and First Aid certified, or be willing to obtain certification before working with clients; and other duties as assigned
• Experience working with children/adolescents in a therapeutic, community-based treatment environment
• Ability to work flexible hours consistent with program needs and work at multiple location
• Exemplary organizational, communication, mentoring, and teaming skills
• Sensitivity to other cultures and socioeconomic levels
