The Independent Contractor - Family Support Services will deliver targeted family support services aimed at enhancing family dynamics and promoting child well-being. Services may include, but are not limited to:
Coordinating with community providers to facilitate comprehensive care and referrals for families
Participating in collaborative case planning and case review meetings, when requested
Delivering case management activities such as:
Supervised visitation
Parent aide support
Behavioral Aide
Transportation
Crisis intervention
Advocacy and linkage to services (including drug screening facilitation if applicable)
Completing in-home visits to monitor progress and provide ongoing support for families involved with DFCS.
Offering parenting education and modeling positive parenting strategies
Coordinating with external professionals (e.g., schools, therapists, case managers) to ensure service alignment
Maintaining documentation of services rendered, including case notes and service logs, in accordance with contractual requirements and applicable standards
Providing transportation services related to the family service plan (e.g., visitation, appointments) as appropriate
Qualifications
Bachelor's degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice)
or
High School Diploma/GED with at least 5 years of verifiable experience in the field
Strong understanding of family systems, trauma-informed care, and community resources
Reliable transportation, valid driver's license, and proof of insurance
Willingness to complete required background checks and contractor onboarding
Deliverables
Timely submission of service logs, progress updates, and reports
Accurate documentation aligned with billing and reporting standards
Services provided in accordance with contract terms, including hourly or task-based invoicing

The Certified Diabetes Care and Education Specialist will have different areas of responsibility depending on the environment of which they primarily perform their job duties:
Inpatient education staff will be responsible for the provision of diabetes education primarily in the inpatient setting.
Outpatient diabetes education staff will be responsible for the delivery of diabetes education primarily in the outpatient setting. Individual and group education delivered either in-person or via telehealth will be required. Outpatient educator will also be responsible for facilitating Diabetes Prevention Program (DPP) sessions.
Certified Diabetes Care and Education Specialists stationed within the adult Endocrinology offices will primarily educate patients within the practice settings. Registered dietitian credential is required for these roles.

About this Position:
Our Registered Respiratory Therapists administer all therapeutic, diagnostic, and mechanical modalities under the purview of Respiratory Care Services, for which the Therapist has demonstrated appropriate proficiency, and has been documented as competent to perform these procedures. What you will do
Fills medication orders under the direct supervision of a pharmacist.
Assists with inventory and stocking of medications in the pharmacy and patient care areas according to established pharmaceutical protocol and procedures.
Provides routine information and responds to requests within scope of responsibility.
Maintains records and files in accordance with pharmacy practice standards.
Follows established procedures for medication safety, ensures compliance with regulatory requirements, quality standards, compliance activities, and policies and procedures.
Performs other job functions as assigned or requested.

Mathematical skills to perform routine pharmaceutical calculations which include adding, subtraction, multiplication, and division of whole numbers, common fractions, and decimals; computes rate, ratios, and percents.
Practice within the established role for pharmacy technicians in organized health care settings.
Effectively use medical terminology, drug nomenclature, symbols, and abbreviations associated with pharmacy practice
Ability to speak and communicate effectively with patients, associates, and other health professionals.

Physical Demands

The physical requirements of this position are: medium work - exerting up to 100 lbs. of force occasionally and/or up to 50 lbs. of force frequently.
The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
Frequent invasive and non-invasive patient contact.
Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.

The Family Centered Treatment Therapist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Qualifications
Minimum of a Master's Degree in human services or related field is required.
Two years' experience working with youth or families.
Additional state specific requirements include:
NC, VA- QMHP or ability to register as a QMHP
MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.

The purpose of this position is to provide high-quality, therapeutic services to clients, ensuring their mental health and well-being. The Therapist is responsible for conducting assessments, developing and implementing individualized treatment plans, maintaining accurate documentation, and collaborating with other internal and external professionals. Provide consistent and comprehensive services to all assigned individuals on your caseload.
Ensure regular contact and follow-up with individuals to monitor progress and adjust treatment plans as necessary.
Accurately document all client interactions, treatment plans, progress notes, and other required documentation within established timeframes per company policy.
Ensure compliance with all regulatory and organizational standards.
Work closely with other professionals as a multidisciplinary treatment team, to provide holistic care.
Participate in regular treatment team meetings to discuss progress and treatment strategies of individuals served.
Provide immediate support and intervention during crisis situations.
Develop safety plans and coordinate with emergency services when necessary.
Engage in community outreach to promote mental health services and attract new clients.

Qualifications
Master's Degree in Social Work, Counseling, or a related field is required.

Must be eligible for licensure as a LPC, LMFT, or LCSW.
This is a part-time position but can become full-time after 90 days.
