The Certified Diabetes Care and Education Specialist serves as a clinical expert and provides education and consultation services to internal and external groups. The Certified Diabetes Care and Education Specialist will have different areas of responsibility depending on the environment of which they primarily perform their job duties:
Inpatient education staff will be responsible for the provision of diabetes education primarily in the inpatient setting.
Outpatient diabetes education staff will be responsible for the delivery of diabetes education primarily in the outpatient setting. Individual and group education delivered either in-person or via telehealth will be required. Outpatient educator will also be responsible for facilitating Diabetes Prevention Program (DPP) sessions.
Certified Diabetes Care and Education Specialists stationed within the adult Endocrinology offices will primarily educate patients within the practice settings. Registered dietitian credential is required for these roles.
Regardless of the primary location of the role, the Certified Diabetes Care and Education Specialist may be requested to assist at other locations depending on department needs.
Requirements
Education:Associate degree required in health-related profession.
Experience:Minimum of 2 years experience professional practice experience in Diabetes Self-Management Education required. Minimum of 1000 hours in Diabetes Self-Management Education within the past 5 years preferred.

About this Position:

Engage in building new outreach events/curriculum based on community needs to minimize gaps in veteran and military family support.
Support clinic events through online registration management/promotion and event material preparation.
Assist with content creation for clinic's social media platforms in adherence to Centerstone and CVN brand standards.
Assist in the creation of marketing materials, including event flyers and other digital assets for distribution in the community.
Monitor the market media landscape and provide insight on new ways to reach target audiences.
Attend weekly or designated meetings with the Outreach Director and Clinic Communicator to stay in sync on projects or assignments.
Assist with preparing outreach and community involvement reports for Clinic Leadership and Cohen Veterans Network Central Office staff, including Salesforce data entry, JotForms, and Connect.

JOB DESCRIPTION:
SUMMARY OF POSITION
As a school-based behavioral health liaison, you will be responsible for providing face to face consultation with classroom teachers to assist them in creating positive, trauma-informed classroom environments, to enhance the learning environment for children at risk of developing an emotional/behavioral problem or substance use disorder, or with serious emotional disturbance (SED). Training and education regarding childhood mental health and trauma-informed behavior responses in the classroom will also be incorporated in the consultation to assist the teachers in promoting a healthy teaching environment. Each SBBHL shall provide psycho-educational groups and other support as needed to youth identified by teachers and/or or other school officials. The SBBHL program will provide Liaison support between the family and the school and will, when invited, assist parents and youth with the Individualized Education Plan (IEP) process and with trauma-informed support plans.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide face-to-face consultation with classroom teachers to assist in creating a positive, trauma-informed classroom that enhances the learning environment, and to assist the classroom teacher in developing effective, trauma-informed behavior responses for the service population and other students in the class.
Provide at least one face to face training per year to LEA personnel on a variety of mental health and substance abuse prevention topics.
Assist in the development of school wide mental health initiatives and universal prevention strategies.
Provide Liaison services to include communication between the school and the child's family to build open lines of communication and to build home-school partnerships. Liaison services can include assisting in the IEP process when requested, assisting in the alternative school placement process, developing trauma-informed support plans collaboratively with the school and families of the students to meet student individual needs, etc.
Provide information and support for school personnel in navigating the local behavioral health system, including substance abuse and mental health services, and accessing mental health crisis services.
Maintain logs, tracking documents or other appropriate management tools needed to track all aspects of these services. The State will review these documents during the annual monitoring visit.
Ensure that each Liaison provides support opportunities to students at each school to include individual student consultations or interventions to assist in building positive coping and de-escalation skills, as needed; and at least three psychoeducation group activities per year that cover the following topic(s): mental health problems and disorders; the risk of substance use/abuse and substance use/abuse disorders; healthy ways to deal with stress including mindfulness, coping and de-escalation strategies; and other topics as decided and requested by each school.
Address specific mental health or behavioral issues through individual, group, and/or family therapy services. Formulate a diagnosis upon completion of intake assessment. Create and update treatment plans according to policy.
Maintain timely and accurate documentation, including treatment plans, progress notes, and report, to comply with grant and organizational requirements.
Compliance requirement for bi-annual completion of performance evaluation.

Implement plans of action to correct deficiencies.
Complete medical forms, reports, evaluations, studies, charts, etc.
Supervise resident's care, conduct compliance rounds throughout the day to assure quality of care and to address problems, as necessary. Make appropriate documentation.
Conduct tour and obtain essential information for possible admission.
Inform staff of new admissions, ensure that rooms are ready; greet new admissions and escort to rooms. Ensure that any equipment, medication, treatment and other supplies are available.
Supervise Nursing Assistants, Charge Nurses, LPNs and Med Tech, if applicable.
Assist in counseling, evaluating, and terminating nursing services personnel with guidance from Administrator and DON.
Ensure schedules and work assignments, including vacation and holiday schedules, are implemented to ensure adequate staffing, including replacing call-ins.

The Certified Peer Recovery Specialist (CPRS) assists in providing a safe, fun, and supportive environment where individuals with a mental illness or addiction gain confidence, build social skills, and engage in learning and leisure activities with peers. The CPRS will achieve the program goal and operation through various job duties to include actively facilitating groups and activities on recovery topics.
