Responsibilities
The Family Development Specialist supports clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. This role involves effective communication, compliance with regulatory standards, thorough documentation, and active recruitment of new referrals.
Develop individualized treatment plans and coordinate care for clients and families with the input of the treatment team and the Clinical Supervisor.
Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
Attend and actively participate in weekly Treatment Team meetings.
Link clients and families with specific services and resources as identified in the treatment plan.
Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring their needs are met in a comprehensive manner.
Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
Actively recruit new referrals to maintain a full caseload.
Provide 24/7 on-call support for clients as required by the program.
Ensure compliance with all state regulatory bodies and COA standards.
Participate in ongoing training and professional development activities as assigned, to stay current with best practices for quality service delivery.
Qualifications
Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field, is required.
Minimum of two years experience working with children and/or families. This experience may include internships and volunteer work.

The Family Support Specialist provides best practice, quality clinical services, ensures all documentation is completed per Payer's, state licensing agent's, COA, & HCA's standards and established guidelines and assist with marketing for new referrals.
Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns.
Develop treatment plans with the input of the treatment team and the Clinical Supervisor

SUMMARY OF POSITION
The Team Leader for Military Services will serve as the operational lead for all activities related to the SSG Fox Suicide Prevention Grant. This role will oversee day-to-day program operations, supervise staff teams, ensure compliance with grant deliverables, and coordinate outreach, screening, and navigation services for Veterans, service members, and their families. The Team Leader will serve on the Military Services operations team, act as the primary liaison between Centerstone Military Services and the VA Fox team, and report directly to the Director of Military Services.

SUMMARY OF POSITION
This position assist in hiring and supervises all behavior coaches at Murrell Special Day Treatment School. The Behavior Coach serves all students, who are under the Exceptional Education umbrella and are covered under an Individual Educational Plan (IEP). These students have severe emotional and behavioral problems that have affected their ability to be maintained in a mainstream school setting. Behavior Coaches work in the classroom setting alongside Metro school staff to develop behavioral interventions and incentives, assist in crisis, and work to assist in the success of each student. This position requires a collaborative approach with school staff and a teamwork attitude. We are seeking applicants with a strong work ethic and a professional attitude that have a passion for working with children and youth.
COMPENSATION
$42,000.00 - $46,000.00 for 11-months paid over the course of 12-months of employment.
ESSENTIAL DUTIES & RESPONSIBILITIES
Supervises the daily work of all behavior coaches contracted at Murrell Special Day Treatment school, including all levels of accountability and following Just Culture Algorithm
Assists in training all behavior coaches, implements training of new staff and ongoing training as necessary
Provides social-emotional learning in special day treatment classrooms K-7th grade (all students are under an Individualized Educational Plan {IEP} and display severe emotional and behavioral issues)
Intervenes with students and clients at school when there is a mental health crisis, communicating with the guardian and school personnel on steps needed to stabilize the student. This includes contacting other agencies, DCS, mobile crisis, etc., Make sure all coaches follow standards in dealing with crisis and contacting other agencies as needed.
Assist on a daily basis with managing students with severe emotional and behavioral issues. Assessment of all students in crisis which can include (Conducting threat assessments and assessing for self-injurious behaviors, Suicidal and homicidal ideations). Makes sure behavior coaches are providing clinically sound crisis assessments and meeting student's needs.
Supervisor Behavior coaches in their development and conduction of a minimum of two behavioral health group sessions each week, and daily morning and closing meetings. Fills in in their absence to develop and conduct groups
Follows PBIS (Positive Behavioral Intervention Support) plans and assist staff in understanding, and implementing these policies
Provides trauma informed care and makes sure all behavior coaches are providing the same care.
Consults and collaborates frequently with outside agencies, as well as MNPS staff and Centerstone behavioral staff.
Collects data from behavior coaches, MNPS and other sources. Complies and analyzes this data. Makes sure behavior coaches completes assessments, to include (Adverse Childhood Experiences {ACES}, School Social Behavior Scales {SSBS-2}, etc. in a timely manner. Shares this data with the School team and helps implement changes in programing for students
Makes sure all behavior coaches complete required daily, weekly and monthly documentation, administrative and clinical duties
Must be certified in Satori Alternatives to Managing Aggression (SAMA) within the first 30 days of hire, this consists of verbal de-escalation, object retrieval and physical containments (provided by MNPS)
Attends and actively participates in clinical treatment team supervision meetings. Makes sure Behavior coaches are in attendance and prepared to make clinical presentations
Attend all MNPS staff meetings and makes presentations as needed per contract. Attends Student Assistance Team (SAT) meetings, IEP meetings, data meetings and other meetings as assigned. Assist behavior coaches in preparations for these meetings, presents data, and assist in interpreting this data to make informed decisions, intervention planning and implementation for behavioral support strategies
Leads a weekly behavior coach staff meeting, using agenda from meetings with coordinator
Maintain clear and timely professional documentation records including weekly student/client summaries
Provide consultation and training to school staff and other behavior coaches as needed
Provide excellent customer service to individuals seeking our services and to Metropolitan school staff
Maintains frequent engagement with parents/legal guardians. Supervises behavior coaches to make sure they meet with/contact parents and guardian a minimum of twice a month
Assist in transitioning students to the least restrictive school settings
Assist in classroom coverage as needed including implementation of MNPS lesson plans
Develops, leads and participates in trainings during MNPS school breaks (Spring, Christmas and Spring Breaks and other MNPS planning and staff development days)
Assist coordinator in maintaining polices and procedures and developing administrative and clinical trainings as needed as well as other duties as needed.

Jump start your career in the mental health field as a Psychosocial Educator. Work within a team environment to provide clinical psychoeducation to Centerstone eligible program participants. Provides support, assistance, psycho-social education and supervision to Adult Residential Services residents.
HOURS

Responsible for keeping file and closet locked while on duty.
Review referral information as it is presented to help determine eligibility for admission and continued stay for Supported Housing and Crisis Respite where relevant Complete authorizations for stay as needed with each Managed Care Company
Supervise meal preparation and compliance of residents with dietary guidelines, providing education as needed.
Receive and dispense residents' funds from locked filing cabinet in office as needed and document all money received and given to residents in residents' charts.
Plan and supervise psycho-educational activities recreational activities during residents' free time and provide van transportation on outings.
Inform Residential Coordinator of all medical emergencies, crisis situations with residents or any disruptions in program operation.
Receive briefing at beginning of shift from Residential Coordinator or other staff and provide briefing to other staff at end of shift.
Document a progress note on each resident once per shift and at any time there is a significant incident.
Attend all Program staff meetings and all scheduled staff development activities.
Conduct room inspections as needed to monitor and educate on cleanliness of rooms, residents' compliance with hygiene/grooming and household task completion.
Each residential program within Adult Residential Services will provide a safe, comfortable supervised housing option for mental health consumers, and assist consumers in reaching and maintaining stabilization of symptoms, while encouraging consumers to assume increasing responsibility for their lives.

JOB DESCRIPTION:
SUMMARY OF POSITION
As a school-based behavioral health liaison, you will be responsible for providing face to face consultation with classroom teachers to assist them in creating positive, trauma-informed classroom environments, to enhance the learning environment for children at risk of developing an emotional/behavioral problem or substance use disorder, or with serious emotional disturbance (SED). Training and education regarding childhood mental health and trauma-informed behavior responses in the classroom will also be incorporated in the consultation to assist the teachers in promoting a healthy teaching environment. Each SBBHL shall provide psycho-educational groups and other support as needed to youth identified by teachers and/or or other school officials. The SBBHL program will provide Liaison support between the family and the school and will, when invited, assist parents and youth with the Individualized Education Plan (IEP) process and with trauma-informed support plans.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide face-to-face consultation with classroom teachers to assist in creating a positive, trauma-informed classroom that enhances the learning environment, and to assist the classroom teacher in developing effective, trauma-informed behavior responses for the service population and other students in the class.
Provide at least one face to face training per year to LEA personnel on a variety of mental health and substance abuse prevention topics.
Assist in the development of school wide mental health initiatives and universal prevention strategies.
Provide Liaison services to include communication between the school and the child's family to build open lines of communication and to build home-school partnerships. Liaison services can include assisting in the IEP process when requested, assisting in the alternative school placement process, developing trauma-informed support plans collaboratively with the school and families of the students to meet student individual needs, etc.
Provide information and support for school personnel in navigating the local behavioral health system, including substance abuse and mental health services, and accessing mental health crisis services.
Maintain logs, tracking documents or other appropriate management tools needed to track all aspects of these services. The State will review these documents during the annual monitoring visit.
Ensure that each Liaison provides support opportunities to students at each school to include individual student consultations or interventions to assist in building positive coping and de-escalation skills, as needed; and at least three psychoeducation group activities per year that cover the following topic(s): mental health problems and disorders; the risk of substance use/abuse and substance use/abuse disorders; healthy ways to deal with stress including mindfulness, coping and de-escalation strategies; and other topics as decided and requested by each school.
Address specific mental health or behavioral issues through individual, group, and/or family therapy services. Formulate a diagnosis upon completion of intake assessment. Create and update treatment plans according to policy.
Maintain timely and accurate documentation, including treatment plans, progress notes, and report, to comply with grant and organizational requirements.
Compliance requirement for bi-annual completion of performance evaluation.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports, and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications

Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.

Responsibilities
The Family Development Specialist supports clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. This role involves effective communication, compliance with regulatory standards, thorough documentation, and active recruitment of new referrals.
Develop individualized treatment plans and coordinate care for clients and families with the input of the treatment team and the Clinical Supervisor.
Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
Attend and actively participate in weekly Treatment Team meetings.
Link clients and families with specific services and resources as identified in the treatment plan.
Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring their needs are met in a comprehensive manner.
Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
Actively recruit new referrals to maintain a full caseload.
Provide 24/7 on-call support for clients as required by the program.
Ensure compliance with all state regulatory bodies and COA standards.
Participate in ongoing training and professional development activities as assigned, to stay current with best practices for quality service delivery.
Qualifications
Bachelor's degree in human service discipline, including but not limited to Social Work, Psychology, Counseling, Sociology, Criminal Justice or a related field.
Minimum of two years experience working with children and/or families. This experience may include internships and volunteer work.

JOB DESCRIPTION:
SUMMARY OF POSITION
The Certified Peer Recovery Specialist (CPRS) assists in providing a safe, fun, and supportive environment where individuals with a mental illness or addiction gain confidence, build social skills, and engage in learning and leisure activities with peers. The CPRS will achieve the program goal and operation through various job duties to include actively facilitating groups and activities on recovery topics. The CPRS assists clients with articulating personal goals for recovery; assisting participants in determining reasonable and holistic steps in recovery; and identifying barriers to recovery goals.
ESSENTIAL DUTIES & RESPONSIBILITIES
Verbalize understanding of what recovery means and articulate it to others.
Assist clients with identifying personal strengths in daily living and recovery skills.
Be professional, with an attitude of mutual respect and encouragement.
Encourage involvement in the center by modeling a positive attitude.
Respect confidentiality and program ethics.
Facilitate groups that promote recovery through education, advocacy and positive role modeling.
Document peer progress in electronic medical records.
Attend all trainings.
Assist with daily activities that may include group preparation, meal preparation, provide transportation via company van.
KNOWLEDGE, SKILLS & ABILITIES
Utilize excellent communication skills and the ability to meet requirements of certification
Basic computer literacy is required
Willing to share personal recovery story to assist others
Be active in own recovery and model recovery in a positive way
QUALIFICATIONS
Education Level
High school diploma or GED is required
Years of Experience
18 years of age or older
Lived Experience with Mental Illness/ Substance Use Disorder.
Minimum of one year of recovery.
Certification/Licensure
Must have a valid driver's license and proof of insurance
Must acquire F-endorsement within 30 days of hire date
CPRS Certification is required. Must be able to verify completion of state-approved training program. If not currently certified, must be certified within 90 days of hire unless otherwise approved by the supervisor.
