    ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES ● Increase awareness in the Native American/Alaskan Native Community around COVID-19 mitigation ● Assist in vaccine registration and provide non-clinical staff support at vaccination events ● Conduct in-person, social media, phone call, and email outreach (according to COVID-19 safety regulations) to provide impacted communities with COVID-19 information about testing, contact tracing, quarantine/isolation, vaccinations, and referrals for other resources ● Provide accurate information regarding COVID-19 vaccines and vaccine safety (what it is, understanding rollout and timeline, description of the different vaccines, how to register when you’re eligible, actively registering eligible individuals in high need communities, accessing vaccine distribution sites, review of potential vaccine reactions, and aftercare guidance if a reaction occurs) ● Distribute PPE to the Native American community in Los Angeles County ● Promote UAII services and recruit eligible Native Americans/ Alaskan Natives living in Los Angeles County to enroll in UAII services ● Help community members navigate necessary resources including social services, healthcare, mental health, unemployment services, and more as needed ● Deliver culturally appropriate follow up with eligible participants ● Conduct at-home and clinical visits with community members to ensure adequate access to health resources and services ● Increase knowledge of chronic conditions and other communicable diseases ● Utilize cloud-based data entry systems to accurately document, track, and measure CHW outreach efforts include learning, navigating, and using new applications/software for phones (Android) and computers (Microsoft) ● Comply with Los Angeles County Department of Public Health and UAII training regarding confidential information related to personal information ● Travel throughout Los Angeles County ● Accept responsibility for other duties, as assigned
