POSITION SUMMARY
The goal of the UAII Community Health Outreach Worker (CHW) Outreach Initiative is to amplify existing COVID-19 prevention and vaccination messaging to the UAII community and urban Native American Community at large by providing general education, information, and resources regarding COVID-19 to the public.
As a CHW , you will work to ensure that accurate and up-to-date information regarding COVID-19 reaches all members of the UAII community, especially those that are most highly impacted by the virus. You will ensure that messaging regarding Coronavirus, COVID-19, and COVID-19 vaccination is delivered in an efficient, effective, and culturally appropriate manner.
The CHW will educate the public on UAII Services, recruit eligible individuals to register with UAII for services, and promote linkages to partner programs and services when applicable. UAII Community Health Outreach Workers will also work with American Indian/Alaskan Native community members using a case-management approach, assisting community members in 1-on-1 appointments to find and utilize needed health and human services.


ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES
● Increase awareness in the Native American/Alaskan Native Community around COVID-19 mitigation
● Assist in vaccine registration and provide non-clinical staff support at vaccination events
● Conduct in-person, social media, phone call, and email outreach (according to COVID-19 safety regulations) to provide impacted communities with COVID-19 information about testing, contact tracing, quarantine/isolation, vaccinations, and referrals for other resources
● Provide accurate information regarding COVID-19 vaccines and vaccine safety (what it is, understanding rollout and timeline, description of the different vaccines, how to register when you’re eligible, actively registering eligible individuals in high need communities, accessing vaccine distribution sites, review of potential vaccine reactions, and aftercare guidance if a reaction occurs)
● Distribute PPE to the Native American community in Los Angeles County
● Promote UAII services and recruit eligible Native Americans/ Alaskan Natives living in Los Angeles County to enroll in UAII services
● Help community members navigate necessary resources including social services, healthcare, mental health, unemployment services, and more as needed
● Deliver culturally appropriate follow up with eligible participants
● Conduct at-home and clinical visits with community members to ensure adequate access to health resources and services
● Increase knowledge of chronic conditions and other communicable diseases
● Utilize cloud-based data entry systems to accurately document, track, and measure CHW outreach efforts include learning, navigating, and using new applications/software for phones (Android) and computers (Microsoft)
● Comply with Los Angeles County Department of Public Health and UAII training regarding confidential information related to personal information
● Travel throughout Los Angeles County
● Accept responsibility for other duties, as assigned


MINIMUM QUALIFICATIONS
Experience & Qualifications:
● Must be “Fully Vaccinated” against COVID-19
● Case Management Experience (1 year)
● Outreach or Marketing Experience (1 year)
● Data Entry Experience (1 year)
● Ability to schedule various meetings to discuss the Outreach objectives
● Ability to explain 1-1 and groups of what COVID-19 is, how it can spread, what to do when you have it, and how individuals and families can stay safe
● Ability to explain how to access services including, but not limited to, social services, health insurance coverage, medical and behavioral health services, and availability of COVID-19 prevention services (e.g., testing, contact tracing, and vaccination)
● Ability to explain current directives, what’s open and not open, employee/employer requirements, and worker rights
● Ability to dispel myths and conducting rumor control, including where to find accurate information and updates
● Demonstrate excellent customer service skills
● Strong communication skills
