JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The BHID Specialized Residential Service provides direct care including support, structure, assistance, skill training and therapeutic interventions, for service participants, in collaboration with BHID Integrated Clinical Services team. Services are provided in the home and community.

QUALIFICATIONS:

  • High school diploma or equivalent and 12 credit hours from an accredited college/university OR , prior to hire, an approved waiver from CBH is obtained that includes a rationale for hiring without the required credit hours
  • 2 years’ experience in human services, paid or unpaid, preferably with adults with psychiatric and/or intellectual disabilities.
  • Able to use an Electronic Health Record.

PHYSICAL DEMANDS:

Must be able to assist in crisis management interventions, administer CPR/First Aid, and assist individuals with mobility issues as needed.

SPECIFIC DUTIES:

1. Provide supervision of residents at all times in accordance with their integrated individual support plan.
2. Maintain a supportive, therapeutic relationship with each service participant.
3. Be aware of each service participant’s individual service plan, history, medications, current needs and any special accommodations required by the participant.
4. Act as a role model at all times, maintaining appropriate boundaries and demonstrating effective communication skills.
5. Maintain clear and consistent expectations of each service participant while maintaining a safe, supportive environment.
6. Implement, on a consistent basis, direct service interventions as defined in the individual’s service plans.
7. Oversee, teach skills and actively assist in the completion of household chores ensuring the home is neat and clean at all times.
8. Oversee, teach skills and actively assist in the completion of personal hygiene and other activities of daily living as needed by the participant.
9. Assist service participants in developing and practicing skills defined on their individual service plan. This may include accompanying residents to appointments, social activities and/or job opportunities, and other activities of daily living.
10. Maintain open and ongoing collaboration and communication with all team members such as: residential direct care staff, clinicians, behavior specialists, psychiatrist, nurse and peer specialist.

Description

Northwestern Medicine, Ben Gordon Center is seeking both new graduates and experienced Behavioral Health On-Call Crisis Referral Services Social Worker to join our team!

This position is responsible to respond and provide crisis services in the evening and on weekends within the greater DeKalb County area within one hour of being called. The duties include but are not limited to completing a crisis risk assessment and evaluation to determine safety plan and disposition.

The Social Worker is responsible for providing high quality services to the patient and the patients family in order to promote optimal social and emotional functioning. These services include direct service to patients and their families both on an inpatient and outpatient basis. The Social Worker coordinates options and services to meet the patients needs through collaboration and consultation with physicians and other healthcare personnel. On-call social services are provided primarily to patients in the Emergency Department; however, the Medical, Surgical, ICU, Pediatrics and Obstetrics may also require the services of an on-call worker.

Qualifications

Required:

* Bachelor's degree with major in social work or related field with equivalent in education, training, or experience and documented mental health training or experience.
* Documented experience providing direct service to patients.
* Knowledge of the mental health code.
* Knowledge of normal characteristics of growth and development throughout the span of life.
* Demonstrates ability to work productively and effectively in a timely manner, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interactions both inside and outside the host hospital.

Job Description
Primarily focused on the provision of safety, and connecting to needed services for mental health care (outpatient), the Behavioral Health Worker provides services to support inpatients, outpatients and/or other individuals or families referred for care at El Camino Hospital. Works under supervision of charge RN during assignment to the inpatient department, and under the supervision of outpatient clinical and business leadership during assignment to the outpatient department.

Emergency Dept
To provide professional interface with clinicians, physicians and others making referrals to Behavioral Health programs, as well as with patients and their families. Under the supervision of licensed staff is responsible for a coordinated entry and discharge experience for patients and referring agents. Provides services to support patients in the treatment environment, primarily focused on the provision of safety. Works under supervision of charge RN during assignment to department.

QUALIFICATIONS:
Associates Degree in Psychology or related field required; Bachelor's degree preferred.
One (1) year experience in a behavioral health or inpatient psych setting is required.

License/Certification/Registration Requirements
Basic Lifesaving Certification - American Heart Association
