Summary/Objective:
Primary job responsibilities involve assessing needs, service and resource linkage, and care coordination to support youth and families/ caregivers in the OhioRISE plan in achieving their health and outcomes goals. Care Coordinators are primarily community-based and may be able to perform some tasks from home. Care Coordinator Non-Licensed will work with individuals, parents/guardians, Boundless staff, community members, and other service and support providers via face-to-face engagement, telephone, video conferencing, and electronic communication. Day-to-day activities are varied based on the needs of the individuals and families/ caregivers on the caseload. The Care Coordinator Non-Licensed serves as a primary point of contact and liaison for all the vital support providers in a youth or family's life, including scheduling meetings, tracking, exchanging documentation, following up on needs and appointments, and reporting outcomes.

Link individuals and families served to the resources as needed and update databases to share resource information.
Track and report when less than two providers of a needed service are available within a community to offer an individual or family. Participate in team and community meetings regarding necessary resource development specific to the communities served.
Facilitate the completion of necessary forms and documentation, including consent for release information with relevant stakeholders and child and family teams.
Establish and maintain positive relationships with the local system of care partners such as the children's services system, Title IV-E agencies, boards of developmental disabilities, local corrections and court systems, schools, family and children first councils, managed care organizations, providers of behavioral health services, and primary care providers.
Support individuals and families in accessing medically necessary services and supports.
Serve as a liaison between individuals, families, and care team members to positively promote collaboration.
Complete all documentation as required, including gathering and reporting of all health and outcomes data.
Manage phone calls and communication from individuals, families, guardians, and stakeholders, etc. Coordinate meetings and ensure information is exchanged consistently.
Respond to and be available for clients and families in crisis, as needed, including referencing the crisis plans and facilitating the appropriate linkage to de-escalate the crisis.
Meet engagement, documentation, and contract requirements as established by management.
Provide excellent customer service and communication coordination between team members, individuals, families/ caregivers, community members, and collaborating organizations.

Responsibilities may include:
Address the behavioral and physical health needs of clients receiving treatment for psychiatric symptoms.
Perform health care screenings, check vital signs, and administer medications as needed while providing patient education.
Gather client information and history from the Nurse Practitioner.
Case management duties including coordination of community services.
Assist clients in building coping skills.
Delegation of required DODD law and rules based on setting and review Dr/s orders checking for accuracy.
Individual specific training to certified staff on new medications.
Assist Programming in getting referral for doctors or equipment needed for the client.
Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Requirements for this position include:
Graduate of an accredited nursing program.
Active RN/LPN license.
Experience in psychiatric, mental and behavioral health services.
Willingness to travel throughout assigned service area.
Preferably has experience working with individuals with developmental disabilities.

The Safety Specialist is a crucial member of the Versova Management Safety Team, working across all farms to ensure compliance with Federal, State, Company, and other agency regulations. This role supports the development and implementation of safety programs, conducts inspections, maintains safety records, and coordinates safety initiatives to create and maintain a safe work environment for all employees.
Essential Job Functions
Develop and implement safety policies, procedures, and programs
Conduct routine safety and hazard inspections across facilities
Maintain comprehensive safety records, including employee training, inspections, and incident reports
Coordinate and assist in safety training for employees at all levels
Investigate accidents and incidents, identifying root causes and trends
Assist with workers' compensation processes, including record-keeping and coordination with site managers
Ensure compliance with OSHA recordkeeping requirements
Prepare and distribute routine safety dashboard reports and analytics
Administer safety incentive programs
Assist in coordinating safety audits and inspections
Communicate regularly with team members and stakeholders on safety matters
Support the review and update of LOTO (Lock Out/Tag Out) procedures
Other duties as assigned
Required Qualifications
Associate's degree in Occupational Health and Safety or related field (or equivalent experience)
1-2 years of experience in safety administration or related role
Proficiency in Microsoft Office Suite, especially Excel
Strong analytical and problem-solving skills
Excellent written and verbal communication abilities
Knowledge of OSHA regulations and safety standards
Ability to work overtime, weekends and holiday hours as required based on the production demand
Preferred Qualifications
Bachelor's degree in Occupational Health and Safety or related field
30 Hour OSHA General Industry Certificate
Experience with safety management software
Familiarity with workers' compensation processes
Bilingual in English and Spanish
Work Environment
This position involves a mix of office work and on-site safety activities across multiple farm locations. The Safety Specialist must be comfortable in various agricultural settings, which may involve exposure to different climate conditions, including extreme temperatures. Some travel between sites is required. The role may involve responding to safety incidents outside normal business hours. This position offers the opportunity to significantly impact workplace safety culture and practices across the organization.
