Provides culturally competent and appropriate behavioral health treatment to people served by Community Counseling Solutions. The position provides treatment in accordance with the Oregon Administrative Rules (OAR's) and the mission and values of CCS. This position provides prompt and thorough assessment, evaluation, and intervention via telephone or in person, and provides written and verbal communication where indicated. The position works closely with other involved community partners and is professional in all aspects of service delivery. Additionally, this position assists administration in meeting the behavioral health needs of the community.

RESPONSIBILITIES
Demonstrate ability to assimilate data from a wide range of observations and use various clinical techniques and skills in gathering the client's personal, family, medical, and psychiatric histories.
Establish short-term goals with clients, provide follow-up services as needed, and make appropriate/expeditious referrals.
Provide home visits and other outreach services to schools and other agencies/providers that are necessary and appropriate.
Thoroughly screen referrals to assure proper utilization of Outpatient Services.
Promptly and accurately complete clinical records and all collateral forms as necessary and required.
Arrange for hospitalization of clients, either on a voluntary or involuntary basis, and thoroughly assess to assure that it is the least restrictive alternative. In facilitating a patient's admission, Crisis Worker must accurately assess for special needs and alert other staff and/or physician to the apparent needs of the patient. Crisis Worker will act expeditiously in assisting police and all community agencies, including hospital staff.
Arrange emergency/temporary placement as appropriate.
Will work as part of a mental health emergency response team and avail themselves to work various non-traditional work hours covering call-offs when feasible.
Cooperate with all referral sources and will provide assistance, directions, and feedback as necessary and appropriate.
Provide consultation services to hospital departments and other providers as necessary and appropriate.
If varianced, provide clinical services to children, adults and families. This would include but is not limited to screening, assessment, individual and group therapy, consultation, and all other allowed clinical services.
Actively participate in joint case planning and coordination with agency employees and relevant agency contractors. Act as a liaison between clients and other agencies. Act as a client advocate when appropriate.
Actively participate in group and individual supervision sessions.
Work with other staff and community representatives to initiate new programs, reviews, evaluates and revises existing programs as necessary.
Perform joint case planning and coordination in areas of behavioral health with other community agencies including but not limited to: schools, group homes, police organizations, district attorney's offices, courts, community clinics and hospitals and other agencies in the Department of Human Services.
Provide behavioral health and addiction education, prevention and information to various interest groups.
Provide after hours emergency services on a 24 hour, rotating basis to adults. This includes telephone consultation and crisis intervention in the community. If varianced, provides same services to children.
If varianced, initiate holds, provide civil commitment investigations, and work with the hospitals, case workers and the state and other individuals/agencies for all individuals in the civil commitment process.
Complete all paperwork in a way that is professional, clinically sound and timely as prescribed by the OAR's and other pertinent guidelines. Required to ensure that all paperwork is present, and/or the client record is well maintained.
Attend trainings, meetings, conferences, etc. as directed by administration and use the information gathered to benefit the agency and the customers.
Assist other clinicians in the facilitation of groups as requested.
If varianced, provide intensive children's treatment services.
Utilize agency software/computer systems to prepare current client records of treatment and reports as necessary and required.
Transports clients as required
Administer forms for urinalysis testing (UA's), provide accurate information on the UA's to clients, in some cases observes the UA, and record all pertinent information.
Must report all abuse to appropriate legal office, complete all necessary paperwork and investigations, and develop safety plans as required.

Education and/or Experience
MHS IV - Either a doctorate degree from an accredited college in social work, psychology or other human service-related field is required, or a master's degree with accompanying certification as an Licensed Professional Counselor (LPC), a Licensed Clinical Social Worker (LCSW), or a Licensed Marriage and Family Therapist (LMFT).
MHS III - Requires a master's degree from an accredited college in social work, psychology or other human service related field.

Certifications

CRIMINAL BACKGROUND CHECKS
Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 through 410-007-0380.

Must show proof of $300,000 or more liability coverage for bodily injury and $100,000 or more in property damage, and maintain said level of coverage for the duration of employment at CCS.
The employee's insurance is primary with CCS insurance being secondary. CCS reserves the right to deny any employee the use of a vehicle owned by CCS.

Housing Rehabilitation Specialist

Salary: $54,600 (plus benefits) Non-union - Grant Funded Summary: Under the Supervision of the Community Development Coordinator, the Housing Rehabilitation Specialist manages projects of the Home Improvement Program (HIP). This is a non-certified, grant-funded position that that is subject to review on a yearly basis and subject to discontinuation based on funding.
Duties and Responsibilities:
Reviews Home Improvement Program applications.
Inspects eligible applicant properties.
Writes detailed work specifications based on home inspections.
Facilitates the bidding process.
Inspects renovation work in progress and at completion.
Reports information to relevant agencies.
Related duties as required.
Preferred Skills, Knowledge, and Experience:
Experience in the building and construction industry.
Experience in lead abatement.
Knowledge of all aspects and phases of residential renovation.
Knowledge of building regulations, codes, and practices.
Knowledge of health and safety regulations, codes, and practices.
Ability to work cooperatively with contractors and members of the public.
Experience using industry-specific software applications and platforms.
Ability to communicate effectively orally and in writing.
Minimum Requirements:
Graduation from high school.
Current Rhode Island Construction Supervisors License.
Training in practices of the construction trade.
5 years of experience in the construction trade.
Lead Inspector (LIT) License (preferred)
A valid driver's license.

How you'll contribute
A Clinical Rehabilitation Specialist who excels in this role:
Educate the community on acute rehabilitation to develop a census through face-to-face contacts.
Develop business based on the strategic goals of the rehabilitation program.
Face-to-face connections within the territory to build relationships with referral sources to increase census.
Identifies barriers to the admission process and creates solutions with the assistance of the program director.
Requires onsite and in-territory work through face-to-face contact with patients, families, and referral sources.
Completes in-person in-services and presentations to educate on acute rehabilitation programs and services.

What we're looking for
At a minimum, should hold a 2-year degree. A graduate holding a four-year degree from a college program with a bachelor's degree in a health related, business or marketing area of concentration, nursing preferred.
Previous clinical liaison/marketing/sales experience preferred
Applicable and current license to practice in accordance with the state licensure act
Other professional licensure considered based on experience and training as outlined in the state licensure act
Valid driver's license and own reliable transportation required
A track record of successful experience in a health care setting, strong interpersonal skills, and experience working with physicians, nursing management, discharge planners, case managers, and social workers
Clinical experience is preferred
Approximate time of external liaisons to be in the market territory: 80%

Summary
Home Based Residential Supports direct support staff are responsible for providing high quality direct care services to an adult with developmental disabilities and high medical needs. Each direct support staff member is responsible for ensuring the human rights of all Individuals served are met. Staff are responsible for treating all Individuals with dignity and respect.
Direct Support staff assigned to the individual are responsible for supporting the Individual in the completion of their daily routines. Direct Support staff are responsible for ensuring Individuals are provided with opportunities to learn and exercise increasing independence as much as practicable.
Essential Job Functions
* Responsible for the health, safety and wellbeing of the Individual served, seeking assistance and medical interventions as appropriate.
* Serves as a Mandated Reporter and is required to advise direct supervisor, DDS and DPPC immediately if abuse and/or neglect of an Individual is suspected.
* Handle crisis situations according to agency policies and procedures, seeking resources when necessary.
* Monitor home site for safety, cleanliness and upkeep and take necessary action to ensure same. Report safety concerns per established protocols.
* Establish rapport with Individual participating in the Home-Based Residential Supports program.
* Support the learning of the Individual by use of motivational techniques, effective interventions and various communication approaches including verbal, gesture, and written communication.
* Assist the Individual with any activities of daily living (ADLs) as applicable to each Individual's abilities.
* May be responsible for scheduling and accompanying individuals to medical appointments/treatments, ensuring accurate reporting to physician and follow through with respect to doctor's orders and recommendations.
* Responsible for producing accurate, legible and complete medical documentation for Individuals served, including but not limited to treatment charts and data sheets.
* Ensure Individuals are provided nutritionally sound food options along with the education to help them understand the impact of their food consumption.
* Carry out all interventions in an Individual's Behavioral Protocol Plan as written and trained. Redirect and/or physically support Individuals who may be aggressive using proactive techniques while maintaining self-control.
* Communicate effectively, professionally and respectfully with Individuals, co-workers, management, guardians, and others while on company time.
* Responsible for being familiar with and abiding by Agency, Department, and Program policies and procedures and for acting in a manner consistent with organizational values, mission, and goals.
* Attend and actively participate in mandatory staff meetings and trainings as well as supervision meetings.
