Central Ohio Primary Care Physicians is seeking a Population Health Analyst located in Westerville, OH. The Population Health Analyst supports COPC's population health initiatives by analyzing and compiling data from multiple systems and designing the analysis in an efficient format. Responsibilities include working with data management teams to compile datasets from a variety of sources to provide a comprehensive picture of health risks and consults with Population Health Management leaders on areas of opportunity. The Population Health Analyst understands population health analytics, and an understanding of clinical and claims-based data systems and measures. This position is responsible for visibility on progress and results against value-based programming goals.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: *

* Develop dashboard or reoccurring reporting to measure & drive achievement of goals and highlight opportunities to pivot programs or goals.
* Collaborate with administrative teams to drive development of standardized datasets/additional tools/metrics on behalf of the Population Health Services.
* Develop approaches to presenting information including presentations and communications.
* Analyze program(s) data and trends to identify opportunities for process improvement, acting as a resource for both internal and external teams.
* Assist in design, implementation, and maintenance of standard internal and external (public) dashboards and population health.
* Support advance project management, improvement initiatives and tactical decisions/plans.
* Provide training and education to staff related to the data available, how to access it, and what metrics might be of interest.
* Identify opportunities for optimal clinical and utilization impact and work with programing leadership & teams to operationalize patient identification for enrollment in population health management programs.
* Support coordination of committee, workgroup and sub-group meetings including supporting agenda creation and presentation development. This includes coordinate of attendees and their contributions/deliverables for each meeting.

LifeBridge Health Medical Group Radiology faculty supports Sinai Hospital of Baltimore, Northwest Hospital, Levindale Hospital, and Grace Medical Center, a practice that encompasses over 1000 beds and over 200,000 imaging encounters per year. Over the next year we will be doubling the size of our radiology faculty.

The LifeBridge Department of Radiology offers a full spectrum of inpatient radiology services utilizing the most up-to-date techniques and state-of-the-art equipment. **Responsibilities:**

+ Technical responsibilities include maintaining a working knowledge of customers.
