Job Summary
The Administrative Assistant serves as a part of the Operations team in the MATCH Program. The MATCH Program works in conjunction with the Baltimore City Department of Social Services to ensure all children in foster care receive health services and ensure children's health needs are met.
Duties and Responsibilities
The primary responsibilities of this position are:
Administration
Provide support to the MATCH clinical team by requesting supporting documentation, screening incoming calls and entering notes in the MATCH Program's database.
Perform clerical duties for the MATCH management staff.
Calling provider offices to confirm appointments and request medical documentation.
Archive paper records and save them into program databases.
Responsible for retrieving medical records as necessary.
Daily review MATCH mailbox and address all email inquiries.
Schedule meetings and record minutes for meetings.
Daily answer MATCH phone line.
Assist with troubleshooting IT related issues.
Operations
Conduct safety checks twice a day when in office.
Take attendance of all staff
Responsible for carrying out the inventory of all office and facilities supplies
Contact Landlord for facility needs.
Reorder supplies as necessary.
Greet all visitors for the program
Data Entry
Maintain and update MATCH and funder program databases
Weekly, Monthly and Quarterly provides reports for MATCH program
Reconciliation of data for BCDSS and HCAM internal and external reports.
Utilize multiple databases to update the demographics of children in foster care and upload documentation containing personal identifiable information (PII).
Human Resources
Timely and accurately submit timesheets.
Daily complete Task Log
Complete all required HCAM and funder trainings.

QUALIFICATIONS:

The Safe Sleep coordinators provides linkages to family planning services, education, and counseling, as well as referrals to other resources such as WIC, Infants and Toddlers, and Home Visiting.
Duties and Responsibilities:
Case Management:
Receives, tracks, reviews, and documents case services referred Safe Sleep Clients.
Outreaches all postpartum clients within 48 hours of case receipt.
Outreaches all pregnant clients within 5 business days of case receipt.
Ensure 100% on time closure for Safe Sleep Outreach.
Provide education on Safe Sleep practices, available resources to clients with a demonstrated need.

Job Summary
The Care Coordination Program (CCP) at HealthCare Access Maryland (HCAM), is an outreach program that provides short-term care coordination services to Baltimore City Residents. The Care Coordination Program assists Medicaid/HealthChoice eligible beneficiaries in accessing, using, and maximizing their Medicaid benefits. The CCP multidisciplinary team educates clients about health benefits, link residents to primary care and specialty services, work to improve birth outcomes in Baltimore City with innovative prenatal outreach strategies and strive to protect our city's most vulnerable populations by providing access to health services and community resources.
Duties and Responsibilities
The primary responsibilities of this position are:
Administration
Work collaboratively with Emergency Room Staff and electronic CRISP alerts to identify and outreach pregnant/postpartum client for HealthChoice education, connection to prenatal/postpartum/family planning services and other community-based resources.
Timely and frequent communication with Deputy Director.
Care Coordination
Complete face-to-face/telephonic outreach services to provide linkage to Medicaid benefited services in primary care, dental care, specialty care, health screenings and vaccinations.
Complete Social Needs Screening and create an individualized care plan.
Triage and referral MCH clients to B'More for Healthy Babies Programs; WIC, Home Visiting, Parenting Support Groups, Doula Services and Safe Sleep Resources.
Aid with the Social Drivers of Health barriers to accessing healthcare including through referral to various community resources
Documentation
Within 30 days document assessments, care plans and submit referrals into the HCAM electronic database.
Create and send Service Response communications with Medicaid Providers, and Managed Care Organization departments and Hospital Partners.
Complete and submit monthly reports
Human Resources
Complete and submit timesheets timely and accurately.
Complete all required training courses.

QUALIFICATIONS:
Education, Experience, Certifications and Licensures:
High School Diploma or GED with five years of human services experience. Bachelor's degree in a Human Services field is preferred.
At least two years of experience providing Home/Community Outreach to the maternal and child health population in Baltimore City
At least two years' experience working with HealthChoice Medicaid recipients and Managed Care Organizations
Bilingual (Spanish/English) candidates are strongly preferred.
Maryland Community Health Worker Certification strongly preferred. Candidates will be required to complete this certification within one year of hire date.
Reliable transportation required.
KNOWLEDGE, SKILLS & ABILITIES
Ability to work independently and as part of a team to achieve project success in outreaching clients identified for care coordination
