Case managers provide goal-oriented and individualized support focusing on improved self-sufficiency for the persons served through needs assessments, planning, linkage, advocacy, coordination and monitoring activities.
ESSENTIAL FUNCTIONS:
Outreach to encourage the participation of the person served.
Assistance with achieving goals as defined by the persons served.
Optimize resources and opportunities through community linkages and enhance social supports
Provide transportation to and from appointments.
Provide case management, referral, and referent contact services to patients and documents same in-patient chart.
Coordinate with licensed discharge agencies to transition to recovery or permanent housing.
In calibration with the client devises and presents recommended service plans in weekly treatment plan review meetings.
Assists residents to achieve the goals of mutually agreed upon care plans for substance abuse rehabilitation service needs and aftercare.
Completes required forms pertaining to patient case management needs within the program.
Finds and develops for program use needed resources including visiting, inspecting agencies and attending network meetings.
Assists resident to link with needed family and community resources as coordinator and advocate.
Attend patient staffing and /or interventions.
Seeks appropriate supervision requiring clinical or programmatic decisions.
Performs other duties as assigned by supervisor.
Requirements:

Valid Illinois driver's license.

Provide direct psychiatry services to clients of Haymarket Center. Care includes, but is not limited to a comprehensive psychiatry history, exam, assessment and development of a treatment plan. Order and interpret laboratory tests and diagnostic studies as needed.
Prescribe or recommend pharmacologic and non-pharmacologic therapies including controlled substances as the law permits.
Document treatment services provided or recommended in the EHR.
Assess patient needs for additional internal (e.g., case management) and external services. Refer and assist patients to ensure access to additional supportive services.
Follow clinic protocols and procedures in providing and documenting services.
Participate in quality improvement initiatives, research projects, and continuing education activities to enhance clinical practice and patient outcomes.
Continually update own knowledge of Psychiatry and HIV/AIDS through reading, conference attendance, lectures and other educational opportunities.
Comply with agency Compliance Standards of Conduct and related policies and procedures.
May be responsible for the administrative management, supervision and collaboration of other medical partners.

Qualifications will include the following;
MD or DO degree from an accredited program.

Position will conduct outreach on the transit trains and buses with a team providing direct services to individuals experiencing homelessness, mental illness and substance use. Identify target populations for engagement in project services; conduct assessments, create service plans, engage and link clients with appropriate services in hopes of connecting homeless individuals to needed treatment and housing resources.
Essential Functions:
Conduct outreach on transit platform/trains and platforms to engage members of the target populations into services;
Conduct brief screenings and identify clients with substance use and/or mental health disorders in order to link them with appropriate resources;
Outreach worker will need to utilize motivational interviewing techniques to engage the client in an assessment in order to identify needs and link to services;
Collaborate to locate, create and maintain relationships with the homeless through outreach;
Provide outreach to clients by providing food, clothing, transportation, case management and referrals for housing and shelters among other needed services;
Completion of documentation in appropriate database in a timely manner;
Follow all safety protocols;

WORK DESCRIPTION:
To develop and maintain cordial relationships with officers and departments of the court, including private attorneys who represent clients of the court who are charged with offenses involving substance abuse. Provides individual services to adult, substance-using clients by following evidence-based practices; assists clients in building recovery resources; and develops individual Recovery Plans that help clients enhance self-efficacy, maintain engagement in ongoing HIV medical care, reduce HIV risk and transmission behaviors, and continue recovery post-treatment.
ESSENTIAL FUNCTIONS:
To serve as a liaison between Haymarket Center and assigned Circuit Court(s) of Venue.
Provide recovery support services at time of discharge from substance abuse treatment services.
Engage clients in motivational interviewing and motivational enhancement techniques
Work with clients to develop a Recovery Plan that addresses their comprehensive needs
Develop a clear understanding along with implementation of the Recovery Coach Model implement Contingency Management in accordance with project protocols.
Meet with clients at least once a week to ensure implementation of the Recovery Plan
Assist clients in accessing community resources to meet their additional needs
Link clients to recovery support services
Support clients to engage family members and significant others in recovery planning
Monitor client engagement in medical care
Collaborate with other service providers to ensure linkage to needed services
Provide medication adherence counseling
Maintain contact with clients post-treatment for at least one year
Document all contact with client and collateral contacts in a timely manner
Participate in Health & Wellness events
Assist in completion of program evaluation and reporting

This position is responsible for providing alcohol, drug and mental health evidence-based treatments to adults, including those with co-occurring disorders. Responsibilities include assessment, treatment planning, on-going individual and group therapy, case management, brief intervention and related documentation. This position works collaboratively with other providers daily. Duties will include:
Perform assessments to evaluate Mental Health & Substance Use Disorders;

Provide psycho-educational/didactic groups educating patients about various aspects of mental health and recovery;

The Co-Manager, Mac's Deli works to ensure high food quality, service delivery, and profitability, and help to create a diverse, professional, and inclusive learning environment for transitional workers who are clients of Haymarket Center programs. The Co-Manager will interview and hire new transitional worker staff. The role will help clients enrolled in the supported employment program living with substance use disorders and mental health conditions articulate their interests, values, and skills. The role will assist these clients with obtaining transferable skills and securing PT or FT competitive employment in the community. The role will report directly to the Manager, Supported Employment Center.
This is a full-time restaurant management and supported employment role within an urban addictions treatment center serving those with substance use disorders and mental health conditions.
Selected individual will;
Ensure compliance with company policies and procedures with respect to departmental operations, and quality & safe food handling.
Comply with state, federal and OSHA safety and sanitation regulations.
Supervises the day to day functions of the deli and monitors temperature of deli and bakery items.
Schedule service deli associates to ensure adequate coverage and service levels.
Provides training on department procedures, safe food handling and sanitation regulations.
Manages the daily activities of employees to improve sales performance, cooking, serving, packaging and operating counters.
Prepares work schedules, grows the food service program in the community and supports a team based environment.
Requirements:
Associate or Bachelor's degree in culinary arts, hospitality or business.
Minimum one year of managerial experience.
Must have a valid City of Chicago Food Service Sanitation Manager Certification.
Must have retail management experience in the food service industry.
Must be at least 18 years of age due to equipment use and work environment.

•Minimum of one year experience in the security related field and at least 20 hours of training in security or an equivalent combination of training and experience
