Job purpose The Medical Interpreter provides English/Spanish interpretation services to patients and staff to facilitate clear, accurate, and effective communication between the medical provider and the patient. This role ensures that patients with limited English proficiency are able to understand and fully participate in their care, supporting Heartland Community Health Center's mission to provide comprehensive, equitable, and compassionate health care to all. The Medical Interpreter serves as a linguistic and cultural bridge between patients and healthcare professionals, ensuring that communication is not only accurate but delivered with sensitivity, professionalism, and respect for confidentiality. Duties and Responsibilities
Participates in the Care Team by actively being involved in pre-visit planning, morning huddles, and patient follow-ups as required
Facilitating oral communication, through accurate and complete interpretation services, between Limited English Proficiency (LEP - patients, families and HCHC Staff
Renders all messages accurately and completely without adding, omitting, or substituting.
Advise parties that everything said will be interpreted
Assists patients by translating forms or making phone calls for referral specialists and/or other outside medical procedures
Provides access to medical providers and other clinical staff by answering Spanish language message line, interpreting these messages, and assigning telephone encounter to appropriate staff member
Reports test results to patients, both English speaking and Spanish speaking
Relays instructions from Provider on follow-ups to these tests
Assists patients to in-house Patient Assistance Program as needed
Assists patients in communicating with pharmacy regarding medications or refill related questions
Assists Nurse in tracking critical patients with abnormal test results and ensuring patients adhere to tracking needs
Scans patient documents into EMR
Provides/relays educational information to Diabetic patients regarding follow-up care and insulin use
Supplies patients with resource information regarding Diabetes
Assists in rooming patients as needed
Protects written patient information in his or her possession
Promotes direct communication among all parties in the encounters
Alerts all parties to any significant cultural misunderstandings that arise
Avoids sight translation, especially of complex or critical documents, if he or she lacks sight translation skills
Follows Corporate, Ethical, and HIPAA policies and procedures to ensure the confidentiality of all patient information
Effectively practices teamwork, respect for coworkers, good attendance, and punctuality.
Mentors/trains new hires as needed
Assists with translating paperwork into Spanish as needed
Performs other work as assigned or requested
Qualifications
Must be able to speak, read, and write English and Spanish fluently.
Associate's Degree preferred; equivalent experience in medical interpretation may be considered.
Knowledge of medical terminology is required.
Customer service-oriented with a strong patient-centered focus.
Demonstrates excellent organizational skills and attention to detail.
Strong interest in working collaboratively with an interdisciplinary healthcare team and culturally diverse patient populations.

Operate a scanner to obtain images used to diagnose and treat of pathologies.
Prepare and position patients and select anatomic and technical parameters accurately to produce cross-sectional images of the body.

Check patients in for dental visits:
perform needed x-rays

What you will do in this role:
You will provide case management services for both inpatient and observation patients.
You will work to identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another.
You perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family.
You evaluate the progression of care using evidence-based tools and approved criteria (InterQual).
You document professional recommendations, discharge plan, care coordination interventions, and case management activities to effectively communicate to all members of the health care team.
You act as a liaison through effective and professional communications between and with physicians, patient, family, hospital staff, and outside agencies.
You actively seek ways to control costs without compromising patient safety, quality of care or the services delivered.
You serve as an advocate for patient's rights, needs, and values and direct activities to identify and provide for the needs of the under-resourced patient population.
What you will need for this role:
Current Licensure in the State of Kansas as a Registered Nurse (RN) or Current Multi-State Licensure as a Registered Nurse (RN)
Current Basic Life Support (BLS) verification course through American Heart Assoc. and American Red Cross - Healthcare Provider CPR
3 years of experience in a clinical setting hospital nursing experience.
Bachelor's degree from an accredited institution is preferred
2 years of case management experience or 2 years of critical care experience is preferred
Certification in case management or utilization review is preferred
InterQual experience is preferred
